What is a Community Information Exchange?
An integrated network of health and human service organizations that

▪ Use a common language to facilitate data sharing
▪ Make bi-directional, closed-loop referrals
▪ Contribute to a longitudinal record for individual clients

• You will be invited to sit in a pre-determined circle (as assigned by HealthierHere), to listen to one another’s thoughts and reflect together on a shared question:

• Circle: where participants will sit equally together, creating a space for the group’s thoughts, feelings, insights and threads to weave together a new conversational tapestry of co-creation. Inspiration from contemporary circles have come from our own Native Americans.
• Talking Object: Allows for opportunity to stimulate deeper speaking and listening. We do not call it a Talking Stick out of respect for the sacredness that the Talking Stick represents in Native American culture
• Agreements: guidelines (next slides) that provide a common ground for participation. The HealthierHere hosts will support the group to create that safe space
• Rounds: Two rounds will give everyone the opportunity to participate without the pressure to speak, as you can “pass”
• Final Round: Provides a time for reflection and an opportunity to express what was challenging, meaningful, or inspiring for each individual. These take-aways will be captured (anonymously) by HH staff on a flipchart

• Each group will have a HealthierHere host, who will gently keep all on track.
• Round 1: Each person will get 2 minutes each, holding the object, to share their response to the question. In this round, please take the opportunity to introduce yourselves. All others are to only listen (no interruptions, comments or questions).
• Round 2: Each person will get 2 mins, holding the object, each to share their thoughts and feelings on what they heard others say in Round 1. All others are to only listen (no interruptions, comments or questions).
• Round 3: Open conversation for 15 mins. This will be an opportunity to all to ask questions and respond to comments, and build upon what was shared earlier.
• Round 4: Each member will get 2 minutes, holding the object, to share their take-aways. These take-aways will be captured by a HealthierHere host on a flipchart.

• Suspend judgment as best as you can
• Respect one another
• Seek to understand rather than persuade
• Invite and honor diverse opinions
• Speak what has personal heart and meaning
• Go for honesty and depth, without going on and on
The HealthierHere host will gently keep everyone on track along the above agreements and take notes in Round 4.

Write down on an index card your biggest, most innovative idea to:
“Engage frontline staff and clients / patients in planning and usage of a CIE.”
Underneath it, write down:
“What would be the first step to implement this idea?”
How it will work

Shuffle the cards – pass it along several times.
When prompted, score the Big Idea on the back of the index card.
Rate it anywhere between 1 and 5.
(1 is lowest and you do not like it, 5 is highest – you think it’s amazing!)
