Guide, your individual agencies may have handbooks, guides, and established policies that will help you during your investigation.

• It’s important to remember to use the appropriate

guide for your agency as there are specific processes and formats that need to be followed.

• There may be other agency review processes that

exist i.e. Accident Prevention Analysis, Facilitated Learning Analysis; this course only addresses the Interagency Serious Accident Investigation process.

• From previous classes, we have learned that you will have questions early in the session that are answered in lessons later in the course.

• This can be frustrating for you if all you hear from us is “We‘ll get to that question later.”

• We want to give you a quick overview of the course so you can understand how the investigation process works and where your questions will be answered.


• We have developed an accident investigation process chart. It is located on the wall for you to reference during the class and we have included a copy on your Flash Drive.

• The course is chronological in nature, so it

takes you from start to finish in a sequential manner, as you would experience it during an investigation.

• Here is the process (Exhibit 1-1):

or have personal accident investigation experience. They can tell you what it is like to conduct accident investigations because they have been there.

can be improved. There will be daily critiques and a final course critique. Please provide us with any constructive feedback that will assist us in improving the course.

• Daily Critiques - We review your critiques each evening and will report back to you each morning on any issues/questions. We Consider these during out AAR.

• Final Course Critique - We also conduct an “after action

review” at the end of the course. We review your comments and suggestions to make future changes and adjustments to the course. We believe that the overall course has been improved due to the valuable critiques given to us by the participants.
