Completing Health Plan Enrollment Form Form must be returned within 30 days or client will be
enrolled in a default plan Amount of paperwork from health plans can be
overwhelming for clients Important to explain that MA and MNCare plans have
basic services that must be covered- coverage very similar from plan to plan.
