Trip Release Form Request to Participate: I, the undersigned desire to travel to one or more foreign countries in cooperation with P4Npassions4nations world missions network, a non profit ministry of River Rock Christian Fellowship. On behalf of my self, my heirs, representatives and assigns, I agree to the following in it’s entirety:

Discharge and Release: I hereby release and forever discharge P4Npassions4nations world missions network, River Rock Christian Fellowship, it’s respective members, employees, officers, directors and representatives from and against any and all claims for any and all injuries ( including death), losses or damages I might have or sustain on or in anyway relating to any such outreach activity, any travel to , in and from any foreign countries and the USA, whether before, dur-ing or after any outreach activity, or otherwise.

Acceptance of Risk: I understand that travel, particularly for missions purposes, may involve significant risks and the travel to some areas in the world involves greater health and safety risks than foreign travel in general. I understand that it is my sole responsibility to gather whatever information I need in order to access the risk involved in any travel, stay, or any other activity contemplated by this RELEASE. My signature in this RELEASE and my participation in any such activity means that I have, to my full satisfaction, obtained all the information necessary for me to access the risk for me to participate willingly.

Authority in Emergency: I give P4Npassions4nations world missions network, River Rock Christian Fellowship and it’s representative (s) to request and authorize med-ical and/or hospital treatment for my benefit in the event of any injury or sickness sustained by me while on my such travel, stay or other activity, includ-ing without limitation, while traveling to and from any foreign country or in the USA. I agree to pay for any such treatment and to reimburse P4Npassion-s4nations world missions network, River Rock Christian Fellowship for all costs and expenses incurred by it with respect to such treatment.

Agreement for Conciliation: I am a Christian and I understand that the Bible commands Christians to make every effort to live at peace and to resolve disputes with each other in pri-vate or within the Christian church (see Matt 18:15-20 and I Corinthians 6:1-8) Therefore I agree with P4Npassions4nations world missions net-work, River Rock Christian Fellowship that any claim or dispute covered by, arising from or related to this RELEASE will be settled by Biblical based medi-ation, and if necessary, legally binding arbitration in accordance with the Rules of Procedure for Christian Conciliation of the Institute for Christian Concilia-tion. I agree with P4Npassios4nations world missions network, River Rock Christian Fellowship that such mediation and arbitration will be the sole means available to me for resolving any controversy or claim covered by, arising from, or related to this RELEASE, and that I expressly waive my rights to file a lawsuit in any civil court against one another for such disputes, except to enforce an arbitration decision. Nothing in this paragraph shall present P4Npassions4nations world missions network, River Rock Christian Fellowship from asserting or pleading this RELEASE as a defense in any action or proceeding brought against P4Npassions4nations world missions network, River Rock Christian Fellowship.

BIBLICAL UNITY STATEMENT

THE GOAL OF UNITY The task of finally finishing the Great commission is bigger than any one part of the Body of Christ can accomplish. God wnats to use the entire Body of Christ to finish this task. P4Npassions4nations world missions network, River Rock Christian Fellowship seeks to be an answer to the prayer Christ Himself prayed as reflected in John 17:20-21 ( NASB)

UNITY BASED ON TRUTH P4Npassions4nations world missions network, River Rock Christian Fellowship, seeks to serve as a catalyst for spiritual, biblical unity in the Body of christ as we serve various parts of the Body to conduct evangelistic outreaches, conferences, discipleship, planting churches and bible studies. While we pray and work toward unity, we also are mindful of these words especially in Jesus’ prayer, “ ...that they all may be one, as YOu, Father, are in Me, and I in You.”

STATEMENT OF FAITH Unity must be based on one core truth. Cooperation that results in a dilution or compromise of the core essential truths of the Bible is ecumenism, not Holy Spirit-breathed biblical unity. We seek to work with all those who believe in the following Statement of Faith:

GOD. We believe that the Godhead eternally exists in three persons - the Father, the Son, and the Holy Spirit - and that these three are one God having pre-cisely the same nature, attributes, and perfections.

JESUS. We believe in the deity of Jesus Christ, His virgin birth, sinless life, miracles, death on the cross to provide for our redemption, bodily resurrection and ascension into heaven, present ministry of intercession for us, and His return to earth in power and glory.

