a) CDEC MVC update on construction will be presented as an information item ATI completing all damaged drywall and everything is progressing as quickly as possible. Tina Elm spoke about new fire alarm system, new conduit and new vaults and will keep group posted as progress is made with campus construction.

BIT Tune up refresher took place yesterday. b) Meeting reminder will be presented as an information item

BIT Meeting coming up next week, Executive Director of NABITA, nationwide guru will present many items for CARES Team and will include the written word and how it relates to violence along with exact ways to evaluate.

a) Risk Management Introduced Lura Castillo new Risk Management team member b) Phone system changes - Marcus provided updates on voicemail and phone system upgrades, calling out number being switched to 8 and phones will be deployed to classrooms by end of semester. c) Update on water leak in CDEC - MVC Water leak case is now resolved and CDEC is up and running. d) SWACC Property & Liability re-inspection scheduled for 10/26 e) Service Animal workshop recap well attended workshop on both campuses with guest/student speaker at each campus. Next workshop will take place in Spring semester. AP is going to be updated to include assignments for students who are unable to participate due to presence of service animals and the potential of an unsafe condition (in science related spaces)

Fred & Steve went to Sacramento to present updated Clery incident in our restroom at MVC, received excellent remarks on case was handled. The Great Shakeout took place and the district had approximately 30 volunteers with over 250 participants this year.

VIII. M & O Facilities Update (Twitty/Wilkes)

The Unsafe Work Conditions form was presented for discussion along with how it coordinates with the work order system. Form can be located on the Facilities/planning homepage. The question was posed for expanding the work order system beyond supervisors and administrators, at this time the page is not ready to expand. For an actual emergency on campus, dial X 5188 or 911 when applicable. Tine will meet with BOTs on safety recommendations for prioritization.

Tina reported on Fiber/Vector project, alerting campus community they will be onsite. SJC solar project is starting, there will be lots of work trucks and workers.

I. INTRODUCTION

On October 26, 2016, Keenan & Associates conducted a Property & Liability Inspection Follow-up Audit for the Mt. San Jacinto Community College District located in San Jacinto, California. The purpose of the Inspection Follow-up Audit is to review all of the priority recommendations listed in the previous SWACC Property & Liability Inspection and to note any progress made in addressing or correcting those priority issues.

Kendall Evans, Consultant, conducted the inspection, and Carol Lynch, Sr. Project Coordinator, processed the Follow-up Audit.

The goal of Keenans inspection services is to (i) assist the client in evaluating its self-audit procedures, (ii) facilitate the identification of conditions which may pose a risk of injury and/or property damage, and (iii) provide recommendations and/or suggestions to help mitigate the risks identified. Keenan does not guarantee that it has identified every potentially hazardous condition existing on the Clients premises. Similarly, it does not promise that following the recommendations contained in this report will eliminate all risk of injury or property damage.

Keenans services are not a substitute for regular, ongoing inspection and maintenance, or any required service that is to be performed by a licensed or certified service professional (e.g., HVAC contractor, electrician, plumber, elevator service personnel, etc.). Please keep in mind that changing circumstances can affect your risk exposures and periodic reassessments of your operational environment are recommended.

IV. IMMEDIATE/HIGH PRIORITY RECOMMENDATIONS AUDITED

INITIAL FINDING Location: Mt San Jacinto Community College - MENIFEE VALLEY CAMPUS - 600 Ceramics Priority: High Description: This type of power strip is not made for high wattage appliances. Fires and electrical shock may be caused by an extension cord or power strip that is not properly grounded. The other issue is that the power strip is not splash proof. Reduce the use of extension cords and power strips by replacing them with hardwired ground fault circuit interrupter (GFCI) protected outlets whenever possible. When an extension cord/power strip must be used, purchase the type with a GFCI built into it.

INITIAL FINDING Location: Mt San Jacinto Community College - MENIFEE VALLEY CAMPUS - 700 CDC - Laundry Room Priority: High Description: Hazardous chemicals and cleaning products should not be stored in areas that are accessible to children. The door to the laundry room was unlocked and the detergent container was left open.

AUDIT FINDING Status: CLOSED Close Note: The chemicals in the Child Development Center are secured and they added signage stating that the door is to remain "Closed & Locked."

INITIAL FINDING Location: Mt San Jacinto Community College - MENIFEE VALLEY CAMPUS - 700 CDC - Playgrounds Priority: High Description: Broken down, compacted or displaced loose fill surfacing material presents an elevated risk of head impact injuries. Adequate loose fill surfacing material depth should be determined based on the fall height of adjacent play equipment. Surfacing material should be added or re-distributed in this area to improve conditions and reduce this risk of head impact injuries.

