SOCIAL CARE

POPULATION AGING GROWING SOVEREIGN DEBT

HIGH UNEMPLOYMENT IMMIGRATION

HOMELESSNESS SCHOOL ABANDON

Young unemployment (in EU southern countries > 25%, 2014 data)

Labor market: employment remains below its pre-crisis level and it is growing very slow

Soaring of Not employment, education or training index (in OEDC countries 39 million people, 2013 data)

Weak connection between education and training systems and labor market

Job security is weak

Low-skilled workers have less chance of getting a job

A high number of small service providers co-exist within a fragmented system that is difficult to navigate and coordinate.

In many social sectors the services has expanded in an ad hoc and uncoordinated way.

A lack of coordination and information sharing means interventions are not always sequenced to optimize results and not always promoting co-designing and participation.

A lack of coordination and information sharing means interventions are not always sequenced to optimize results and not always promoting co-designing and participation.

A high number of small service providers co-exist within a fragmented system that is difficult to navigate and coordinate.

In many social sectors the services has expanded in an ad hoc and uncoordinated way.

Promotion of Integrated care. The goal of integrated care systems is to improve the cooperation of health care providers within and between different health care sectors. First of all, integrated care projects attempt (i) to improve and intensify the cooperation between general practitioners (family physicians), specialists, and representatives of other health professions such as physio- or psychotherapists as well as (ii) to optimize the coordination of ambulatory (outpatient) care and hospital (inpatient) care

Contribute to synergies of the various evaluation modules and initiate common activities including, e.g. publications on inter-modular research question. Considerable comparative savings in relation to normal care

Providing a coordination of service interfaces between general and specialized care

Creating a common database containing the health data of the patients

INPUT

OUTPUT OUTCOMES IMPACTS ACTIVITIES

Integrated care can overcome the economic inefficiencies of the traditionally fragmented health system, resulting from a poor coordination of service interfaces between general and specialized care as well as between inpatient and outpatient care. By implementing this approach, the initiative has generated a reduction in the operational costs due to a better coordination of health care systems and by integrating the existing delivery resources.

Overview of some promising cases: Welfare to Work (Poland)

Objectives: Engaging the unemployed differently according to their "distance" from the job market (i.e. long-term unemployed may be more distant than recently graduated) with information to find and maintain a job Dimension: Regional Beneficiaries: Unemployed people Funders: Local government European Community Providers: Government of Malopolska Region

Develop and implement a model by providing services from public tender execution support and employee training through ongoing monitoring, evaluation, and quality control for the program

Known as the ‘express train to employment’, the approach introduced many new or deeply redesigned features to the Polish market such as: • Contracting out employment services to private companies based on the principle of payment for outcomes, under which installments are paid

only when participants find and maintain employment. • Providing freedom to non-public operators to select the engagement mechanisms and tools, beyond a ‘minimum suite’ of services that must be delivered by the operator. • Using careful selection criteria to categorize and select target groups of participants, and thus

focusing activities on those people who have serious barriers to finding employment. • Monitoring the status of project participants after finding employment, which is critical to sustaining the new remuneration model

New model for engaging the long-term unemployed, that opens the labor services market to non-public operators (specialized private organizations that undertake contracts for performing engagement services). The model enables an individualized, and thus more effective approach to engaging the unemployed and increases the efficiency of public spending, as payments are only made when specific outcomes are achieved

The Specialisterne

concept proves that

people with autism and

similar challenges can be

valuable and worthy

contributors to the labor

market, once they are

understood and get the

support they need to

excel. The traits that

usually exclude people

with autism from the labor

market are the very traits

that make them valuable

employees at

Specialisterne; such as

attention to detail and a

persistence to get the job

done.

They offer a youth

education program and

training program to help

develop each student’s

personal, social and

professional skills,

preparing them for future

placement in the labor

market

Specialisterne seeks to enable one million jobs worldwide for a target population that would otherwise face substantial barriers to employment

At the core of Specialisterne

is a 5 month assessment

program where candidates

with autism go through

different exercises, tasks and

work situations. This

assessment program helps

our staff to see past the

diagnosis to the candidate’s

real personality, skill-set and

potential. Specialisterne

Denmark also operates a 3

year education program for

young adults with autism and

similar challenges. The

purpose of the school is to

create a holistic approach to

education and focuses on

social and cultural

understanding, health and

well-being, social behavior

and skills. The school works

closely with Specialisterne

Denmark’s business partners

and supports students with internships and work-
