Seek medical attention at either The Geary Community Hospital Occupational Health Clinic or Dr. Mace’s office. 2. In case of an emergency, Geary Community Hospital’s Emergency room may also be used. Please note that if you use any other doctor, the KASB Workers Compensation Fund is liable only for an amount up to $500. 3. As soon as possible, report injuries sustained on the job to Mrs. Sharp or myself. 4. Fill out a “Report by Injured Employee” form. This form is available in the Teacher Resources folder located on the JCHS website under Staff Document Center. It is important the “Report by Injured Employee” is detailed and includes date, time, location, eyewitnesses and circumstances surrounding the incident. 5. Provide the completed report to me. 6. Eyewitnesses to the incident will be asked to complete a “Report by Eyewitness” 7. An “Accident Investigation Report” must be completed within 10 days of the incident. The report includes the “Report by Injured Employee” and the “Report by Eyewitness.” This process cannot be delayed and includes an interview of the injured employee. 8. If a doctor places limitations on the staff member’s return to work, the staff member must be counseled by their academy administrator or myself and have an “Employee Counseling Form” completed.
