Criteria for referral
In a mainstream setting, the child is

Brief Description of Service
The Early Years Area SENCO will support the PVI nursery setting SENCO to support children with any additional needs in their nursery. This support includes observations of the children in nursery, offering advice and supporting strategies and attendance at child-review meetings with parents and other partner agencies. The Early Years Area SENCO will also support the transition process for the children she is involved with when they transfer to a school nursery or into a reception class. The Early Years Area SENCO works borough-wide within all areas in Knowsley.
Criteria for referral
The child will need to attend a PVI nursery in Knowsley.
The setting SENCO will need to ask permission from parents’ and a parents’ signature is required to request the involvement of the Early Years Area SENCO.

Names and contact details of any other services already supporting the child and his or her family, for example Health Visitor, Portage, or Children’s Centre involvement.

Brief Description of Service
We are a Community Paediatric Team based in Alder Hey and we are part of a wider team within the Integrated Community Services Clinical Business Unit of Alder Hey Children’s Hospital NHS Foundation Trust. We cover the whole borough of Knowsley.
We provide statutory services to the Local Education Authority and the Local Authority Adoption and Fostering Service. We also provide support to health visitors, school nurses and community paediatric nurses with regards to children with developmental problems, complex health problems and disability.

Any additional evidence to show differentiation action plans or any reasonable adaptations to the nursery environment already in place to support the child.

Brief Description of Service
Stay and Play sessions - fun - filled sessions where you and your child (0-4yrs) can meet others and engage in quality play. We have songs, rhymes, games, creative activities and much more.
SEND - activities are all-inclusive. Services can be tailored to provide one- to-one support in the home or stay & play sessions made smaller and families attend by invite only.
Free bookable sensory rooms in all areas
Infant Massage - promotes bonding between parents and their babies. Baby Massage helps alleviate difficulties such as wind, colic, constipation and irregular sleeping
patterns.
Bosum Buddies - an informal parent - led support session for mums and mums to be who are thinking of breastfeeding.
Weaning - this is an awareness session.

Sleep Pathway - need advice on sleep routines? Or is sleep or lack of sleep becoming difficult for your child? Please drop into our clinic for advice on realistic expectations of sleep and how to create good habits to gradually support change and ensure that everybody gets a good night sleep.
Speech and Language Pathway - this pathway offers language support to parents/carers who have concerns around their child’s speech. The sessions are tailored to meet individual needs and can be delivered in small groups in local children’s centre/ nurse-ries or on a one to one session within the home. Sessions include fun and exciting activities for children to take part in. All activities are planned to offer parents/carers ideas and support to be confident in encouraging their child to achieve their full potential in Communication, Language and Literacy.
Freedom Programme - this programme is aimed at providing women with knowledge around the issues of what domestic abuse is and how perpetrators use a variety of different tactics to control them.
Early Years Sessions - a variety of targeted play and development sessions.
Language Enrichment Programmes - this pathway offers language support to parents/carers who have concerns around their child’s speech. The sessions are tailored to meet individual needs and can be delivered in small groups in local children’s centre/ nurseries or on a one to one session within the home. Sessions include fun and exciting activities for children to take part in.

All activities are planned to offer parents/carers ideas and support to be confident in encouraging their child to achieve their full potential in Communication, Language and Literacy.
YO Preventative Family Support - low level support for families receiving 2 Year Offer who do not meet the criteria for a Family First referral. The support offered will address issues such as basic routines, emerging concerns around behaviour issues and coordination of or signposting to additional services for the child or family.
Universal Plus - short term targeted intervention around a particular area such as toileting, sleeping , developmental concerns, post natal depression etc.
Level 3 – Universal Partnership Plus
Family First Support - Family First is a borough wide support service for families with children aged 0-11 and Stronger Families is a borough wide support service for families with children aged 11- 18. These services support families where there are significant concerns about the impact of the family’s situation on the children’s lives. Family First is focussed on families with needs which require a (CAF) but don’t meet the thresholds for specialist services such as Children’s Social Care, Youth Offending Service and Child and Adolescent Mental Health Services (CAMHS).
The criteria for the service are outlined below:
Families with children aged 2 and under;
Families where parents have mental health difficulties;
Parents with substance misuse concerns (drugs and alcohol);
Families where domestic abuse is a concern;
Families where there is significant criminality.

Brief Description of Service
We are a team of seven Educational Psychologists. We are friendly and experienced professionals who work in all Knowsley schools and nurseries. Our job is to help plan what your child may need when they are in nursery or school. We listen to parents’ views of their child, observe and assess the child and talk to nursery staff and anyone else who is involved.
We help the nursery put together a plan of action to help the child, then come back to see how it is going and plan the next steps. We aim to be part of the process of working together in a team with your child’s needs at the heart of what we do.

