Assessing Practice In Pre-Registration Midwifery Programmes
Background
The NMC (2009) have directed that practice in pre-registration midwifery programmes require that student midwives pass allaspects of practice prior to being permitted entry to the professional register held by the NMC. Students who fail practice wereand are still permitted one further attempt only, at practice.
Standards for Student Supervision and Assessment (NMC, 2018)
This document sets out the expectations for the learning, support, supervision and assessment of students in the practiceenvironment. The term mentor will no longer be used and the traditional role of the mentor will change. The new roles tosupport supervision and assessment of students within practice will be that of practice supervisor, practice assessor andacademic assessor. The students will work primarily with their allocated practice supervisor and periodically with the practiceassessor.
What will remain the same?
Students will remain supernumerary requiring the support of a registered practitioner.Induction, learning contracts- reflective practice, formative/ intermediate assessments, action plans and summative/ finalassessments will continue.Students will receive continuous feedback to inform their progress.Midwives will continue to respond to student concerns, safeguarding issues and issues around clinical competencies andprofessionalism.
Roles to support practice learning

It is possible to be a practice supervisor and a practice assessor - but not for the same student
The role of the Practice Supervisor:
Identifies student learning needs at the beginning of each shift and sets realistic goals to achieve these.Seeks learning opportunities to support the student to achieve skills and practice learning outcomes.Uses coaching skills and appropriate coaching models/ and the use of skilful questioning to support student learning.Collects and documents evidence of student performance continuously.Provides verbal and written formative/ intermediate feedback to the student.Discusses the student’s performance with the Practice Assessor.
The role of the Practice Assessor:
To receive feedback from Practice Supervisors/Service Users on student performance.Observes aspects of the student’s practice.Undertakes and records the summative/final assessments.Gives constructive feedback to the student.Uses The NMC Code (2018) to inform all decisions regarding student’s performance and professionalism.

Undertakes preparation or evidences previous relevant experience to undertake their role.Discusses student performance with the Academic Assessor regarding student progression.
The role of the Academic Assessor:
Supports and works in partnership with Practice Assessors and Practice Supervisors to confirm student achievement andprogression.Works in partnership with clinical colleagues to ensure quality assurance and enhancement of practice areas and thestudent experience.Liaises with Practice Assessors, where appropriate, if a student action plan is needed.Supports the Practice Supervisor, Practice Assessor and the student with the implementation of the action plan.Ensures relevant documentation is completed.Will respond to student concerns, and/or Practice Supervisor, Practice Assessor concerns regarding safeguarding issues,clinical competencies and professional behaviour.
Documents to be completed for assessment of practice
NMC Standards for each academic year/level currently marked as pass/fail. This is still completed during the year as the studentattends each practice placement.
NOTES FOR PRACTICE SUPERVISORS AND PRACTICE ASSESSORS
The Practice Supervisor can provide verbal and written feedback to the student for formative/ intermediate assessments(mid-point for individual NMC Standards). Summative/ final assessments should be completed by the Practice Assessorand takes place following consultation with the Practice Supervisor/s. Dates and times when the assessment will take placeshould be mutually agreed. Time should be allocated to gather and co-ordinate feedback from practice supervisors toallow reliable and fair decision making around assessments and progression documents.
Formative/ Intermediate Assessment
The formative/ intermediate assessment is designed to assist the student to identify gaps in their knowledge. It is thePractice Supervisor’s responsibility to meet with the student to provide feedback and to discuss any issues that may impactupon the student’s summative/ final assessments.
Summative/ Final Assessment
The summative/ final assessment will be conducted by the Practice Assessor, following observation of aspects of thestudent’s practice. This will be in collaboration with feedback from the Practice Supervisor on the student’s clinicalperformance and professional behaviour.
Only two attempts at the summative/ final practice assessments are permitted by the NMC, therefore twofails in practice will lead to discontinuation from the course.