AUDIT FINDING Status: CLOSED Close Note: Additional sand was added and is raked on a regular basis eliminating the elevated risk of head impact injuries. Close Date: Wed Oct 26 2016

INITIAL FINDING Location: Mt San Jacinto Community College - MENIFEE VALLEY CAMPUS - 800 LRC - Learning/Tutoring Center Priority: High Description: 1. Some of the electrical boxes on the floor are missing covers. The ones with covers create a change in elevation and are a bit loose to walk on. The box covers present a trip hazard especially if they are within the walking pathways. It is recommended to relocate or permanently cover the outlet boxes so that walking pathways are free of trip hazards. 2. It is reported that students often use the electrical boxes to plug

in their laptops, which may explain why some of the covers for the boxes are removed. The other issue is that the computer cords underneath the workstations can be damaged when students' feet or chairs continually get caught in them. Adding tabletop outlets will help keep cables off the floor and prevent them from being damaged.

AUDIT FINDING Status: CLOSED Close Note: The outlet boxes that were in the walkway were permanently covered eliminating the trip hazard. Close Date: Wed Oct 26 2016

INITIAL FINDING Location: Mt San Jacinto Community College - MENIFEE VALLEY CAMPUS - Campus Pathways Priority: High Description: Raised sections and potholes in sidewalks present a trip hazard. Such trip hazards should be ground down or filled. Note: The small pothole was found next to Building 200 and the elevated concrete was found between Buildings 600 and 700.

AUDIT FINDING Status: CLOSED Close Note: The elevated concrete was removed and replaced with grass, eliminating the trip hazard that it presented. It is no longer a walkway.

INITIAL FINDING Location: Mt San Jacinto Community College - SAN JACINTO CAMPUS - 1200 Building - Rm 1255A Office Priority: High Description: 1. Housekeeping should be improved in this area. Aisles and passageways should be kept clear at all times. It is recommended that lunch breaks are taken in appropriate lunch areas and not at workplace stations. 2. There is a potential for unsecured and unstable filing cabinets/bookshelves to topple over and injure students/staff. All filing cabinets/bookshelves should be secured to the walls or fixed firmly to the ground. 3. Heavy items stored on top of cabinets or overhead shelves present an elevated risk of struck-by

injuries. They can also block emergency egress if they fall to the floor. Heavy items should be stored at a lower level to reduce this hazard. Other items stored overhead should be secured in a manner that prevents them from falling. 4. Electrical Hazard - The microwave is plugged into a power strip. High wattage appliances need to be plugged directly into an outlet. 5. Housekeeping should be improved to allow space for the items stored on top of cabinets/overhead shelves and to reduce the number of cabinets in the room. This will make it easier for Facilities to secure all filing cabinets to the walls.

AUDIT FINDING Status: CLOSED Close Note: The housekeeping has improved in this area. At the time of the audit, the teacher was still removing excess materials from this area. The District secured all cabinets and bookcases to the wall. Heavy items were no longer stored overhead eliminating the struck-by hazards they present. Close Date: Fri Oct 28 2016

INITIAL FINDING Location: Mt San Jacinto Community College - SAN JACINTO CAMPUS - 1250 Building - Rm 1258A - Staircase Lobby Priority: High Description: The placement of the fish/turtle containers and plants adjacent to the power supply is a potential electrocution hazard due to the exposed electrical cords and outlet. It is best that these items be moved to another area. Another suggestion is to re-route the electrical wiring to a nearby wall and connect the outlet to a ground fault circuit interrupter (GFCI).

INITIAL FINDING Location: Mt San Jacinto Community College - SAN JACINTO CAMPUS - 1408 Ceramics Lab Priority: High Description: Portable emergency eyewash stations should be inspected monthly. Solution bottles should be replaced before the expiration date or if they have been used. There is no eyewash station in this room, which is highly recommended for industrial arts courses.

INITIAL FINDING Location: Mt San Jacinto Community College - SAN JACINTO CAMPUS - 1408 Ceramics Lab Priority: High Description: Combustible material should be removed from the area around the kilns. It is unclear whether the kilns are in use or not, but a minimum clearance of 36" should be maintained around each kiln.

AUDIT FINDING Status: CLOSED Close Note: The combustible materials around the kiln were removed and the area was clear of combustible materials at the time of the audit. Close Date: Fri Oct 28 2016

INITIAL FINDING Location: Mt San Jacinto Community College - SAN JACINTO CAMPUS - 700 Facilities Priority: High Description: All visible wiring should be properly insulated and in good condition. Cords with damaged insulation were observed at several golf cart charging stations.

AUDIT FINDING Status: CLOSED Close Note: The cords with damaged insulation have been removed from the site and replaced with new charges. At the time of the follow-up audit, the new golf cart charger was missing the ground. The District had it replaced immediately showing a proactive approach to safety.

INITIAL FINDING Location: Mt San Jacinto Community College - SAN JACINTO CAMPUS - Baseball Field Priority: High Description: 1. There are holes in the carpet mats, which presents an elevated trip hazard. 2. The gym equipment has been damaged from weather and prolonged use. It is reported the equipment is still being used by students. Gym equipment should be inspected regularly to ensure that the parts and cables are in good working order. Equipment with worn or frayed parts and cables should be removed from service until it can be repaired or replaced.