HOLY SPIRIT. We believe in the personality and deity of the Holy Spirit, that He performs the miracle of new birth in an unbeliever and indwells believers, enabling them to live godly lives.

BIBLE. We believe in the divine verbal inspiration of the Scriptures. The whole Bible, in the original manuscripts, is without error. As the revelation of God, we believe the Bible to be the sole authority and sufficiency with regard to Christian faith and practice.

MAN. We believe that man was originally created in the image of God. Adam fell through sin, and as a consequence of his sin, lost his spiritual life. This spiritual death, or total depravity of human nature, has been transmitted to the entire human race of man, the Man Christ Jesus alone being excepted. Thus every person is born into the world with a nature that is essentially and unchangeably sinful apart from divine grace.

SALVATION. We believe that, due to universal death through sin, no one can enter the kingdom of God unless born again (born from above). Our redemption has been accomplished solely by the blood of our Lord Jesus. We believe that the new birth of the believer comes through faith in Christ.

The following elements are crucial to the effectiveness, quality and safety of our P4Npassions4nations world missions network, River Rock Christian Fellow-ship, missions trip together.

AUTHORITY ISSUES AS a member of the P4N missions team, I agree to

1. RESPECT my team leader (s) and his/her decisions. 2. RESPECT the leadership and decision making of local and national ministries

HOST AND NATIONAL ISSUES As a member of the P4N missions team, I agree to

1. RECALL that I am a guest who is working at the invitation of one or more local pastors. 2. REMEMBER that I have come to learn as well as to impart. I may run across procedures that i feel are inefficient or attitudes that I find closed - minded. I

will resist the temptation to inform our hosts about “how to do things.” I will be open to learning other people’s methods and ideas. 3. RESPECT our hosts’ views of Christianity. I recognize that christianity has many faces throughout the world, and I agree that a purpose of this trip is to wit-

ness and experience faith lived out in a new setting. 4. DEVELOP and maintain a servant’s attitude toward nationals, my team leader, and my teammates. 5. RESPECT the work that is going on in the country (ies) with the particular church (es) or person (s) with whom we are working. I realize that our team is

there for just a short while but that the local church is there for the long term. I will respect their knowledge, insights, and instructions. 6. REFRAIN from negative political comments or hostile discussions concerning a host country’s politics. 7. AVOID any activity that could be construed as a romantic interest toward a national. I realize certain activities that seem innocent in my own culture may

seem inappropriate in other cultures. As a general rule, I will not hug the opposite gender, spend time alone with the opposite gender or make other gestures that may be misunderstood.

8. ABSTAIN from the consumption of alcoholic beverages, the use of tobacco, and any un-Christlike words or behavior while on the mission field. Again, I un-derstand that other cultures are often much stricter or relaxed than my own in their expression of Christianity.

ATTITUDES AND GENERAL TEAM ISSUES As a member of the P4N missions team, I agree to

1. REFRAIN from complaining under all conditions. I know that world travel can present numerous unexpected and undesired circumstances, but the rewards of conquering such circumstances are innumerable. I will try to be creative and supportive in my endeavors and attitudes.

2. REFRAIN from gossip. I my be surprised at how each person will blossom when freed from the concern that others may be passing judgment. 3. REMEMBER not to be exclusive in my relationships. If my sweetheart or spouse is on the team, we will make eery effort to interact with all members of the

team and not just one another. 4. RETURN to the states for any reason will be at my expense and will in no way leave P4N, River Rock Christian Fellowship, INC, and leaders or members of

P4N, RRCF or any missionary team members, hosts or nationals, liable for the cost. This will include but is not limited to return because of violation of P4N Missions Contract or personal reasons beyond the control of P4N or River Rock Christian Fellowship, INC

I understand and agree to comply with the team policy as outlined above. Infraction of these guidelines can be grounds for termination of the missions trip and being sent home at my own expense.

TEAM CONTRACT

By signing this contract, I am indicating my agreement with each of the following.

FUND RAISING I would like to participate in a P4N missions trip, and I plan to earnestly obtain the funds necessary to do so. I understand that a plane ticket does not guarantee a place on the ministry team. The entire amount of trip must be paid for at least 30 days before departure. I agree to earnestly aim towards installment dates to insure that my lodging, airfare and travel accommodations are met on time.

