5a. This handout is a review for a test which is aligned exactly with the format of test. In a lower level math class, I find that it is often the case that students understand how to do the mathematics, but simply don’t understand what the question wants them to do. Aligning the review to the test helps these students tremendously.

5e. This is a link to Edline where I post grades at least once a week to communicate with students and parents progress of each student.

Faith f. Kairos is one of the best experiences a student or teacher could have at Dowling. To be able to build and participate in community worship with students at Dowling is why I teach in the first place. My faith is strengthened every time I attend Kairos. The student talks are amazing and working with Christy Sheaf is the best!

1. IF I DID A FORMAL OBSERVATION - any modifications you made in lesson(s) design following any lesson observation I did this school year – I generally make suggestions in the comments section of these reports – briefly, tell me how you incorporated my suggestion(s) into a future lesson(s) – if I didn’t observe you formally, ignore this one.

-No Formal Observation in 20092. Gold seal lesson # 1 – please explain how this went for you (planning the lesson, teaching it, etc., and be sure to include your perception of how the students reacted to the lesson)

3. Gold seal lesson # 2 – (this semester) – same as #2 above

4. Explain how you’ve used 2-3 of the strategies in the Instructional Strategies Handbook - - - if you used these in conjunction with one of the GSL’s, then explain in your GSL reflection.

5. Self-evaluation - - - look back over the year, and prepare a 1-2 pp. written self-evaluation of your work this year. Explain what worked (and what didn’t) and review your own progress toward the goals you had for the year. (see #7 below)

6. Review your evaluation (annual progress check) from last year – explain how you’ve improved and any steps you took this school year to follow suggestions I might have made

7. I’d like your self-evaluation to include your thoughts on your own readiness to do the 21st century instruction you’re being asked to do (competence with technology –smart boards, white boards, etc. that will be in the new rooms, gold-seal lesson design, etc.)

8. Review the draft IPDM you sent me on March 13 – if you want to make any changes, do so and re-print it and submit with all the above – if you have no changes to make, note that to me and I’ll review it as is
