WithaglobalFinancialandeconomiccrisistouchingvirtuallyeverycorneroftheworld,‘change’istheorderoftheday.Aperiodofintensivechangeisalsoinstorefor Wonca. In the last decade, Wonca has seen anunprecedentedmembershipgrowththatcurrentlybringstogether119organizationsfrommorethan100countries.Thisgrowth inmembershipcoincided inmostof thesecountrieswithhealthcaredeliveryandfinancingchangesenhancing primary care. This is, in itself, an objectivemeasureofWonca’sextraordinarysuccessasaneffectiveandincreasinglydiverse,globalandregionalacademic-politicalorganizationofprimarycareandfamilymedicine.Indeed,Woncahastraveledalongwayfromitsbirthin1972asa13-memberAnglo-Saxonorganization.Butnosuccess is without its shadows, and therein resides acompellingandurgentneedtoaddressWonca’sstructureandobjectivepotential tocontinuetoprovideeffectivegloballeadershipforprimarycare.

There are three aspects of change to address. Thefirst concerns the Wonca structure and organizationthat consists of a professional staff of three. Woncaowes a great deal of its success to the dedication ofthis threesome - Chief Executive Officer Dr.Alfred Loh,Administrative Manager Yvonne Chung and AccountsExecutive Gillian Tan – but what may have been anattractive‘leanandmean’humanresourcesstructureisbecominganincreasingsourceofvulnerability.Togetherwiththelimitedfinancialresources,thishumanresourcesshortagehampersWonca’spresenceandparticipationintheWorldstage.Earlier thisyear,under the leadershipof President-Elect Rich Roberts, the GROW project wasstarted to address this challenge. I refer you for moredetails to the CEO’s column in the February issue ofWoncaNews.

The second aspect of change is the need to bringintotheWoncaleadershipabetterrepresentationoftheglobal representation that isWonca’s strength. ‘Equity’,‘Gender’and‘region’arethekeywordsinthisrespect.CapitalizingontheHERstatementoftheWorkingPartyof Women and Family Medicine, the Wonca Executivehas prepared bylaw changes to be tabled at the 2010CancunCouncil,thatwilladdressthis.Tobeabletocareforthehealthofpeopleoftheworld, it isessential tobringtogethertheverybest inprimarycareandfamilymedicinethatWoncahastooffer.

The third aspect to be addressed captures the firsttwo challenges and has to do with how Wonca putsitsmission into concreteactions.Back in 1972, gettingprimarycaregoingwasageneralpointandthismadetheWoncaactions fairlyuniversal:advocatingprimarycare,developinganacademicinfrastructureofnationalCollegesand Academies, and building a teaching, training andresearchagenda.Thishasresultedinthrivingdevelopmentof familymedicine ina largenumberofcountries.Asaconsequence, there is an increasing variability, fromcountrytocountryandregion-to-region,ofwhatWoncahas to offer. Professional and patient advocacy andprimarycarepractice,researchandacademicinfrastructuredevelopmentarestilloftheutmostimportanceinmanypartsof theworld.Butat thesametime, theneedsofdevelopedprimarycaresystemsshouldnotbeignored.For too long,Woncahaspresented itsmodel toothersas ‘one size fits all’. It is time to diversify in order tobetruetoWonca’suniversalmissionofbetterhealthforpeoplethroughprimarycare.Withsomeurgency,Woncaand itsmemberorganizations should addresswhat arethe needs and opportunities under ‘developed’ primarycareconditions.

To where does change lead if we are to secure afutureinwhichpeople’shealthneedsaremetbyahealthcaresystemthatistimely,effective,efficientandsafe?Inmyview, ‘research’and ‘health informatics’present twoprioritiestoequipandstrengthenWoncaanditsmemberorganizationsforthejourneyahead.

TheWoncaCancun2010WorldCouncilwillbeatimelyand critical venue for these discussions. I am lookingforwardtoit.

In the past five years, it hasbeen my practice as the WoncaCEO to make visits to the varioushead-offices of the pharmaceuticalmultinational companies in Europeand the United States sometime inthefirstquarterofeachyear.