AUDIT FINDING Status: IN PROGRESS Action Notes: The gym equipment was removed from this area and this area is going to be under construction soon due to the District building a new baseball field.

INITIAL FINDING Location: Mt San Jacinto Community College - SAN JACINTO CAMPUS - Library - LRC Priority: High Description: 1. Suitable signage still needs to be posted to identify all routes of evacuation. 2. Exits and emergency evacuation routes should be kept clear of obstructions at all times in order to ensure safe egress. Exit and emergency evacuation routes shall be no less than 30" wide at any point.

AUDIT FINDING Status: IN PROGRESS Action Notes: Signs have been added, but will be moved so that they are visible from the large room. The objects that were obstructing the exit were removed and clear access was observed at the time of the audit.

INITIAL FINDING Location: Mt San Jacinto Community College - SAN JACINTO CAMPUS - Theater 1520 Scene Shop Priority: High Description: Damaged cord insulation presents an elevated risk of fire and electrocution. This particular cord needs a strain relief. Damaged cords should be removed from service to reduce this risk.

AUDIT FINDING Status: IN PROGRESS Action Notes: The cord's insulation still appears to be damaged; however, the District is going to make sure that the cord insulation is intact and that the appropriate strain relief is provided for the cord in this area. It was noted that the housekeeping in this area has improved significantly.

INITIAL FINDING Location: Mt San Jacinto Community College - SAN JACINTO CAMPUS - Theater Costume Storeroom Priority: High Description: Due to the lack of space, the District has had to utilize high level storage in the costume storage room. Vertical ladders are used to access these two platforms or mezzanines, which raises the potential for a fall hazard. Stage contractors and students use this room and there is a potential for items to be carried up as individuals climb the ladders (despite there being a pulley system). Consideration needs to be taken in either moving the storage room to a safer location, or providing safer access to these two high level platforms. The District is looking into creating a safer access without having to make major structural changes or a change of location.

AUDIT FINDING Status: IN PROGRESS Action Notes: The District is looking into different options to correct this storage and safety issue. They are putting a plan in place and are going to implement it soon.

INITIAL FINDING Location: Mt San Jacinto Community College - SAN JACINTO CAMPUS - Theater Stage 1500 Priority: High Description: Access to the electrical panel raises the potential for falling from height. The District should consider placing a guard on the ladder to prevent unauthorized use by students and redesign the ladder to provide safer access for employees.

AUDIT FINDING Status: IN PROGRESS Action Notes: They are working on getting a new ladder with a ladder guard and handrails already in place.

V. ADDITIONAL COMMENTS

The Biology Prep Laboratory, Room 1258 at the San Jacinto Campus, had housekeeping issues at the time of the SWACC follow up audit. Housekeeping should be improved in this area. The aisles and passageways should be clear at all times. The Menifee Valley Campus Student Enrollment Office is still working on addressing the concerns with students' personal information being overheard during the enrollment process. The clerks have difficulty hearing the information through the small glass opening at the counter. Consider adding Wait Here signage with line guiding ropes to direct people and have them wait in an appropriate area until it is their turn to give their information. Or mats can be used that have a line that states Wait Here. Senior Loss Control Consultant, Dennis Burgess, did a hazard assessment in this area and I agree with his findings to consider adding an audio telephonic type handset with a small keyboard on the students' side of the counter where they can enter their personal information.

VI. PROPERTY & LIABILITY LOSS ANALYSIS

Mt. San Jacinto Community College District has had six property losses in the last three years. One claim occurred in the 2014 fiscal year which involved severe wind damage for a total incurred of $17, 010.00. One claim occurred in 2015 that had to do with a power outage for a total incurred of $20,000.00. Four claims occurred in 2016. Three of the claims were theft related for total incurred of $80,000.00; one claim was due to a pipe breaking that caused water damage to the Child Development Education Center for a total incurred of $357,500.00.

On the liability side, there have been eleven claims filed in the last three years with a total incurred of $601,770.85. There were six claims that occurred in 2014-15 for a total incurred of $383,120.85; one auto liability claim for $1,974.04 to date; one claim of injunctive relief for a total incurred of $70,000.00; two employment practice liability claims for a total incurred of $166,200.00; one claim involved a student who allegedly fell off a chair in the cafeteria for $51,446.81; one claimant allegedly tripped on an uneven sidewalk sustaining an injury for a total incurred of $93,500.00. Three claims occurred in the 2015-16 fiscal year for $196,000.00; one claim of injunctive relief for total incurred of $10,000.00; one claim of dangerous premises for a total incurred of $11,000.00; one claim where a three year old minor was riding a scooter - the scooter tipped and the child sustained significant injury to their little finger for a total incurred of $175,000.00. There were two claims in 2016-17 for a total incurred of $22,650.00; one auto liability claim for $10,150.00; one claim of error and omission for a total incurred of $12,500.00.

VII. SUMMARY

In summary, sixteen of the sixteen priority recommendations listed in the Property & Liability Inspection have been addressed or a plan is in place by the District.
