Dear Friends,
Victory or success measured in terms of material gains and the
applause associated with it is surely going to make the humblest of
souls slightly egotistical and haughty at some point of time. But at the
end of the day when you go into the twilight you realize that you have
merely set a precedent for others to emulate and follow and you are
just a remnant etched in memory and time.
The brilliance of the crowning glory that was AMACON 2016 and
its magnanimous success has made us evaluate and introspect that we
ourselves have raised the bar one notch higher and all future events at
AMA will be minutely scrutinized and laid bare. But again who says
success is not sweet. The annual banquet with its lavish and
sumptuous spread was savoured by every one's taste buds and was
truly well received by all those present.
It is that part of the year when a multitude of changes affect the
person at all levels- whether it is the climate, the finances, political
scenario, sports adventures as well as matters related to medical
practice. Spring has just been shown the door and summer has already
set in with all guns blazing. With the world T20 being held in the
subcontinent and the subsequent Indian cricket festival IPL, cricket
has enraptured audiences and captured the senses of one and all
across the length and breadth of the country. To cash in on this cricket
fever, AMA has organized a tournament for the doctor fraternity as th thwell. It is scheduled to be held from 7 May to 15 May, 2016 and we are
sure it is going to be a grand success from its inception itself.
The doctors' interest in CMEs has not been dampened by the
slack season in previous months and subsequent vacations in the
coming months. All programmes organized by the AMA have seen a
decent audience and the orators have been heartened to see such a
receptive audience.
Let us keep our spirits high and not be disheartened by certain
vicarious elements that are hell bent on destroying peace and harmony
in Indian society. Hope to keep you entertained in the coming months
too.
Friends, along with self learning programmes & CMEs, it is our
professional obligation to participate in implementation of various th
national health programmes. 25 April is the national malaria day- let
us all join hands to do our best to eradicate this deadly disease. The
vector- borne disease control board has put forward the malaria
guidelines for the diagnosis and treatment of malaria. It is our humble
appeal to one and all to strictly follow these guidelines and co- oparte
in the malaria eradication programme.

REPORT OF AMACON2016
As per tradition of Ahmedabad Medical Association this year also thAMACON2016 was organised on 10 April, 2016 at the premises
of Ahmedabad Medical Association. More than 375 members
were registered out of which more than 100 were registered as
R.C. members.
We are thankful to each one of them for giving this overwhelming
response.
thOn the previous day, on 09 April, 2016 two (02) Pre Conference
workshops were organised one at HCG Hospital, Mithakhali and
other at CBCC Apollo Hospital, Parimal Garden. They were
organised between 01:30 PM to 04:00 PM.
thOn 09 April, 2016 in the evening from 08:00 PM onwards Pre
Conference Seminar was organised on Non Medical topics.
thOn 10 April, 2016 for the whole day there were Scientific
Programmes.
Dr. Nitin Vora, President of Gujarat Medical Council was Chief
Guest of the inaugration ceremony.
In the end there was lucky draw and prizes worth more than Rs.
40,000/- were distributed.
Food was mouth watering and was appreciated by all.
Over all it was a memorable conference and delegates will

RECENT ADVANCES IN VITREO RETINA-
INTRODUCTION-
Eye is a special sense organ comprising of many complexities .So for proper
diagnosis of a disease and treatment it has arbitrarily been divided into anterior
and posterior segments. While the anterior segment deals mainly with cornea,
glaucoma, lens and disease of the adnexa, the posterior segment deals with the
diseases of vitreous and retina. A few decades ago the more and more emphasis
in ophthalmology was on diagnosis and treatment of the anterior segment
diseases but after the invention of fundus camera and Vitrectomy machine a
large group of surgeon’s focus was shifted on posterior segment disease, a
domain which was suppose to be not only risky but inaccessible till then.
What is the latest?
For the sake of understanding we can subdivide the topic into
1. Diagnostic tools and techniques
2. Newer pharmacological agents
3. Surgical instruments
1. Diagnostic tools and techniques
The biggest advancements are
a) ULTRA WIDE FIELD FUNDUS CAMERA-
Older cameras use to give a view of central 45 degrees and it use to be mandatory
to dilate the pupil but with the new type of fundus camera we can take a fundus
image of almost up to the periphery and there is no need of dilatation of the pupil.
The scan can be taken from a 2mm of pupil so patient remains comfortable and
we can screen more patients in less time but as it is said that quality comes at a
price , the machine is quite expensive.
b) MULTIFOCAL ELECTRO RETINOGRAM-
As all of us know that electro retino gram and electro oculogram are the methods
use to screen the retinal rod and cone cells and condition of retinal pigment
epithelium in cases of hereditary diseases of retina and many degenerative and
inflammatory conditions. This multifocal erg machine not only does a fast scan
but gives more information in many conditions than conventional machine
especially in macular pathologies.
c) NEW OCT MACHINES WITH AUTOFLUORESENCE AND ENHANCED DEPTH IMAGING-The new oct machines are fast, have more resolution and tissue penetrance with features of enhance depth imaging and auto fluorescence. So many of the conditions are diagnosed at an early stage and we can see their progression before they clinically become more evident.
d) COMBINED OCT AND ANGIOGRAPHIC MACHINE-
the conventional optical coherence (oct) machines gives a cut section of the
live tissue but the drawback was the vascular conditions where the role of
angiography becomes more important but this is an invasive procedure with
problem of dye reaction. with the invention of new machine we get a better cut
section of oct and we get an idea of the blood vessels condition directly without
seeing the real leakage and hence making it noninvasive and very fast without
the need of dilatation of the pupil.