Guidelines For Completion
NMC Standards
This document measures the student’s level of achievement within the practice area. The Practice Supervisor can provide verbaland written feedback to the student for formative/ intermediate assessments.
Summative/ final assessments should be completed by the Practice Assessor, and take place following consultation with thePractice Supervisor. In order to provide continuity, the student will access the electronic document for each placement, the aimbeing to complete the document during the year as the student gains experience in all areas of midwifery care.
The Standards are determined by the NMC (2009) and MUST NOT be altered by students, Practice Supervisorsor Practice Assessors.
At the beginning of a placement the Practice Supervisor should orientate the student to the placement area and document thatthis has been completed using the form provided. There is a separate form for non-midwifery placements such as HealthVisiting, Gynaecology, OPD, A & E etc.
As part of the introduction to the placement, the Practice Supervisor and the student should discuss the learning opportunitiesthat are available in the placement area and develop a learning contract that can be discussed and reviewed by the practicesupervisor and the practice assessor.
The statements may be completed at any time during the year, once the Practice Assessor is satisfied that the student hasreached the appropriate level. There is no requirement for the same Practice Supervisor to sign every mid-point / intermediatesection. There is no requirement for the same Practice Assessor to sign every summative/ final section. It is recognised that thestudent will work with different Practice Supervisors / Practice Assessors in practice, and the purpose of this document is that itis completed as an on-going record of achievement.
The levels of achievement are evident in the tables and are detailed at the front of the document. To achieve a ‘pass’, thestudent must reach the required level of performance as indicated. Practice Supervisors and Practice Assessors shouldsign and date the box appropriate to the student’s level.
At the intermediate/ formative assessment the Practice Supervisor should complete the practice report and comment onthe student’s performance following discussion with the practice assessor. The student may also comment on the placement. Ifthe student’s performance falls below the expected level the Practice Supervisor must contact the Practice Assessor, AcademicAssessor and the Practice Education Facilitator (PEF). A remedial plan of action should be developed.
At the summative/ final assessment, completed by the Practice Assessor, the comments should include identification ofoutcomes that could not be met if the experience was not available, although where possible care may bediscussed/simulated and an appropriate comment made (see examples within the documentation).
If the student’s performance falls below the accepted level at the summative/ final assessment, the student will be deemed tohave failed in practice. The Practice Assessor must contact the relevant Academic Assessor and the Practice EducationFacilitator (PEF).
If the student fails in practice usually one further attempt is permitted following ratification by the module assessment boardand a period of re-allocation to the practice area. Students will be informed of the dates for submission of assessmentdocumentation by their Personal Academic Tutor (PAT).

The following guide to assessment and documentation were produced following re-validation of the programme in May 2012. Astep by step guide has been included to help Practice Supervisors and Practice Assessors.

Placement Aims and Learning Outcomes
Accident and Emergency Aim:The aim of this placement is to develop your knowledge and understanding of teamwork in relation to the management of emergency situations such as cardiac arrest.Also this placement aims to give you experience of women's health conditions whichmay present via the Emergency Department such as ectopic pregnancy andmiscarriage.
Learning Outcomes:At the end of this placement you will be able to:• Identify key aspects of effective team work within emergency situations.• Discuss the patient journey through the Emergency Department for a womanpresenting with a pregnancy related health problem• Identify strategies for caring for and managing people who are dealing with stressfuland distressing situations• Discuss the importance of effective communication and documentation withinemergency situations.
Out-patients/medical unitDepartment
Aim:The aim of this placement is to develop your knowledge and understanding of themanagement of long term medical conditions. You should try to gain as much insightas possible into the effects of medical conditions on patient's general health, lifestyleand well- being. This should also include the long term management of patients withmedical conditions. Some medical conditions which you should try and learn moreabout whilst on placement include: Diabetes; Hypertension; Renal; Cardiac; Blooddisorders including coagulation disorders(This is not an exhaustive list)
Learning Outcomes:At the end of this placement you will be able to:• Discuss at least 3 medical conditions including signs, symptoms, monitoring andbasic management of the conditions.• Discuss the long term effects of at least 3 medical conditions on a patient's longterm health and lifestyle.• Identify key health and social care professionals who may be involved in the patientjourney for individuals with long term medical conditions (3 different conditions).