SPECIAL ACCOUNT I realize that all checks for contribution given to me, if the giver wants a receipt that will verify a tax-deductible contribution, should be made payable to River Rock Christian Fellowship or RRCF and sent to me for forwarding to P4Npassions4nations. I understand that in order to qualify, the check must not only “missions” within the check memo section. Those funds will be administered as a personal support account within a fund established at the time the monies are submitted. I realize I am given giving coupons for instructing my supporters so they will not write on the check memo my name.

COST INCREASES I understand that P4N will notify me at the earliest possible date if any of the originally estimated costs of the trip should increase.

NOTIFICATION TO CONTRIBUTORS If I am unable to participate in the trip, I will notify the people who contributed funds in support of my mission trip and let them know that their non refundable funds (those already spent towards the team outreach) will be used for the team outreach, and the surplus support that is left in my account will be credited towards my next trip within a 12 month period from the scheduled trip. After that the donations will be used for future team missions. Your participation will not affect the tax deductible status of their donation.

CHARITABLE CONTRIBUTIONS For the purposes of tax reporting, a gift to River Rock Christian Fellowship P4N is classified as a charitable contribution to the extent permitted by law. Tax-deductible gifts cannot be returned. Consequently, in the event I do not participate in the outreach trip, any financial gifts made in my name will be used in support of other outreach costs.

SELF DONATING Payment for services and products do not always qualify for tax deductions. I understand that I must check with my own tax accountant regarding the tax - deductible status of donating to my own trip.

1. Taking a Missions Trip with Reno Rock Christian Fellowship means that you will be representing Jesus Christ and River Rock and P4Npassions4nations world missions network oversees. Keeping this in mind, those who wish to travel abroad should see it as their duty to voluntarily adhere to certain standards of conduct.

P4N Missionary Dress Code

2. Taking a Missions Trip with Reno Rock Christian Fellowship means that you will be representing Jesus Christ and River Rock and P4Npassions4nations world missions net-work oversees. Keeping this in mind, those who wish to travel abroad should see it as their duty to voluntarily adhere to certain standards of dress code as is applicable to the culture in which we will be ministering. Violation of this dress code will in no way be tol-erated but will result in consultation and if not immediately corrected then grounds for being sent home early at the applicants expense.

Men: On ministry days & nights: casual/dress slacks (no jeans, or shorts), comfortable ca-sual/dress shoes (no flip flops, sneakers, sandals or bare feet), casual/dress shirt, short or long sleeved button up or pull over golf type shirt, P4N logo or Host shirts provided (no t-shirts, bare chests, tank tops) On days off - in public: jeans, shorts ok in public, sneakers, sandals, sleaved t-shirts which edify the church and christianity, bathing suit trunks only when swimming at beach or at a pool, (not to walk around the town, no tank tops or bare chests) At the compound: jeans, shorts, sneakers, sandals, t-shirts that edify the church and christianity, ok. (no bare chests, unless swimming or sunbathing)

NOTE: Different cultures will demand different clothing for ministry nights, ministry days and days off. Please note that when paying for your own missions trip costs, it will NOT be considered a charitable donation because goods and services will be provided to you. Well, shall I say “Here I go again”? I have been asked to lead two separate teams and join with Women with a Mission to the nations this year.

Your generous financial gifts and your ongoing prayers in the past have enabled me to fulfill my calling and bring the gospel around the world. Again, I thank you so much!!

This year I am excited to move forward towards my upcoming 2012 missions:

Cameroon Africa – Sept 9 - 22, 2012 - $3900.00 This mission will be to take a team of a minimum of 10 men & women to Cameroon & Liberia, Africa (up in the North West) We will be preaching the gospel to thousands of Africans throughout the unreached villages, in open air arenas, the market places, hospitals, schools and jails as the Lord opens the doors for these outreaches. Many bible studies and new churches will be started as a result of these meetings. The trip will end with a powerful Pastor’s Leaders Conference that will equip and activate leaders into a deeper level of ministry and power.

These are opportunities that I don’t want to miss and would love your partnership again!

So this is where you come in… Due to the rise of fuel costs and the devaluing of the american dollar, the costs of these trips have increased more than ever before. I vowed to the Lord many years ago that I would not let the outward circumstances of the world stop me from moving forward in His call on my life, coupled with my passion to win the lost at any cost.