2. Pharmacological agents-
a) USE OF VARIOUS DYES-
With the invention of modern surgical system the use of newer pharmacological
agents to stain various structures of retina and vitreous has made the surgical
outcome better and predictable.e.g
-use of tiramcenolone acetate to stain the vitreous
-brilliant blue dye to stain the internal limiting membrane in macular hole
surgeries etc.
b) ANTI VASCULAR ENDOTHELIAL GROWTH FACTORS-
with the increase in life expectancy of human life as a whole over the globe and
change in the life style of life now we are finding more cases of age related
macular disease and diabetic retinopathy etc. so with the invention of many anti
vegf intra vitreal injections like Ranibezumab and aflibercept 2mg etc many
patients get a useful and stable vision in such potentially blinding conditions.
c) DEPOT PREPARATIONS-
In many conditions of retinal disease like central retinal or branch retinal vein
occlusion there is persistent macular edema and we need to give depot
preparations of the steroids in the eye to avoid the systemic complications of
steroids so the implants like ozurdex and iluvein (fluocinolone acetonide 190
micro gram) etc give very good results.
d) MEDICAL VITRECTOMY-
Though with the advances in new Vitrectomy machines the Vitrectomy has
become a very safe procedure but still there are inherent risks involved in that.
With the advent of Ocriplasmin which dissolves the vitreous fibronectin and
laminin with its protease activity once injected into the vitreous cavity relieves
the traction over the Vitreo macular traction syndrome cases and hence patients
are saved to undergo surgical Vitrectomy.
e) NEWER RADIO ACTIVE PLAQUES FOR MALIGNANCIES-
In the cases of malignant choroidal melanoma initially people use to apply
radioactive plaques in the form of cobalt etc. but with the new plaques like
ruthenium 106 and Iodine 125 the complications of fibrosis of adjoining tissue
and erosions of the tissue are quite less.
3. Surgical instruments-
a) NEWER ADVANCE MICROSCOPES WITH VIEWING SYSTEMS-
With the new type of microscopes we not only can zoom the image and play in
x, y and z dimensions but with the advent of oct mounted microscopes we can
see the condition of the retina on table and newer viewing system like Bioms,
Eiboss and Resight etc we get a large view of the fundus without the need of a
contact lens and we can do the surgeries more boldly.
b) MINIMAL INVASIVE VITRECTOMY SYSTEM OR SUTURELESS VITRECTOMY-
With the new machine with a high cutting rate of 7500cuts/min and very good
dynamics of fluidics and with the use of 23/25/27 trocar and canula systems
now majority of the Vitrectomies are sutureless and safe. The use of port
optimization technology of cutters, different duty cycles and better internal
drainage devices have made Vitrectomy machine very user friendly. With the
incorporation of high resolution cameras and 3d technology now viewer of a
conference can enjoy a very well 3d view of the live surgery, which makes it
fancier.

c) MULTI SPOT PATTERN SCAN LASER AND DIFFERENT COLOR LASERS-
Now various multiple pattern multisport laser system are available in system
which are very fast and hence we can complete the pan retinal photocoagulation
of an eye in a single sitting or in two sittings with least collateral damage to the
blood flow. Similarly new laser systems with multiple wave lengths in a single
machine have made the use of laser very easy in ophthalmology in different
conditions.
d) ARTIFICIAL RETINA/RETINAL IMPLANTS-
In certain blinding conditions like retinitis pigmentosa now certain
epiretinal implants are available which can transmit the impulses to the
visual area in the brain through a set of electrodes and camera. It is slowly
and steadily improving in outcome and a better outcome is being
expected now with the new machines.
d) GENE THERAPY AND STEM CELLS –
