In its USFIJCC Transformation Order and subsequent Orders, the Federal
Communications Commission ("FCC" or "Commission") requires Eligible
Telecommunications Carriers ("ETCs") to submit a five-year build-out plan in a manner
consistent with Section 54.202(a)(l)(ii) of the Commission's Rules by July 1, 2014 and
to submit annual progress reports thereafter.1 Section 54.202(a) (1) (ii) states in part that
ETCs are to [s]ubmit a five-year plan that describes with specificity proposed
improvements or upgrades to the [ETC's] network throughout its proposed service area

In its March 5, 2013 Order, the FCC specified that for rate-of-return carriers, the
five-year plans "should describe the carrier's network improvement plan, which should
provide greater visibility into current plans to extend broadband service to unserved
locations in rate-of-return service territories."3 Golden West Telecommunications

five-year network improvement plan for study area 391659.
L The Challenges Faced by the Company in Providing Voice and Broadband to its Rural Service Area
A. Description of the Company and its Service Area
Golden West Telecommunications provides advanced telecommunication services
across a large portion of South Dakota. Golden West's service area spans over 24,500
square miles, or nearly one-third of South Dakota's total land mass. Golden West's
service area is sparsely populated, with an average customer density of less than 1.5
customers per square mile. Golden West provides service to over fifty small
communities across South Dakota, including portions of five tribal reservations, with the
population of the largest community in Golden West's service area being nearly 4,000
residents. Golden West service area includes farm and ranch lands, as well as areas with
significant rock and/or forest land. Golden West also works with a variety federal, state
and local governmental entities in order to secure right-of-way for the placement of
telecommunications facilities, including national parks, national forests, national
grasslands, tribal lands and state parks as examples.
B. The Exchanges Contained Within the Company's Study Area

Il. The Company Has Used and Will Use Universal Service Support Only For the Intended Purposes
Section 254( e) of the Communications Act of 1934, as amended requires ETCs to
use Universal Service support ("USF") "only for the provision, maintenance, and
upgrading of facilities and services for which the support is intended."4 Pursuant to
Section 54.314 of the FCC's rules, in order for state-designated ETCs to receive USF for
the coming year, states must annually file certifications by October 1 stating that all
federal high-cost support provided to such carriers within the state "was used in the
preceding calendar year and will be used in the coming calendar year only for the
provision, maintenance, and upgrading of facilities and services for which the support is
intended."5 ETCs not designated by a state must file similar certifications with the FCC.6
In its USFIICC Transformation Order, the FCC clarified that prior to making the
Section 254(e) certifications, states should conduct a "rigorous examination of the factual

information" contained in the annual Section 54.313 reports, of which the five year
network improvement plan and annual progress reports are a part, in determining whether
they can certify that carriers' support has been used and will be used only for the purpose
for which the support was intended. 7 The FCC said that it would also use the reports to
verify certifications filed by ETCs that are not state-designated. 8 In this context, the
Commission stated, "[i]n light of the public interest obligations we adopt in this Order, a
key component of this [Section 254( e)] certification will now be that support is being
used to maintain and extend modern networks capable of providing voice and broadband
service."9
Essentially, under the existing rules and processes, the federal USF received by
the Company and other incumbent rural telephone companies are, in fact, an integral part
of the recovery of expenditures of rural incumbent local exchange carriers incurred in the
provision, maintenance and upgrading of their provision of facilities and services for
which the USF is intended. Golden West Telecommunications Cooperative, Inc. depends
upon its receipt and utilization of federal universal service support to provide rural
telephone customers with affordable and quality voice and broadband services.
Accordingly, given the critical role the network improvement plan and the
progress reports will have in the annual Section 254(e) certification process, Golden West
Telecommunications Cooperative, Inc.'s plan and progress reports will demonstrate not
only how the Company has used and will use USF not only for improvements and

upgrades, but also for the provision and maintenance of the facilities and services to
which the support was intended.
ID. The Company's Five-Year Network Improvement Plan
When the Commission adopted its five-year plan requirements for FCC-
designated ETCs in its 2005 ETC Order, it set forth the following criteria as to how the
ETC is to describe with "specificity" the proposed improvements or upgrades to the
ETC's network throughout its service area:
(1) how signal quality, coverage, or capacity will improve due to the receipt of high-cost support throughout the area for which the ETC seeks designation; (2) the projected start date and completion date for each improvement and the estimated amount of investment for each project that is funded by high-cost support; (3) the specific geographic areas where the improvements will be made; and (4) the estimated population that will be served as a result of the improvements.10
In that order, the FCC clarified that service quality improvements in the five-year plan
"do not necessarily require additional construction of network facilities."11 Accordingly,
the improvements listed in the plan may be projects related to the expansion of the
network (one or multiple services), projects related to updating technology to
accommodate new services or higher bandwidth or maintenance projects, such as to
reduce trouble reports or replace outdated equipment. Additionally, in some cases, the
projects may be ones that improve or upgrade the entire network rather than discrete

