    %or the @rst desired outcome "patient will have improved kidney

    function aeb a lowered BUN level from the previous day and a continued

    normal creatinine level'# this was partially met because upon admission# the

    patient,s BUN and creatinine levels were abnormally hih and on the day I

    was carin for her# her creatinine level returned to normal and her BUN level

    decreased from her initial admission level. Patient,s lab values will need to

    continue to be monitored for further improvement or worsenin.

    %or the second and third desired outcomes "patient will maintain a

    normal blood pressure and temperature for the duration of her hospital stay'#

    these outcomes were met on the day that I took care of her# but they are

    only partially met for now because she is still oin to remain in the hospital

    for at least several days more. %or each remainder day in the hospital# the

    patient,s vital sins# speci@cally her blood pressure# pulse# and temperature#

    will need to continue to be monitored at least once a shift.

    %or the fourth and @fth desired outcomes "patient will improve in her

    mobility by bein able to walk down the full hallway and back to her room

    and patient will show improvement in her D3s by bein able to feed herself

    with minimal assistance from the sta4 before dischare'# these outcomes

    were both not met by the end of my shift with her. )he was only able to walk

    down half the hallway and the patient was not able to feed herself at all

    without full assistance. 6he patient will need to continue to work with P6 and

    if she was not seein O6 already# a referral would have needed to be made

    for her to work with them as well to improve her strenth and independence.

    %or the last two desired outcomes "patient will show a decrease in her

    symptoms of infection and will be free of bed ulcers for the duration of her

    hospital stay'# the @rst outcome was not met and the second outcome was

    only partially met. %or the outcome of a decrease in her infection symptoms#

    the patient,s symptoms remained the same from the previous day and her

    rash was still very bad. 6he patient,s skin will need to continue to be

    assessed daily and monitored for improvement or worsenin as evidenced by

    what,s written in her assessments in her chart. %or the outcome of bein free

    from bedsores# this was met on the day that I cared for my patient# but since

    she was still oin to remain in the hospital# her buttocks area will need to

    continue to be monitored durin every brief chane to see if this outcome is

    fully met. Bedsore formation will be prevented by continuin to turn the

    patient every ! hours# performin fre0uent brief chanes after elimination#

    and applyin skin barrier as well to the patient.
