The 28% rate for Federal backup withholding was recently extendeduntil December 31, 2012. To avoid the need to continually monitor forrevisions to the NADOA Model Form Division Order, the NADOA Boardhas voted to alter the language on the bottom of the Model Form.Please review your company’s adaptation of the Model Form forinclusion of altered language. The newly revised Model Form isavailable on the NADOA website (www.nadoa.org).

Rate/Percentage is 28%, effective for all subject payments afterDecember 31, 2002 until December 31, 2012. The rate afterDecember 31, 2012 will be determined at a later date.
