Triple Rhomboid Flap A 72-year-old man with a squamous cell

carcinoma on the vertex of his scalp. He was treated with hexagonal excision and

closed with three local, rhomboid flaps.

Triple Rhomboid Flap After hexagonal excision of the lesion, the

three rhomboid flaps based on the component rhomboids of the hexagon are planned.

Page 6: Triple Rhomboid Flap

Triple Rhomboid Flap The rhomboid flaps are incised and then

elevated. The elevation should be complete; to achieve adequate transposition, undermining should be taken beyond the bases of the flaps.

Page 8: Triple Rhomboid Flap

Triple Rhomboid Flap When the flaps are transposed, there should

be minimal tension on them.

Triple Rhomboid Flap The triple rhomboid flap offers significant

advantages over described methods for lesions of the vertex. The area removed is converted into a hexagon. The hexagon is then excised and closed by rotation of three rhomboid flaps from the periphery.
