Click here and select what kind of question you would like to add to
your questionnaire.
Click here and select what kind of theme add to your questionnaire.

Here is where you select the theme of your questionnaire click on one to preview

If you are happy with your theme then click “Apply”
If you are not happy with your theme click “Cancel” and chose
another

If you are happy with your questionnaire then click “Email this
form”

Log back into Google and go to more and select “Documents”
Click “Documents”

Click on the “Form” button and select “Show summary of responses”

Click to stop responses being accepted.
You can stop taking in responses by going to “Form” and selecting “Accepting responses”
