a Do you have a travel history within the past 2 weeks to China, Iran, Europe or Korea
and any other suspected no travel countries?
b Has you body temperature been over 100.0 degrees in the past 14 days?
c Do you have a cough or shortness of breath?
d Do you have any related sickness amongst family members within you home?
e Have you had any contact with individuals who have been to China, Iran
Europe or Korea in the past 14 days?
f Have you traveled overnight out of state recently?
4 Departments / EHS / Admin follows established CDC protocol [ complete COVID 19 Form ]
Suspected Case - Credible or Confirmed5 If credible, associate is a suspected case and placed on leave or works remotely (14 days)

Take your temperature everyday BEFORE coming to work. If your temperature is 100.0 or above, do not go to work. Call in and explain your health situation to Admin.
During shift change it maybe necessary to stand outside to maintain proper social distancing. You will be exposed to various weather conditions, be aware and plan accordingly.
