Background Information:
Chani is 4.4year old girl who has been receiving services since December. Chani resides at 947-51st street in
Brooklyn, NY. The primary language spoken at home is English. Chani is in good health and has been
attending Beth Jacob of Boro Park regularly since September. Chani has been receiving 8x60 sessions of
SEIT per week. She also receives 2x45 sessions of speech therapy and 2x30 sessions of occupational therapyper week.
General Classroom Performance Based On Current IEP Goals:
Cognitive:Chanidemonstrates weaknesses in her verbal and nonverbal skills. In verbal areas, she had difficultylabeling items, displays a weak knowledge of words, as well as a short term memory for sentences. In
nonverbal areas she has difficulty understanding quantitative concepts such as more/less. She also has
difficulty completing a puzzle and has poor problem solving skills. Chani has difficulty maintainingattention during teacher guided activities, as she cannot sit and attend for a significant amount of time. She
cannot follow or complete an ABAB pattern. Chani has recently begun to attend and remain on task for a
slightly more considerable amount of time, however only when in a group setting. She can identify basic
shapes and count from 1-10, but is not consistent with it. She can also identify some letters of the alphabet.
Chani learns best during group activities as opposed to 1:1, but requires lots of prompting and directguidance.
Speech and language:Chani presents with delays in both receptive and expressive language. She has difficulty answering wh
interrogatives, understanding commands involving spatial concepts as well as pronouns and possessives. In
addition, she has difficulty maintaining a conversation for more than three exchanges. Chani remains quietduring circle/lesson time, but does not pay attention to the lesson, as she is unable to retell the main idea of
the lesson when prompted. She can communicate her wants and needs effectively. She has also made some
improvement in following commands to complete a project.

Self-help skills:Chanipresents with age appropriate self-help skills.
Social/emotional:Chani has made significant progress in this domain. She initially presented some social awkwardness which
her peers picked up on. She does, at times, get mildly aggressive with peers, however for the most part sheinteracts and gets along nicely. She has made some improvement at staying with an activity of choice
during play time; however improvement in this area is still necessary. Chani needs to be reminded about
maintaining personal space between teachers and peers.
Gross/fine motor domain:Chani has a poor grasp on writing utensils. She has difficulty shading in pictures completely and when toldto draw something she will only write a line. She has recently begun to imitate a circle. She does not fully
understand the concept of cutting out specific shapes or drawings, as she displays extremely poor cutting
skills. Chani requires lots of practice and repetition of required skills to improve her fine motor skills.
Teacher/Parent Interview:
Chanis teachers are truly amazed at the progress shes made thus far. She is beginning to participate better
and act more age appropriately among her peers. She has learned much through the direct instruction she
has been receiving, as well as through observing and imitating her peers. She can now join in an activityand stay with it for a more significant amount of time, yet improvement is still needed. In addition, she is
well liked and gets along nicely with her classmates. With continued help and guidance, Chani will continueto improve, as progress is seen daily.
Concerns (if applicable):
Chani demonstrates difficulty in a wide range of academic skills. She cannot attend and participate for anappropriate amount of time, particularly in 1:1 settings. She has difficulty answering general wh questions, as
well as questions pertaining to a lesson taught. She requires lots of prompts to remain focused on specific fine
motor exercises and activities. She cannot follow or complete an ABAB pattern and has difficultyunderstanding the main idea a story.
