NJWELL – A New Incentive for DPCNJWELL is offering a $250 wellness incentive to active SHBP and SEHBP members and their covered spouses or partners who participate in the state’s DPC program.
MEMBERS AND COVERED SPOUSES OR PARTNERS CAN EACH EARN $250.Any active member of Paladina Health or R-Health will automatically qualify for the NJWELL reward card when you:• Complete a health assessment through your health plan’s online portal• Complete a comprehensive physical examination and at least one follow up
appointment with your Paladina Health or R-Health doctor• Create a health map in partnership with your Paladina Health or R-Health
doctor• Complete all of the above between November 1
and October 31, 2019

Who Can Use the DPC Program?• Anyone who has their health benefits through the New Jersey
SHBP or SEBHP with one of the following plans:• Horizon NJ Direct• Horizon Omnia• Aetna Liberty• Aetna Freedom• New CWA PPO Plans
• Available for employees and their covered dependents (family members)
• It’s an individual selection – not all family members need to use a DPC doctor
• Not available for members with HMO or high deductible plans• Not available for Medicare retirees
