Some Of What We Learned

• Access to genuinely local information is difficult

• The information and interface design needs to support the individual in their current situation

• The interface needs not only support self-management but should also educate about self-management

• Needs to work in unison with other sites such as My Condition, My Life and NHS Inform

• Be targeted towards specific points along the patient journey, with the information tailored to that point

• Help not just with the condition but with the practical issues that surround things like accessing services, being aware of patient responsibilities

• Interface needs to help the individual to make good decisions

• Should be designed around the genuine needs of those with Long Term Conditions in terms of management, diagnosis and relapse

• Locally SID strong brand for this type of site