Neonatal Unit Aim:The aim of this placement is to develop your knowledge and understanding of themanagement of babies within the Neonatal Unit (NNU). This will also include supportof the parents of the baby. You will develop a clearer understanding of the role of themulti-professional team in caring for and managing babies within the unit and the roleof the midwife within the team.
Learning Outcomes:At the end of this placement you will be able to:• Discuss at least 3 different conditions which may require a baby to be admitted tothe NNU.• Identify key health and social care professionals who may be involved in the care of ababy (and his/her family) within the NNU• Outline the role of the midwife in supporting parents who have a baby in the NNU• Discuss the importance of good communication between the midwifery team andNNU giving at least 1 example to demonstrate this.

Aim:The aim of this placement is to develop your knowledge and understanding of the management of long term medicalconditions. You should try to gain as much insight as possible into the effects of medical conditions on patient's general health,lifestyle and well- being. This should also include the long term management of patients with medical conditions. Some medicalconditions which you should try and learn more about whilst on placement include: Diabetes; Hypertension; Renal; Cardiac;Blood disorders including coagulation disorders(This is not an exhaustive list)
Learning Outcomes:At the end of this placement you will be able to:
Discuss at least 3 medical conditions including signs, symptoms, monitoring and basic management of the conditions.Discuss the long term effects of at least 3 medical conditions on a patient's long term health and lifestyle.Identify key health and social care professionals who may be involved in the patient journey for individuals with long termmedical conditions (3 different conditions).

Assessments - A Step By Step Guide
Midwifery Placements

Preliminary interview The student should have entered the details of the placement name and the datesinto the NMC Standards document. The Practice Supervisor should sign and printtheir name on the form provided to create a record of all those who have assessedthe student.• Go through the Placement Orientation form – both student and Practice Supervisorsign it.• Discuss the experience available in the placement.• Discuss possible dates for formative/ intermediate and summative/ finalassessments.
Formative / Intermediateassessment
Practice Supervisor to discuss the student’s progress so far and complete a formative/intermediate practice report form. As a Practice Supervisor, if you have concernsabout practice then an action plan should be completed, in conjunction with thePractice Assessor. The Academic Assessor and the PEF should also be informed.The Practice Supervisor can complete the individual mid-point NMC Standards andmust feedback on the students’ progress to the Practice Assessor.
Summative / Final assessment Practice Assessor will complete a final/ summative report following direct observationof the student and discussion with the Practice Supervisors. If you have concernsabout practice then an action plan should be completed, and you should inform theAcademic Assessor and the PEF.• Sign off any items in the NMC Standards document if appropriate.• Document a summary of your discussion on the discussions page.
End of year assessment At the end of years one and two the Progression form within the NMC Standardsdocument must be completed by the Practice Assessor, who should have access to allpractice report forms completed during the year. If appropriate, an action plan shouldbe completed.At the end of year three the Progression form at the end of the NMC Standardsdocument must be completed, by the Practice Assessor who should have access to allpractice report forms completed during the programme. The above progression andcompletion points will be confirmed by the Academic Assessor.

The aim of this 1 day spoke placement is to enable you to review and extend your knowledge around breast feeding problems.This will facilitate reflection on your experiences to date.

Please read the following instructions carefully to help you complete all the NMC Standards that need to be achieved for thestudent thoughout the course of the year.
The student is required to be assessed twice during this period (Mid-Point and Final). At each point you will need to completeevery NMC standard so that their progress can be reviewed and the students learning plan tailored to ensure all skills havebeen completed.For each page read the information regarding the standard and choose an option from the drop-down list that best suits thestudents progress to date. The options are:

If the option selected is Not Available - Discussed, this means that this experience was not available in practice for thestudent midwife, so this essential skill was discussed/simulated to the appropriate level.