Criteria for referral to the service
We need to know that parents have been involved and give informed consent to our involvement, so we must have a signature from someone with parental responsibility. If the parents have joint responsibility, both should be involved but it’s OK for one to sign.
It’s always helpful to include any reports or letters from other professionals, with parental consent.

Please ensure that parents’ contact details are up to date.
Please give the email address, phone number and postal address of the nursery and the name of the SENCo.

Please reassure parents that the Educational Psychologists want the best for their child and to work together with nursery and parents. We are all experienced professionals but we believe that parents know their children best and we know how important it is to listen to them and the nursery staff who work with them every day.
Criteria for referral to the service
We need to know what has already been tried, so we ask for at least two Play Plans that have been evaluated and adapted according to how the child has responded.
We need a general indication of what areas of need the child is experiencing, so we ask for the
Early Years Profile.

The Early Support Programme provides
a standard framework and set of
materials that can be used in many
different circumstances, and a set of
expectations about how services
should work with families. Families are
held at the heart of discussion and
decision-making about their children.
The programme aims to better-
integrate service planning and
delivery.
Families with young children with
complex medical needs,
disabilities or on the autism spectrum
are often in contact with many different
services and agencies, so should
benefit from co-ordinated multi-agency
working, which at the heart of the
programme.

Brief Description of Service
We provide an integrated service for
children aged 0-19 across the
St Helens and Knowsley area. We are
working hard to develop a consistent
way of working across the three
disciplines. We are all involved with the
assessment and intervention of children
with a variety of difficulties.
We work within a variety of settings
including mainstream and special
schools, children’s centres, hospitals,
nurseries, clinics, and within the home
environment.
Physiotherapists work towards
maximising a child’s physical potential.
Occupational Therapists promote
independence in all areas of activities of
daily living.

Integrated Therapy Assistants carry out
assessment and intervention under the
guidance of the Therapists. They work
across all three disciplines.
Criteria for referral
Parents can self-refer to the service via
phone, fax, email or letter with concerns
that impact on to the day-to-day
functioning of a child or young person.
Professionals can refer to the service
from Health, Education and Social
Services and the voluntary sector.
Parental consent / knowledge of the
referral is obligatory but a parental
signature is not mandatory.
The person referring needs to complete
a single assessment referral form that
can be completed via phone, email or
post. Admin staff can forward this form
or assist in the completion of the form
over the phone.
The service is for children and young
people who are registered with a
Knowsley or St Helens GP. The child or
young person needs to reside in the
borough and attend a school within the
borough. If a child resides and is
registered in-borough but is schooled
out-of-borough they will be seen if they
have a home-based need. If they have
a school-based need then the referral
will be sent to the neighbouring service.
Looked-after children under the care of
Social Services will be seen
out-of-borough if appropriate with
liaison with local services dependent on
need and appropriateness.
Out-of-borough visits are kept to a
minimum and typically take the form of
a one-off advisory visit or handover
session to local services.

Include any relevant medical history,
diagnosis, concerns and how the
problem impacts on the day to day
functioning of the child. Up-to-date
contact details are very important along
with details of parental responsibility.
Social services involvement if known is
useful. Please include any relevant
reports or further information that will
support the referral.
Teachers /Education staff accessing the
DCD service should include the
screening tool checklist for motor
coordination difficulties given to staff
following the In Syn training.

Families that struggle to engage in
services can be supported to following

Brief Description of Service
Portage is a home-visiting educational
service for pre-school children with
additional support needs and their
families.
The aim of Portage is to support the
development of young children's play,
communication and relationships and to
encourage full participation in
day-to-day life within the family and
beyond the home. Support offered
through Portage is based on the
principle that parents are the key figures
in the care and development of their
child and Portage aims to help parents
to be confident in this role whatever
their child's needs may be.
Portage offers a framework of support,
which respects each family and their
own individual priorities. It is a model
that adapts flexibly to individual child
and family needs. Families are visited
weekly or fortnightly at home by trained
Portage Early Years Officers. A Portage
Developmental checklist is used to
identify strengths and goals for future
learning.
Criteria for referral
To access the Portage Service the child
needs to have a delay in more than one
area of their development.
Referrals can be made by professionals
or parents. Referrals are through the
Child Development Team, with the
consent of the parent/carer. The parent/
carer must sign the Service Request so
that they are fully aware of the referral.