Thepurposesofthesevisitshavebeenprimarily:1. To consolidate and re-establish

goodworkingrelationshipswiththe various key personalities inthese companies and also ac-quaintmyselfwiththenewoffi-cialsappointedduringtheyear.

2. To acquaint these multinationalcompanieswiththevariousnewprogramsandcollaborationsthatWonca has with other medicalNon-GovernmentalOrganizations(NGOs) and the World HealthOrganization. To explore oppor-tunitiesforcollaborationbetweenourtwoorganizationstoensurethe success of these variousprograms.

3. TocreateawarenessofthevariousWoncaregionalandworldconfer-encestakingplaceintheyearortwo ahead and to help securesponsorshipsupportfortheseinconnection with the conferenceproper, scientific tracts, satellitesymposiaandsaleofexhibitionbooths.

4. To learn for myself the manyphysician and patient advocacyprogramsthatthesemultination-als introduce each year as partoftheircontributiontoimprovingglobalhealthandhealthcare.ToexplorehowWoncaastheglobalorganization representing familydoctorsmaybeinvolved.

5. To secure support for the con-tinuedrunningandmaintenance

of the Wonca Website ( www.GlobalfamilyDoctor.com)andthebimonthly publication of WoncaNewsbywayoftheWoncaGlobalSponsorship.Thisisaveryimpor-tant aspect of the annual visitsthatImake.

Over thesepastfiveyears sincethelaunchoftheWoncaWebsite,wehaveseenasignificantgrowthofthewebsite.Thelatestfiguresmonitoredonvisitstothesitehavealsoshownthat there has been gradual and adefinite increasealso in thespreadof countries whose family doctorsuse the website. In fact, it is noexaggerationtosaythattheWoncaWebsitehasnowbecomethe“face”of Wonca to the world. Plans areunderwaybytheWoncaPublicationsand Communications Committee tofurther improve thewebsiteand itscontents to increase its impactandreachtoglobalfamilydoctorsintheircontinuing professional education.Continued support from industryfor the website is vital if Wonca isto succeed in this endeavor, againunderscoringtheimportanceofthesevisits.

Besidesfamiliarizingindustrywiththevariousactivitiesandobjectivesof Wonca, it is equally importantthat Wonca gets to know howindustry in turn views the globalfamily medicine organization andits global mission and objective.It is also important that Woncaunderstands how industry thinks itcanworkwithourworldorganizationonprogramstobetterthehealthofthepeoplesoftheworld.TherearethosewhoonlyviewtherelationshipbetweenanymedicalNGOsandthepharmaceutical industry as a one-sided affair with industry havingmoretogainthantheNGO.Iwouldlike to differ from this view aftermy experiences of working withindustry for thesepastmanyyears.Some of the larger multinationalpharmaceuticalcompanieshavevery

goodandwellthoughtoutphysicianand patient advocacy initiatives orprograms consistent with Wonca’smissionandobjectivesandwithnoproductlinksorpromotionintended.Wonca seeks out these areas ofcollaborationwithindustry.

In summary, Wonca regularly

updates itself on the well-meaninginitiativesandprogramsof industrythat will benefit especially thepatients and to also learn howbest to collaborate with industryto achieve these common goals.It is equally important for Woncato understand how industry viewsWonca’sobjectivesandmissionanddesirestocollaboratewithWoncatoachievethem.

To this end, in this issue ofWonca News, we have started aseriesofinterviewswithkeypersonsin industry with whom Wonca hashad working relations. A fixed setof questions will be posed to eachof them and the answers will befeatured in upcoming issues ofWoncaNews.

Strengtheningrural communities in remote areasand small islands and empoweringimmigrants seem to be a highpriority and well fitted within thecore subjectsofa rural conference.The discussion about assets forhealthanddevelopmenthasrecentlyreceived prompt attention by theWorld Health Organization, whilecertain universities and researchinstitutes foster inter-professionalcollaborationanddialogue.

