Our Geriatric Care Managers can:• Schedule doctors’ visits and coordinate medical care

• Attend medical appointments with clients andreport back to families

• Manage crisis situations such as accompanying aclient to the ER

• Re-assess clients upon hospitalization• Explore options with families for medical andresidential care including assisted living andnursing home choices
