Introduction
Aged & Community Services Australia (ACSA) is the peak body representing aged and community
care providers in Tasmania. This Aged Services Sector Graduate Nurse Transition Program is facilitated
by ACSA of behalf of our members and provides opportunities for both full time and part-time
employment on a Statewide basis.
Aged Services is one of the fastest growing, and rapidly developing, industries in Australia providing
secure employment opportunities with structured career pathways. The Sector in Tasmania
recognises the importance of investing in the future of the industry and developing first class clinicians
and leaders who specialise in caring for older people.
Aged Services appeals to people who are looking for a career that is
complex, challenging, and person-centred: giving you the opportunity
to develop good relationships with clients and their families and cultivate
a teamwork approach with your peers and other health professionals. It
also offers the opportunity for leadership and influence in the field of
Aged Care.
Our program
This innovative program has been designed to support and mentor newly graduated nurses who are
transitioning to work to become confident, safe, competent and professional practitioners.
The program is focused on supporting graduates to become the best aged services nurse they can
be. The graduate will be exposed to various areas of the aged services sector.
As a graduate entering into the Aged Services Sector you will be supported by a workplace Mentor.
From the time you commence in the workplace your Mentor will work with you to ensure that you
are appropriately supported and trained on how to properly assess clients, manage your workload,
formulate therapeutic relationships using person centred care, further develop your clinical skills and
the many other tasks that will part of your day to day activities.
As well as the organisational support you will be supported by the ACSA Facilitator, have access to
an online collaboration site/forum and through monthly sessions in Campbell Town you will develop
a network with other Graduate Nurses who have chosen Aged Services as a Career Pathway.
Duration
The duration of the program is 12 months including structured education and professional
development. Terms of employment vary depending on the organisation you are working with and
may range from permanent full time through to a part-time contract for the term of the program. It
is recommended to organisations that part-time employment be no less than 20 hours per week.
Those participating in the program will have the opportunity to be part of a ‘Clinical Honours
Program’. After the completion of the first 12 months our Sector strongly encourages and will actively
support the completion of a post graduate qualification related to aged care. Scholarships may
also be available.
The chances of those who successfully complete the program of maintaining long term secure
employment in the Aged and Community Care Sector are extremely high.

Education & Professional Development
Structured professional development days and structured workplace
coaching and mentoring in a supportive environment, enables the
graduate to build their knowledge skills and career confidence.
Education and Professional Development focuses on the following key core development areas:

First Class Support
Mentors - Your Mentor will be an experienced registered nurse and will guide and support you on a
daily basis towards achieving your development goals. You will meet regularly with your Mentor to
discuss your progress, share your experience and plan your continued development.
Graduate Network - Participants in the program will become part of the Graduate Network. Through
regular Network Meetings and Online Forums there will be the opportunity to share experiences, gain
support and learn from each other.
Industry Expertise - Through ACSA’s members, and our extended network, you will have access to
experienced aged care practitioners with a range of specialist skills and knowledge. Many of our
leaders and experts are Registered Nurses who have worked across a range of health areas and
have chosen aged care for their career and they will welcome the opportunity to share their
experience with graduates and help them to explore the career paths available to a Nurse in the
Aged Care Services Sector.

Transition to Practice Transition, may be defined as ‘the period of learning, adjustment and socialisation, when the nurse
applies, consolidates and increases their existing knowledge, gaining competence (knowledge, skill
and attitude) that is applicable to the nursing practice of the clinical setting or resident/client
population in which they are expected to perform’ (Patterson 2005). In essence, transition is the
process where the nurse gains confidence and demonstrates competence in a new work setting.
In a transition period Nurses will progress from the induction phase to the consolidation and
development phases.
In nursing, transition has co-dependent practice and professional components. Transition to practice
requires educational support to consolidate theoretical and evidence based knowledge. The
transition nurse has a mentor who facilitates experiential learning and skill acquisition in a supportive
and collegial professional environment. Professional transition to the new role is dependent on a
process of psychological transition. In a new environment and role, whilst the nurse may have chosen
the challenges, it is also recognised that high levels of stress due to a conflict in personal/professional
values, loss of familiar ways and expertise are likely to threaten confidence and endanger her/his
professional identity. The Mentor acknowledges the numerous emphases of professional transition and
takes into consideration both personal and professional factors impacting on the nurse.

Feedback
Timely feedback is a valuable strategy to assist a nurse in meeting his or her professional objectives.
For the Transition to Practice Nurse, formal written feedback meetings are scheduled:

Additional feedback meetings may be initiated by the Transition to Practice nurse, the mentor or the
Director of Care where required. A documented record from these meetings is an important
inclusion in a Professional Portfolio.
The formal meeting is attended by the Transition to Practice Nurse, the mentor and in some cases
the Director of Care.
Prior to the meeting the Transition to Practice Nurse:
reviews their Development Plan and updates where required
reviews and updates their Professional Portfolio for reference at the meeting
Prior to the meeting the Mentor/DOC:
consults with colleagues to ensure that they are confident in feedback to be provided.
The meeting structure will enable participants to collaboratively discuss, detail and document:
identified strengths of the Transition to Practice Nurse
identified areas where further development is required
objectives to be achieved over the next period of clinical practice
reference to the Professional Portfolio, to ensure all aspects are up to date and reflective of
achievement.
It is essential that at the initial meeting (within 2 weeks of commencement) that dates and times are
set for all formal feedback meetings.

of the completed record should be forwarded to ACSA as outlined in ‘Key Dates’.

