everyday life, they will be supported.

The Scheme will:– Assist individuals to identify goals and aspirations

and to develop comprehensive plans to achieve those goals

– Fund reasonable and necessary supports– Better link individuals to mainstream services and

assist services – Direct investment towards evidence based

interventions– Assist in reducing social constraints of disability