The conference’s theme is ofkey importance for communityleaders who could contribute inidentifying and strengtheningexisting health assets and explorepathways for maintenance andchange. The [financial] crisis is now truly global. It strikesatacriticaltimeforhealthinallpartsoftheworld. Inlow-income countries, the impact of the crisis is beingfeltthroughreduceddemandforexports,tighteraccessto capital, less foreign direct investment and fallingremittances.Consequentunemploymenttoooftencomeswith no protective safety net. As incomes fall, publicsector services become the more favoured source ofhealth careat thevery time thatgovernment revenuestofinancethemareundergreatestpressure.Informationisstillpatchy,butweknowthatatleastsevenministriesofhealthinAfrica-includingsomeofthepoorest-havealreadybeennotifiedthatthebudgetforhealthwillbecutastheresultofthecrisis.Othersanxiouslyawaitthenextbudgetcycle.

When local currencies are devalued the cost ofimportsrises.Essentiallifesavingmedicinesmaybecomeeitherunavailableorunaffordable.Weknow that costsof medicines rose in previous crises - we are alreadyseeing the same effect again as prices rise, not justinAfrica but in Europe and CentralAsia (up to 30%).The potential impact extends beyond the individualand family to societies as a whole. Governments havemadecommitmentstokeeppeoplelivingwithAIDSontreatment.Wehavetotakethestepsneededtoensurethat these promises can be upheld. Drug prices arerisinginsomeofthecountriesaffectedbydrug-resistantTB. Failure to contain this threat to public health hasconsequenceswellbeyondnationalborders.

Manyhigh-incomecountrieswithageingpopulationshave been positioning themselves for anticipatedincreases inspendingonhealthandpensions.Severalareintheprocessofundertakingcomplexandpoliticallychallengingreforms.Wemustbeconcernedwhenweseeevidencethatplanstosetasideresourcesandcreatethefiscal space to address the future health needsof theelderlyarebeingshelvedasthecrisisdeepens.

Healthisaglobalconcern.Itisavitalinvestmentineconomicdevelopmentandpovertyreduction.ItiscentraltotheachievementoftheMillenniumDevelopmentGoals.Accesstohealthcareisafundamentalentitlementandresponsibilityofgovernments theworldover.Reducingexclusion, extending universal benefits and protectingpeople from impoverishment are common elements ofagrowingnumberofnationalhealthpolicies.Managingexpectationsandcontainingspiralinghealthcarecostsiscriticaltothecontinuingsolvencyofmanyeconomiesin the industrializedworld.Maintaining the integrityofsystemsthatprotectusfromcross-borderhealththreats,that detect and respond to outbreaks, pandemics andemergingdiseasesisofconcerntoallnations.Progressinonedirectiondependsonalltheothers.Wecompromiseonanyoneoftheseelementsatourcollectiveperil.

Aglobalcrisisrequiresglobalsolidarityandactions.Maintaininglevelsofhealthandothersocialexpendituresis critical to protect life and livelihood and to boostproductivity.

Theimpactofthecrisiswillvarycountrybycountry,but to sustain levels of health there is a growingconsensusastowhatneedstobedone.Weneedgoodquality real-time information to guide the response;weneedtobeabletoidentifygroupsmostatrisk;toensure that safety net programmes are well targetedso they reach the most needy; to seek efficiencies inspendingwherepossible;torecognizethatcrisesoftenoffer opportunities for reform; to sustain spending onprevention(whichisoftenthefirstcasualtyofspendingcuts);andwhereexternalaidisrequiredtoensureitisaseffectiveaspossible.

Peoplearetheultimatetargetofeconomicrecovery.WHO’sconcernispeople’shealth,buthealthisdependenton many factors: employment, shelter, nutrition,education. In some countries, economic stimuluspackagestargetpeople’shealthdirectly(throughreducinghealth insurancepayments,orbuilding lessnumberofclinics). But a well-planned infrastructure programmewillhavemultiplebenefits: rural roads increaseaccesstomarkets,boostfarmers’income,andreducematernalmortality.Assistancetomicrofinanceschemeshelpskeepchildren inschools,empowerswomen,andboosts the

Hewongreatadmirationandrespectfromhispeerswhenheopenedin1969a2-yearresidencyprogramattheJewishGeneralHospital(JGH)inMontrealandbaseduponthestandardsoftheCollegeofFamilyPhysiciansofCanada.HelaterbecameChlefoftheJGH’sHerzlFamilyPractice Center. TheInternationalClassificationofPrimaryCare(ICPC),developedbytheICPCWorkingParty,brokenewgroundin theworldof classificationwhen itwaspublished in1987byWONCA.Forthefirsttime,familydoctorsandotherhealthcareproviderscouldclassify,usingasingleclassification, three important elements of the healthcareencounter; reasons forencounter (RFE),diagnosesorproblems,andprocessofcare.Problemorientationofthemedicalrecordandlinkageofencountersovertimepermitsclassificationoftheepisodefromthebeginningwith an RFE to its conclusion with a more definedproblem,diagnosis,ordisease.