areas within a study area or they may be ones that are ongoing projects that have no
specific start and completion dates.
The instructions to the Form 481 state "[r]ecipients may describe where
improvements are expected to occur by wire center or census block, as appropriate. To
the extent no improvements are planned in specific areas, the five-year plan should so
indicate."12 The instructions also require that in subsequent annual progress reports
which must include the total amount of universal support received must provide this
information "broken out separately by the amount spent on capital expenses and the
amount spent on operating expenses."13 Accordingly, the Company's five-year plan
separately provides both capital expenditures and operating expenses.
A. The Company's Major Network Improvement Projects
Based upon this framework, Appendix A reflects Golden West
Telecommunications Cooperative, Inc.'s major network improvement projects for the
five calendar years 2015 through 201914 along with the start and completion dates, capital
costs, areas and population associated with those projects.
B. How These Projects Will Improve the Network
As detailed in Appendix A, Golden West Telecommunications Cooperative, Inc.
anticipates investing••••lfor network improvements in study area 391659 during
calendar years 2015 -2019. These investments will fund fiber-to-the-home (FTTH),

transport network, and subscriber carrier facilities upgrades, a road move project,
network maintenance and repairs, switch licensing, and general support items.
Major projects planned for 2015 include FTTH builds, upgrades to the transport
network and subscriber carrier facilities, and a road move project.
Improvements planned for the transport network include MPLS (Multiprotocol
Label Switching) Ethernet deployment, upgrades to interexchange transport,
interexchange ring facilities, and the packet core, and spare equipment. These upgrades
will affect the entire service area. Upgrades to the Ethernet network to support MPLS for
the delivery of bandwidth and services will enable the Company to scale network
capacity at a granular level while providing a more reliable method for managing IP
traffic. Investments in the interexchange ring facilities wiH upgrade the SONET and IP
ring facilities that connect the central offices together, enabling the delivery of more
network capacity to each ring, while the upgrades to interexchange transport equipment
will enable the delivery of more bandwidth and services from the rings to each central
office. Upgrades to the packet core will update the legacy IP routing infrastructure to
support more capacity and reliability.

Other investments projected for calendar year 2015 will be directed toward
maintaining the existing network, providing for service to new subscribers, infonnation
technology services, and switch licensing .

transport network that will support increased bandwidth to keep up with consumer

demand. The projections for these years are based on both actual costs from recent years
and current year estimates.
The investments shown for "Other Cable and Wire Additions" will be used to
bring broadband and/or voice services to new subscribers and provide for cable
maintenance and repairs across the entire service area.
Those investments detailed for "Central Office Transmission" and "Central Office
Subscriber Carrier Maintenance and Upgrades" are intended for maintenance and repairs
for the existing transport and subscriber carrier facilities throughout the service area.
They will also be used toward the purchase of new equipment to accommodate new
subscriber growth and increased bandwidth as consumer demand requires. These
investments will improve the quality of both voice and broadband services.
The investments listed for "Building and Maintenance Upgrades'', "Vehicle and
Work Equipment Replacement", and "Information Technology Services" are intended for
the entire service area and will allow for general maintenance and upgrades to central
offices, the purchase of outside plant equipment essential for .the day to day upkeep of the
network, and computers and other telecommunications equipment necessary for the
business operations of the Company.
C. Part 32 and Yearly Breakdown of Estimated Capital Expenditures Associated with the Projects and Operating Expenses
Appendix B included herein specifies the projects listed in Appendix A in terms
of the Part 32 capital expenditures categories and in terms of the years in which the costs
are expected to occur along with depreciation expense associated with the capital

Description of Tribal Engagement
Golden West Telecommunications Cooperative, Inc. (hereinafter "the Company"), study area
391659, provides services to small areas on the Cheyenne River Sioux Reservation and Rosebud
Sioux Reservation and the majority of the Pine Ridge Reservation.
On September 26, 2013, by letter sent via certified mail to the Tribal Chairman, the Company
requested a joint meeting with the Cheyenne River Sioux Tribe (CRST) to exchange information
and discuss issues related to the deployment and provisioning of communications services on
Tribal lands. The Company did not receive a response to the first letter; therefore, a follow up
letter was sent via certified mail to the Tribal Chairman on November 21, 2013. The Company
received a call from the general manager of CRST Telephone Authority on December 13, 2013
to discuss the meeting request. As requested during the discussion, the Company emailed to
the general manager of CRST Telephone Authority an exchange boundary map. A response was
received thanking the Company for the map and stating that CRST would be in contact with the
Company at a later date if they felt a meeting was necessary.
Staff members from Golden West Telecommunications engaged with representatives of the
Rosebud Sioux Tribe (RST) on several occasions during 2013 to address issues related to the
deployment and provisioning of communications services on the Rosebud Sioux Reservation.
The RST Utility Commission's (RSTUC) Public Utility Forum on January 24, 2013 was attended by
Golden West's General Manager/CEO, Director of Member Services, Communications and
Government Affairs, Director of Video Services, and Project Coordinator. At the Forum, Golden
West's General Manager /CEO gave an overview of the services provided on the reservation and
responded to questions from the RSTUC. The Company's General Manager/CEO and Director of
Member Services, Communications and Government Affairs met on April 5, 2013 with the
President and Economic Specialist of the Rosebud Sioux Tribe. Discussions at this meeting
focused on Golden West's services on the reservation, right-of way, and broadband
affordability in tribal communities. Golden West's General Manager/CEO stated that if the Tribe
would designate a building in each community, Golden West would provide free broadband to
the building. RST's President thanked Golden West for its services and the resources the
Company has invested on the reservation. A follow-up letter was sent on April 12, 2013 to the
RST President recapping the items discussed during the meeting and thanking him for the

