We have received information indicating that this organization may be subject to the registration andreporting requirements in sections 12585 and 12586 of the California Government Code. The followingitems must be submitted in order for this office to make a determination concerning the above captionedentity’s compliance with the law and regulations:

1. A copy of the executed founding document. (Depending on how your entity was formed, thatdocument may be entitled Articles of Incorporation, Trust Instrument, Articles of Organization, orArticles of Association.) Please also include a copy of any amendments to the foundation document.

2. A $25.00 registration fee, made payable to the Department of Justice, is required for initialregistration. Please indicate the file number listed at the top of the letter .

3. An executed copy of the most current bylaws.

4. A copy of the Application for Recognition of Exemption (IRS Form 1023) together with a copy of theIRS determination letter, if received. If the determination letter has not yet been received, please provideus with the entity’s FEIN.
