STUDY CONTEXT
Conditions are often treated in isolation. Individuals are seen by multiple agencies, which is confusing and time consuming for the citizens and results in duplicated processes.
A lack of coordination and information sharing means interventions are not always sequenced to optimize results and not always promoting co-designing and participation.

Task 5: In-depth analysis
To validate the in depth analysis methodology, a specific test case will be performed through a pilot
execution on the three most prominent cases out of 15.
In particular the pilot process will follow the following phases:
The study team, in conjunction with the Advisory Group and with the IPTS will identify the 3 initiatives that will be used as a pilot tester of the in-depth analysis.
The in-depth analysis on these 3 chosen cases will be conducted, applying the defined methodology.
The production of a draft of final report, based on the data related to the pilot initiatives, will be produced.

Promotion of Integrated care. The goal of integrated care systems is to improve the cooperation of health care providers within and between different health care sectors. First of all, integrated care projects attempt (i) to improve and intensify the cooperation between general practitioners (family physicians), specialists, and representatives of other health professions such as physio- or psychotherapists as well as (ii) to optimize the coordination of ambulatory (outpatient) care and hospital (inpatient) care
Contribute to synergies of the various evaluation modules and initiate common activities including, e.g. publications on inter-modular research question. Considerable comparative savings in relation to normal care

Integrated care can overcome the economic inefficiencies of the traditionally fragmented health system, resulting from a poor coordination of service interfaces between general and specialized care as well as between inpatient and outpatient care. By implementing this approach, the initiative has generated a reduction in the operational costs due to a better coordination of health care systems and by integrating the existing delivery resources.

Develop and implement a model by providing services from public tender execution support and employee training through ongoing monitoring, evaluation, and quality control for the program
Known as the ‘express train to employment’, the approach introduced many new or deeply redesigned features to the Polish market such as: • Contracting out employment services to private companies based on the principle of payment for outcomes, under which installments are paid
only when participants find and maintain employment. • Providing freedom to non-public operators to select the engagement mechanisms and tools, beyond a ‘minimum suite’ of services that must be delivered by the operator. • Using careful selection criteria to categorize and select target groups of participants, and thus
focusing activities on those people who have serious barriers to finding employment. • Monitoring the status of project participants after finding employment, which is critical to sustaining the new remuneration model
New model for engaging the long-term unemployed, that opens the labor services market to non-public operators (specialized private organizations that undertake contracts for performing engagement services). The model enables an individualized, and thus more effective approach to engaging the unemployed and increases the efficiency of public spending, as payments are only made when specific outcomes are achieved
