The evaluation explored :
•Useful information for the team to review the content and in-depth of the education program to meet the needs of
patients with different knowledge levels.
The outcomes can be used for stratification of education program for patients with different needs and knowledge
levels
A focus group discussion with patients and their families will be useful to address the gaps, and explore those with
different level of needs
For future program, and the necessary support to increase patients’ empowerment towards self-care.
