Our claim forms do the following:

Tally up your medical expenses,add a 5% admin fee,add $10.00 processing fee, &add GST only on the fees.

This is becomes your total health care costs for business expenses.

Once you have the total of the health care costs found on the Claim Form, you write a cheque on your business account to

Or you can use our Credit Card Authorization Form, also found on our website

Pack up your claim form, your original medical receipts,

and your cheque or credit card authorization form
