CVS MinuteClinic Reminder
• Subscribers must visit a CVS MinuteClinic for at least 1 face-to-face
tobacco cessation counseling session before January 31, 2019, or risk
losing their premium credit.
• Subscribers must bring the voucher and their State Health Plan ID card to
each session for the visit to be covered at 100% by the Plan.
• You will need to submit an enrollment exception before January 31, 2019,
for employees who do not live or work within 25 miles of a CVS
MinuteClinic.

Open Enrollment Exception Reminder
• Per State Health Plan policy, January 31, 2019,
will be the last day to submit Open Enrollment
exceptions.
• Enrollment exception requests must be submitted
to the Plan within the following time frame:
• Within 60 days of enrollment, termination, or
change in benefit election, or within 30 days of
paycheck deduction or premium payment due
date reflecting enrollment, termination, or
change in benefit election, whichever is later.

Social Security Number (SSN) Terminations
• A valid SSN is required for any dependent over 6 months of age enrolled in the
State Health Plan. The SSN is required for 1095 reporting, as well as other federal
reports the Plan must submit.
• To ensure compliance, the Plan mails letters to members who have dependents
without a valid SSN.
• If you have impacted members who haven’t updated their dependent’s Social
Security number, you will be contacted directly with the member details and a
request for you to reach out to these members with this letter to obtain their
valid, unique SSN. If valid SSNs are not provided, dependents will be
terminated.
• Dependents will not automatically be re-enrolled when an SSN is added after the
termination is processed. The only way these dependents can be added back to
the Plan is with a valid qualifying life event (QLE) or through the exceptions
process when a valid SSN is provided.
• Additionally, if there is no valid dependent verification information on file for
these dependents, the exception will not be reviewed.

1095 Tax Form Information
• 1095 is a tax document. It provides information needed to report on your tax return for your records only – not for submission.
• This form is NOT needed to submit the tax return. It is simply supporting documentation to keep with your personal tax records.
• Individuals should keep this document and all tax-related documents in the event they are audited.
• Active employees in 2018 should speak with their HBR regarding their 1095 for the months they were employed or on COBRA.
• Individuals with questions about the 1095 should call the number listed on the 1095. This will either be their HR department or the Enrollment and Eligibility Support Center. Retirees will call the State Health Plan office.
• The Plan will be mailing out 1095s by the end of January even though the IRS deadline is 3/2/2019.

New Employee Checklist
• We’ve added a Checklist for New Employee Onboarding to the Enrollment Information page under the HBR section of the Plan’s website.
• The checklist will help you make sure you have successfully oriented new employees regarding their State Health Plan benefits.

Contact Validation Emails
• On Friday, January 11, the Plan began sending out emails to all groups
requesting HBRs to verify contact information.
• This is a yearly process to ensure we are contacting the appropriate
individuals for all issues (QLEs, benefits, billing, etc.) to avoid delays
which may impact members.
• The email will come from [email protected]
• A Verification Form for your group as well as a guide to help you read the
form will be attached for your review.
• Please reply no later than January 18, 2019, with contact updates.
• If we don’t receive a reply by this date, we’ll assume no updates are
needed and will maintain your group’s contact info as is.
• If you receive the email in error, please reply to all and include the
appropriate party.

HBR University – Mandatory HIPAA Training
• The Plan has developed a HIPAA training that will be required for HBRs to
take through HBR University.
• To date, over 330 HBRs have enrolled in the training since its release in
November.
• The HIPAA training will continue to be released in waves over the next few
months.
• You will receive an HBR Alert via email prior to your training’s start date.
• You will have 30 days from your start date to complete the training.
