runny nose, sneezing,nose block and throat pain.Pneumonia can be diagnosed by chest x-ray,sputum test and blood test (complete blood countand culture). Pneumonia can be effectivelytreated by antibiotics including anti viral,antibacterial and antifungal in proper dose andproper duration.
Pneumonia can be prevented by takingvaccinations like Flu vaccination annually forinfluenza including H1N1 (swine Flu) beforemonsoon and pneumococcal vaccine 3 doses(both conjuguate and polysaccharide) forstreptococcal infections. These vaccines arerecommended to all risk groups by the WorldHealth Organisation. Early diagnosis andtreatment is very important to preventcomplications like hospitalisation for sepsis,multiorgan dysfunction, respiratory failuire,ARDS and death. Vaccination, hand and coughhygiene are effective aspects of pneumoniaprevention.

The Prevalence of Diabetes is more in men thanwomen, At present there are 199 million women withdiabetes which is projected to increase to 313 millionby 2040. Diabetes in women is more important sincethe survey in 2007 showed that death rates amongDiabetics between 1971 and 2000 reduced in men butnot In women in whom it has in fact doubledcompared to women without Diabetes accountingfor 2.1 million deaths globally every year and is the9th leading cause of death in women. This is becausethe CVD symptoms are not only atypical but CVDrisk factors are managed less aggressively in womenwhen heart disease is increased ten folds in womenwith diabetes compared to three folds in men withdiabetes and the outcomes are poorer in women withdiabetes with risk of heart failure.

The Prevalence of T2 Diabetes isincreasing in Epidemic proportionsespecially in India and other developingNations, affecting the workingpopulation in India at a time when Indiais poised to become the youngestnation in the world derailing itseconomic development.

Diabetes in women Affects both themother and child and offers anopportunity for prevention of diabeteseven before birth with focus on Fetusas one in seven births is affected byGDM and two out of 5 women withdiabetes ( about 60 million ) are in thereproductive age group.
Unlike Men , Women not only haveadditional risk factors for diabetes like PCOD, historyof GDM , large for date babies ( more than 9 pounds) but also the hormonal changes associated withmenarche child bearing, menopause and oralcontraception posing difficulties in Glycemicmanagement. Therefore the theme for this year worlddiabetes day is women and diabetes - our right to ahealthy future, As women and girls are the key agentsin the adoption of healthy lifestyles to improve thehealth and well being of the future generation andshould have a affordable and equitable access toscreening , care and education.
The key messages:
Screening of GDM should be universal. Womenwith diabetes should have aggressive managementof CVD risk factors, Should be on statins between 40and 75 years. Women above 45 years of age shouldbe screened for diabetes.
Women planning for pregnancy should haveHbA1c less than 6% for better foetal outcomes andshould postpone pregnancy if HbA1c is above 10%.
Pregnant diabetics should maintain fasting sugarless than 90 mg % and two hours PP less than 120mg%and mean plasma glucose of 95 - 105mg for betterfoetal outcomes.

1) Dr.T.N.Ravishankar: CGP courses cannot be concentrated in one place, but it should beconducted all over Tamil Nadu. We can have theshort time courses for a period of 6 months ratherthan 1 year. He requested the members to givesuggestions to improve the CGP courses. Hesuggested courses should have fee of Rs.20,000/- only . He also advised online classesat the earliest.
Dr.R.Gunasekaran: CGP should enroll allcandidates undergoing courses as CGP members.He has suggested not to conduct the samecourses in National DGP s sell as IMA TNSB.New courses can be started
Dr.J.A.Jayalal: Restructuring CGP is ourpriority. CGP courses should spread out. Wehave to rope in good faculties. we can go forAcute Care course in 6 months duration. Inone year we should able to train 100 doctors in 4online classes and 4 in house classes. He alsoasked the Professors to give papers for MedicalPublication.
Dr.M.Parthiban: The courses should havebrand name. Duration must be small. Doctorsattending CGP classes should have computerknowledge. It is also decided to have 2 hoursComputer class compulsorily in all courses.Using smart phone and IMA Apps emphasized

