The need to enhance the practice of using medication card in supporting the “10” rights of medication administration.
Cues:>some medications of
patients were not written in a medication card
>some medication cards were not located on its proper rotation with respect to time frequency
Medication card is a tool use in every nurses’ station which helps in meeting the 1st five rights of medication administration (RIGHT client, medication, dosage, route, time, assessment, documentation, refuse, education and evaluation). It is designed with corresponding colours depending on time frequency.
A. Ask any of the health care team to always check medication card and use it wisely
B. Assign a staff nurse to check all medication card of his clients.