ThefirstversionofICPCpublishedin1987isreferredtoas ICPC-1.Theversion in the 1993publication “TheInternational Classification of Primary Care in theEuropean Community: With a Multi-Language Layer” isknownas ICPC-E.The 1998version2 is referred to asICPC-2. ICPC-2-E refers to a revised electronic versionreleased in2000.Subsequent revisionsof ICPC-2-Earealso labelled with a release date. ICPC is used whenreferringtothegenericclassification.

In 2003, theWorld Health Organization recognisedICPC-2asaWHOrelatedclassificationfortherecordingof data in primary care. The Wonca InternationalClassification Committee (WICC) has been active since1972 and currently has 40 members from 23 differentcountries.

Thenon-commercialuserisfreetouseICPC-2-E,butthe copyright remains with WONCA, and all rights arereservedandthesameconditionsapplyasapplytotheICPC-2book.

ICPC has been increasingly used for reportingthe activity of family doctors and other primary carepractitioners at both national and international level.

TheInternationalFederationofPrimaryCareResearchNetworks (IFPCRN) was organized under the WoncaCommittee on Research at the May 2001 meeting inDurban, South Africa. Nineteen individuals from sevencountriesparticipated.ChrisvanWeeloffered,onbehalfof the Wonca Committee on Research, to sponsor thelistserverandwebsiteforthenewIFPCRN.

ThemissionoftheIFPCRNistowillsupportresearchforprimaryhealthcareforthebenefitofpatientsbythemutual support of research networks, the exchange ofideas and methodologies, advocacy for the quality ofresearch in primary care, capacity building, policy andadvocacyinitiatives,anddoingcollaborativeresearch.

JohnBeasleyfromtheUSwasselectedasinterimchairandHelenSmith(ChairoftheUKFPCRN)asinterimco-chair.FranciscoGomezClavelinafromMexicovolunteeredaseditorofthenewsletterandthewebsite.

Membership from individuals as well as fromnetworks is being solicited at this time. At a small,informalmeetingofsomeIFPCRNmembersatthe2001NAPCRGmeetinginHalifaxsomepreliminarycriteriaforinternational projects were established: an energeticand committed PI and support by clinicians in variouscountries;thequestiontobeansweredisofimportanceand interest to local clinicians; and clear evidence ofwhyaninternationalapproachisneededtoanswerthequestion.

The IFPCRN members at the Halifax meeting alsoaddedtothelistofpotentialprojectsfirstarticulatedinDurbanand identifiedseveralpotentialPI’s.The listofpotentialprojectsnowincludes:theimpactofHIV/AIDSinprimarycare;adayinthelifeofafamilydoctor;howthestigmaofmentalillnessimpactscare;studyofpublication

bias and impact of having a euro-centric literature;management of smoking; access to EBM and otherinformation sources in different countries and differentareas;definitionofalcoholabuseindifferentcountries;contrastingcarepathways(e.g.diabetes,maternitycare);patient expectations of care for pregnancy; perceivedqualityof life indifferentcountries;andtheroleofFP/GP’sinimmunizationprograms.

Twopromising initialstudiesamongtheareas listedabove are: a study of what family doctors do whenconfrontedwithpatientswithHIV/AIDS,andastudyofthebarrierstoimmunizationsandhowfamilydoctorscanbetterassistwithimmunizingpopulations.

MembershipintheIFPCRNisbeingdevelopedrapidly,andnowincludesrepresentativesofnetworksorplannednetworks in 13 countries (Australia, Austria, Canada,Kenya, Korea, Mexico, Malaysia, Netherlands, Pakistan,PeoplesRepublicofChina,SouthAfrica,UK,USA).