Development Plan The Aged Services sector recognises the need to support you as a newly graduated nurse as you transition to your role in an Aged Services Setting.
The transition period incorporates specific professional and practice activities which include:
identification and attainment of mandatory competencies
individual Development Plans
participating in the professional development education that is specific to the new nurse’s role
participating in scheduled feedback meetings with key support staff.
Participants in the Transition to Practice Program are supported with a continuous professional development program specifically designed for the
unique requirements of new nurses in an Aged Services Sector settting:

undertaking individual learning activities (e.g. organisation in-services sessions, developing a professional reading portfolio)
identification of individual objectives and a development plan for documenting achievement

Developing a Professional Portfolio
The Nurse as a Health Care Professional
Nursing identifies itself as a profession and with this there is a requirement to maintain professional
characteristics and responsibilities. The professional nurse:
has a recognised and unique body of knowledge and associated specialised skills;
has an ‘ideal of service’ and is prepared to exercise their knowledge and skills in the interest
of others;
has membership of a strong internal organisation (licensure) and a well-developed group
consciousness (e.g. code of ethics, conduct) leading to self-governance and control over
education and practice;
in essence, is practical in execution rather than theoretical; and
has a high level of individual responsibility and accountability.
The professional nurse has an obligation to engage in lifelong learning, to commit to further
education to maintain competence in their chosen areas of practice, and to continually strive to
improve standards of nursing care throughout their working life.
The nursing and midwifery professions maintain competence through continual self-assessment of
practice performance and engaging in relevant professional development activities. Personal and
professional growth occurs through continuous updating of knowledge and skills throughout the
entire nursing or midwifery career.
The Professional Portfolio
Health professionals have a responsibility to maintain their competence to practice and to provide
evidence of fulfilment of that responsibility. The evidence is most easily represented in a Professional
Portfolio (Portfolio). It is through the Portfolio documentation that the health care practitioners may
demonstrate the continuing acquisition of skills, knowledge, attitudes, understanding and
achievement. A Portfolio can reflect the past and present, covering all aspects of professional life.
As well as evidence of competence to practice, the Portfolio provides:
a central location for recording and storing professional documents;
useful information when participating in professional development planning;
material which will assist in addressing selection criteria when applying for a position; and
a recognised means of recording professional activities.
The Portfolio may include:
most recent curriculum vitae;
copies of academic transcripts;
documents related to significant professional career development;
copies of annual performance development agreements (including goal setting and
achievement and clinical feedback records);
copies of selected work that has been completed for the practice area e.g. patient
information pamphlets, quality improvement activities;
evidence that research is incorporated into practice; and
mementos which demonstrate appreciation and valuing of practice.

Continuous Professional Development activities may be included, such as:
records of completion of mandatory assessment and education requirements;
records of educational sessions attended, including study days and in-service sessions;
flyers and programs from study days, workshops and conferences attended;
evidence of involvement in professional activities e.g. membership of committees, projects
team membership; and
evidence of reflective thinking such as identifying where improvements can occur, or
changes of confidence in practice has or can occur.
Legal Requirement for Evidence of Competence
Under National Law all health practitioners are required to be registered with the Australian Health
Practitioners Registration Agency (AHPRA) AHPRA.
Nurses and midwives must meet prescribed Nursing and Midwifery Board of Australia (NMBA)
registration standards NMBA Registration Standards and comply with codes and guidelines regarding
the expectations of professional nursing practice NMBA - Codes and Guidelines.
All nurses and midwives must meet the minimum continuing professional development (CPD)
standards prescribed by the NMBA NMBA Continuing Professional Development Standard in order to
renew their registration. The NMBA may request evidence of competence to support renewal of
registration and a Portfolio is the recognised format.

Reflection and Practice Narratives
Practice Narratives are useful for:
Documentation of experiences that occur in practice
Recording critical reflection and analysis
Facilitation of feedback
Recording progress in the development of skills
Providing discussion points during the scheduled Practice Review meetings
Developing learning objectives and goals
The Practice Narrative Record
A Practice Narrative Record should be completed when any significant experience has occurred.
The process of reflecting-on-practice analyses one’s self-awareness as a professional.
What constitutes a significant experience?
An experience in which you feel your interventions really made a difference in
resident/client outcome;
An experience that went unusually well;
An experience that did not go as planned; or
An experience that was particularly demanding
What to record
The narrative should include a record of the situation, followed by reflection on the event; detailing
the significance of the episode; identified highlights; where deficits were detected and how things
could be improved in the future.
Provide a brief description of the context, i.e.: where, when and what happened, a
description of action/s taken, the significance of the episode and identification of any issues
or concerns involved. Include reflection regarding:
o What thought processes were taking place at the time?
o What feelings were experienced during and after the episode?
o What was most demanding about the situation?
Describe the actual or potential outcome of the event, and consider what could have
been done differently or what action should or will occur as a result of this experience?
The narrative can be related to the relevant ANMC National Competency Standards for the
Registered/Enrolled Nurse with a record of the element number of the competences that the
episode related to. One episode may be fairly complex and contribute to the demonstration of
several of the competencies.
