Improper Lookout

COLREGs Rule 5: Look-out Every vessel shall at all times maintain a proper look-out by sight and hearing as well as by all available means appropriate in the prevailing circumstances and conditions so as to make a full appraisal of the situation and of the risk of

collision.

STCW Lookout 14 A proper lookout shall be maintained at all times in compliance with rule 5 of the International Regulations for Preventing Collisions at Sea, 1972, as amended and shall serve the purpose of: .1 maintaining a continuous state of vigilance by sight and hearing, as well as by all other available means, with regard to any significant change in the operating environment; .2 fully appraising the situation and the risk of collision, stranding and other dangers to navigation; and .3 detecting ships or aircraft in distress, shipwrecked persons, wrecks, debris and other hazards to safe navigation. The lookout must be able to give full attention to the keeping of a proper lookout and no other duties shall be undertaken or assigned which could interfere with that task.

The Captain on the Pilot Boat was making a telephone call to VTS. When dialing the number the boat turned to starboard. When looking up from the phone the Captain noted a lighthouse on his portside and was satisified with this. However it was the wrong lighthouse.

On December 5, 2009, a Coast Guard shore-based response boat collided with a small passenger vessel harbor of Charleston, South Carolina. Five passengers on the Thriller 09 were injured and required treatment at local hospitals.

Safety Advisory Bulletin 09-02 - Mobile Phone (Cell Phone) Use and Marine

Operations • Restrict mobile phone use by on-duty personnel

to operations-related calls. • Set time limits for those calls—those limits

should allow only brief, important conversations. • Identify locations and circumstances when on-

duty mobile phone calls are permitted, or are not permitted.

• Outline procedures for on-duty personnel to get relief should they receive a personal mobile phone call of an emergency nature.