Greetings from PPLSSS.
I am happy to inform that some of the vacantposts of the District coordinators were filled andI came to understand still 10 posts of the Districtcoordinators are vacant. The branch office bearersof those districts should motivate and all theseposts to be filled before the next ManagementCommittee Meeting.
The district coordinators are requested toselect or appoint lawyers in consultation withbranch office bearers for their branches and theyshould be informed our fee structure for appearingfor cases.
I want all our coordinators should learn towrite reply to the legal notices. Recently some ofthe hospitals in Salem are fined for not dispersingthe wastes and also not keeping the Hospitalsurroundings clean.
I am surprised whether the corporationCommissioner is empowered to put fines on theerring Hospitals without prior notice. We had ameeting with Divisional Engineer of PollutionControl Board and Ramky Agency for disposal ofbiomedical waste and to chart out other problems.
I have put two questions to the DE
1. Whether the Commissioner has right to take
action on the deficiency of Pollution ControlBoard.
2. Is there any guidelines in manual of corpora-tion regarding imposing the fine system.
After our protest they have stopped visitingthe Hospitals. I do agree that our side have alsosome defects which should be rectified beforefinding fault on others. Let us alljoint together toprotect our members from litigation. Recentlyone person filed a case against a Govt. Doctor fornegligence and also for having a Nursing Homeon his name. I know many innocent Doctorsmight have this problem and request no Govt.Doctor should have a Hospital or Nursing Homeon his name and such things should be changedimmediately.

NEGLIGENCE PERSE - in Medical Negligence.
Dear friends, Greetings to all,In the tort of negligence the
burden of proof of negligence is uponthe person who alleges negligence and,to establish a case. The complainantmust prove negligence against themedical practitioner affirmatively, byadducing reasonable evidence of it.
As a rule the mere proof that anaccident has happened, the cause ofwhich is unknown, is not evidence of negligence.
In special circumstances, however, the mere factthat an accident has happened may be prima facieevidence of negligence, casting upon the partycharged with it the onus of proving the contrary, forowing to the nature of the accident it would beinequitable for the plaintiff to prove negligence.
The Doctrine of res ipsa loquitor is thus anexception to the general rule that the person whoasserts negligence must prove the same.
Michale Joans in the “Treatise on MedicalNegligence” has explained the principles of ‘res ipsaloquitor’ as essentially an evidential principle andopined that the said principle is intended to assist aclaimant who, for no fault of his own, is unable toadduce evidence as to how the accident occurred.The illustrations are set out below:� Where gangrene developed in the claimant’s arm
following an intramuscular injection.1
� When a patient underwent a radicalmastoidectomy and suffered partial facialparalysis.2
� Where the defendant failed to diagnose a knowncomplication of surgery on the patient’s hand forPaget’s disease.3
� Where there was a delay of 50 minutes in obtainingexpert obstetric assistance at the birth of twinswhen the medical evidence was that at the mostno more than 20 minutes should elapse betweenthe birth of the first and the second twin.4

� Where, following an operationunder general anaesthesia a patientin the recovery ward sustained braindamage caused by hypoxia for aperiod of four to five minutes.5
� Where, following a routineAppendicectomy under generalanesthetic, an otherwise fit andhealthy girl suffered a fits and wentinto a permanent coma.6
� Where a needle broke in the patient’s buttockwhile he was being given an injection.7
� When an explosion occurred during the courseof administering anaesthetic to the patient whenthe technique had frequently been used withoutany mishap.8
The doctrine was first stated by Erle.C.J in 1865,said, The doctrine may be used where the accident isof a kind which ordinarily does not occur in theabsence of negligence, the apparent cause of theaccident being within the control of the defendantand the plaintiff could not have contributed to it. Theevidence as to the true explanation of the event mustbe more readily accessible to the defendant than tothe plaintiff.
The Supreme Court of India observed that “Themaxim does not embody any rule of substantive law,nor a rule of evidence. The maxim is only a convenientlabel to apply to a set of circumstances in which theplaintiff proves a case so as to call for a rebuttal fromthe defendant without having to allege and proveany specific act or omission on the part of thedefendant.
The usual example is a case in which a foreignbody is left within the patient’s body during surgery.
In Indian Medical Association Vs V.P.Shantha1995 (6) Sec 651, (Para 37) it has been held that thefollowing acts are clearly due to negligence;1) Removal of wrong limb. 2) Performance of an
operation on the wrong patient.