Communications and Government Affairs, and Right-of-Way Technician participated by
conference call in the May 7, 2013 RSTUC Meeting to discuss the processes necessary for
obtaining permission from RST to complete upgrades on tribal lands. Golden West's Right-of
Way Technician also sent an email to the Director of the RSTUC on June 14, 2013 requesting
advice on how Golden West could make the right-of-way process on tribal lands more efficient,
but no response was received from the RSTUC Director. Golden West's General Manager/CEO
and Director of Member Services, Communications, and Government Affairs met on November
25, 2013 with the President and Chief of Staff of the Rosebud Sioux Tribe. Further discussion
was held regarding Golden West coordinating with RST to provide internet access to a building
in one of the tribal communities. A discussion was also held at this meeting about the barriers
Golden West faces when deploying FTTH or other telecommunications facilities within the
Rosebud tribal boundaries. A follow-up letter was sent on December 16, 2013 to the RST
President recapping the items discussed during the meeting and thanking him for the
opportunity to meet.
The Company's General Manager/CEO and Director of Member Services, Communications and
Government Affairs met on two occasions during 2013 with representatives of the Oglala Sioux
Tribe (OST) to discuss issues related to the deployment and provisioning of communications
services on the Pine Ridge Reservation. The first meeting was held February 13, 2013 with the
President of the Oglala Sioux Tribe. Discussions at this meeting related the Company's services
and role on the Pine Ridge Reservation. A follow-up letter was sent on February 14, 2013 to the
OST President recapping the items discussed during the meeting and thanking him for the
opportunity to meet. The second meeting was held on September 4, 2013. Attendees from OST
included the OST President, Treasurer, Fifth Member, and Secretary. Topics of discussion at this
meeting included the Company's provision of broadband on the Pine Ridge Reservation and
ways the Company can work with OST for further deployment of broadband. The Company and
OST attendees also agreed to meet in the future to continue these discussions. Golden West
received notice on December 12, 2013 of the establishment of the OST Utility Commission and
anticipates participating in future meetings held by the Commission .

Sioux Tribe in 2014. The Company, with Tribal input, will continue to evaluate the need for
future service deployments on Tribal lands while working to overcome challenges associated
with right-of-way and permitting processes. The feasibility and sustainability of
communications services on Tribal lands will be discussed and the Company will, with
assistance from the Tribal Authorities, attempt to identify additional steps that can be taken to
make essential communications services deployed on Tribal lands both feasible and
sustainable. The Company and Tribal Authorities will also discuss and explore ways in which
they can coordinate to ensure that services are marketed on tribal lands in a manner that will
relate to the community and resonate with consumers, with the aim of increasing service
adoption.
Lifeline Terms and Conditions
Golden West Telecommunications service to qualified low-income residential consumers for one telephone line per The Lifeline program provides discounts to eligible low-income consumers to them establish and maintain telephone service. Program Based Eligibility
Consumers are eligible for Lifeline if they, one of their dependents or their household participate in one of the following qualifying assistance programs:

Golden West Telecommunications Cooperative, lnc.'s Voice Lifeline Plan does not include any free minutes-of·use for to!L Toll is billed at the standard toll rate depending on which interexchange carrier the consumer subscribes to for toll service. As part of the Lifeline service, toll blocking is available to eligible consumers at no cost
Subscribers may receive the Lifeline credit on any type or grade of local service, including bundled services that are normally offered by Golden West Telecommunications Cooperative, Inc. Advertised rates do not include any applicable taxes or surcharges.
Recertification of Lifeline Eligibility
Lifeline recipients are required to recertify their eligibility annually. Failure to properly recertify a recipient's continued eligibility for the Lifeline program will result in termination of the Lifeline recipient's monthly Lifeline discount and de-enrollment from the Lifeline Program
