I would like to welcome all of you to the 1st Annual Quality & Safety Event 2015, hosted by Dar Al Shifa Hospital. The theme of this year’s event is “Safety Never Hurts;” a point which will be prominently emphasized within the presentations, booths and posters you will see throughout the day.
As healthcare professionals, it is our duty to ensure that safety is embedded in every layer of our organizational culture; in order to guarantee a safe environment for both patients and employees. I hope that this event will offer attendees a forum to meet, learn and share knowledge about safety improvement activities that they can incorporate into their own initiatives to improve health service delivery across the great State of Kuwait.

Infection Control as a Key Solution for Patient Safety
Worldwide, the delivery of healthcare is challenged by a wide range of safety problems. Healthcare associated infections (HAIs) are a continuing threat to patient safety and many HAIs were acknowledged to fall within the category of preventable medical errors.
In 2007, a set of nine Patient Safety Solutions was launched by the WHO with the aim to raise awareness of patient safety problems and serve as information tool to governments and health care organizations. Hand hygiene and single use of injection devices were two important infection control-related solutions.
In 2011, the WHO emphasized a single coordinated approach, pulling individual strands of work from healthcare professionals into a systems-based, team-dependent approach, to ensure patient safety learning is delivered in an integrated way. An evidence-based infection control program is a key to patient safety. Transform-ing healthcare quality includes prevention of HAIs as a priority issue for action to promote patient safety.

Ms. Tina Baramakian is currently the PC Administrator of Dar Al Shifa Hospital in Kuwait, and possesses more than 18 years of wide-ranging, progressive and profes-sional experience in healthcare. She holds a Master’s degree in Population and Health Demographics; is a Certified Professional for Healthcare Quality (CPHQ), and is currently completing her PhD in Health Management in UK.
Ms. Baramakian started her nursing career at the American University of Beirut; carrying forward this interest in her transition to Kuwait, where she has gained a reputation for enhancing operational performance in nursing and quality. She has also consulted several heath care facilities in the region in evaluating their respec-tive management structure, particularly in nursing.

Creating and Sustaining Patient Safety Culture
The challenge of making our health care system safe has been a high priority since the Institute of Medicine’s 1999 report on preventable medical errors and the extent of this problem.Creating and sustaining a culture of patient safety is a major undertaking for health-care leaders and providers. Developing the strategies and infrastructure necessary to effect real and lasting changes – in systems, processes and human behavior- takes a hard work and perseverance. In a culture of safety, people are not merely encouraged to work toward change; they identify areas for improvement, report and take action when it is needed.
Creating a safe culture doesn't happen overnight. But by adopting these strategies, health care leaders can replace entrenched habits that impede progress with transpar-ent processes. Dr. Rana Shams graduated with a Doctor of Pharmacy degree from the Lebanese American University (LAU) in 2002, accredited by the Accreditation Council for Pharmacy Education (ACPE).
She has 12 years of experience in the pharmacy practice field. Her initial employ-ment was at the American University of Beirut-Medical Center (AUB-MC) where her main focus was to broaden both her Pharmacy Clinical experience and Hospital Pharmacy practice. She has been residing in Kuwait since 2006; where she has played a pivotal role in commissioning clinical pharmacy practice within private sector hospitals.
Recently, Dr. Rana joined DASH as the Head of the Inpatient Medication Manage-ment Unit and in the process of developing and structuring Clinical Pharmacy practice within DASH.

Medication Safety
Medication Safety is part of the overall healthcare organization's quality & safety program. It is considered one of the major branches of safety that has a direct impact on patient quality of care in healthcare organizations.
Medications are not the responsibility of the medication experts only i.e. pharma-cists. The whole cycle of medication management starts with the patient's visit to the physicians, the involvement of the all clinical staff like nurses, and finally the direct partnership with patients/carers. This does not overlook the key role of the non-clinical staff in employing direct delivery of quality to patients.
Finally, promoting medication safety and quality in organizations usually requires an up down approach; where health service senior executives plan and review systems, and ensure clear communication of organizational and individual safety and quality accountability throughout the organization.
