H410 Professional Portfolio
Instructions:
This is an example, replace the current information with your own. Expand on the sections, and make it thorough, with detail. You can go all out and design it in a more professional way. Your time on
your porfolio counts as internship hours The intent of the portfolio is for it to help you in the future, so you can show
it at job interviews, career fairs, etc. so make sure to make it your own and something that will be helpful to you.
It is due the Tuesday of Finals week, by uploading it in Canvas. You can use other user-friendly formats, such as pdf, but please no pictures!
If you are splitting your internship between two terms, this is submitted each term.

coursework that they would like to use to showcase their skills. This sample should be something that the student feels is the best representation of what
they have learned, includes some of their best research, and is something that they are proud of.

Students may include more than one work sample if they would like.

Optional Professional Development Section
This section includes options if you need extra internship hours OUTSIDE of your internship to reach 360 hours.
Any activity related to your internship site, counts as internship hours and would not be counted in this optional section.
Professional Development: Each student may complete up to 40 professional development hours. If you are splitting the internship over two terms, you may complete up to 40 hours total over two terms of professional development. It is up to you how you want to divide it. You can do some or all of the professional development options (A-D)
Optional Journal: This can also be done for up to 20 hours. If you are splitting the internship over two terms, you may complete the optional journal up to 20 hours total over two terms. It is up to you how you want to divide it
The PROVIDENCE INTERNSHIP IS AN EXCEPTION: they only provide 240 hours. Therefore, for this site only, you can increase the professional development section to 100, with option C as strongly recommended, and the journal for 20 to reach the 360 hours.
Please include this section with your portfolio when you upload by the Tuesday of Finals Week.
If you do both the optional journal and professional development, that gives you 60 hours, so you only need 300 on site for one term.

Then, write a 1-2 paragraph summary of what you learned from the reading this book, how it relates to the field of public health and how you will use this knowledge you have gained in your future public health practice.

Optional Journal Section
This section is an optional journal if you need extra internship hours OUTSIDE of your internship to reach 360 hours.
Any journal or weekly writing summary/blog related to your internship site, counts as internship hours and would not be counted in this optional section.
The minimum length is 1-2 pages per week, double-spaced, 12 font, Times New Roman.
For each week you choose to do, 2 hours will be counted towards the 360 for a total of up to 20 hours.
Your journal needs to be clearly labeled by week, and then you can choose to add daily headings or keep just the weekly heading.
It can include perceptions, reflections of the internship. You can also discuss career interests, goals, skills, pursuing graduate school, etc. Please keep the journal professional and an opportunity to reflect on your experience and your career goals.
Please note, this document will be kept strictly.
If you choose this option, please put your journal entries after this page, in the portfolio and upload it by Tuesday of Finals week.
