We dust off those winter blahs and welcome spring with open arms. New experiences are the spark of life and we here at Sleep Wellness Center in Fargo and Grand Forks are excited about bringing you our new newsletter to brighten your day. We are here to assist you with your patient’s sleep related needs. The newsletter will be for providers and patients. We hope you find the following articles to be informative and helpful. Enjoy! Sleep becomes a team sport When people start to experience sleeping issues big or small we would like to help resolve those issues and move them forward to a better quality of life as fast as possible. If you can sleep, you can heal. The opposite also is true, if you cannot sleep, you cannot heal. If the latter is true for you or your patient, then a cycle of unpleasant experiences begins. This cycle of broken sleep begins to affect their marriages, jobs, friend and family interactions, as well as, their health. When we don’t feel good we do not perform to the best of our abilities or desires. So, the first step toward enjoying a good night’s sleep is getting properly diagnosed. At Sleep Wellness Center, all of our sleep specialists are registered or certified in sleep technologies. We offer a comfortable, home-like atmosphere where your patients can relax and trust that our sleep experts will help you find answers to your sleep issues and relief from whatever is preventing you from getting the restful sleep you need. We have been helping patients since 1994. Get started with a referral Insurance requires your patient to have a physician’s referral to receive sleep diagnostic care and equipment just like they would for any other type of testing. The majority of patients do not need to see a special sleep doctor; all they need to do is ask you, their primary care physician, to complete a referral form for them to see one of our sleep specialists. It will not require your patient to add another doctor to their healthcare team or initiate a string of additional appointments. We’ll simply complete the diagnosis process and coordinate a treatment plan with you, their primary care physician. All insurances accepted Weacceptallinsurances,includingMedicare,andwhenyourpatientisreferredtoSleepWellnessCenter,we’lldoalltheinsurancepaperworkforthem.

What happens at a sleep study Whenyourpatientarrivesfortheirsleepstudy,theirsleeptechnicianwillmeetthemandshowthemtotheirbedroom.They’llhavetimetochangeandgetreadyforbedastheynormallywould.Theremaybesomewaitingperiodswhilewepreparethemonitorsandsleepconditionsduringwhichthey’llbewelcometoreadorwatchTV.Wheneverythingisready,theirtechnicianwillattachmonitoringequipmentwhichwilltakeabout20minutes.Therewillthenbea10–15minutewaitingperiodinwhichweallowtheadhesivestodry.Thenit’slightsoutandthey’llbefreetosleep.

Whiletheyaresleeping,theirsleeptechnicianwillmonitortheirbrainwaves,bloodoxygenlevels,heartrate,andbreathing,aswellasmuscle,eye,andlegmovements.Ifenoughbreathingdisturbancesarerecorded,thesleepstudytechnicianwillawakenthemtotryusingcontinuouspositiveairwaypressure(CPAP)toseeifthisimprovestheirsleep.SomepatientsmaybeaskedtoreturnforasecondsleepstudytotrytheCPAPtherapy.

Thesleeptechnicianswillnotbeabletogivethemanyresultsthenightoftheirsleepstudy.Atthecompletionoftheirsleepstudy,ourteamofsleepexpertswilltaketimetoanalyzeallthedatacollectedfromthesleepstudymonitorsandprepareacomprehensivesummaryalongwiththerapyrecommendations,ifapplicable.Thisprocessmaytakeuptotwoweeks.

Acompletesleepstudyreportincludingrecommendationsforfollow-upcarewillbesenttoyou,yourpatient’sprimarycarephysician.IftheytriedtheCPAPtherapyduringtheirsleepstudy,therearetwooptionsforyourpreferences:1.WesetupyourpatientwithCPAPandtheyuseditfor1monthandthenyouscheduleavisitwiththemonhowitisgoingor2.Youmeetwiththepatientfirstanddiscussfindingsofthesleepstudybeforetheybegintherapy.Wewillcontactyourpatientoncewegetyourpreferenceandsetupanappointmenttogettheirequipment.

program.WeprovideeachpatientwithaplanofcarethatstartswiththePAPtherapybeingsetup.We’llkeepintouchwithyourpatientstomakesuretheirpersonalsleeptherapycontinuestoworkforthem,andyou’llbehappytoknowtherearenochargesforfollow-upofficevisitsorequipmentchecksat SleepWellnessCenter.Wejustwanttomakesuretheyaredoingwell.

CPAP compliance program Forexample,atthetimeofsetupofthetherapydevice,thepatientisenrolledintheSleepWellnessCenter’shometherapyfollowupprogram.Informationprovidedupontheinitialsetupcanbeoverwhelming,whichiswhyclosefollowupissoimportant.StudiesindicatethatthefirstmonthoftherapyisacrucialpartoflongtermcompliancewithCPAPtherapy.Thepatientiscontacted72hoursaftertheinitialsetup.Ifallisgoingwell,wecontactthepatient30daysfromsetupandevery90daysthereafter.Ifthepatienthasanyconcernsafterthefirst72hours,wewillcontacthim/hereveryfewdaysuntiltheproblemorconcernhasbeenresolved.ForMedicarepatients,inadditiontothefollowupnotedabove,willbefollowedmonthlyforthefirst13monthsoftherapy.ThisadditionalfollowupisrequiredtomeetthestrictMedicareguidelines.Plus,weoffer24houroncallservicetomeettheneedsofanyafter-hoursconcerns.

Compliance and monitoring Monitoringefficacyandcomplianceareanimportantfacetofthefollowupprogram.Adatadownloadisperformedapproximately30daysaftereveryCPAP/BiPAPsetup.Areportisthensenttoyou,theprovider,informingyouoftheusageandsuccessoftheprescribedtherapyforyourpatient.YoumaybeaskedtosendfollowupnotestoSleepWellnessCenterfollowingyourvisitwiththepatient.ThisinformationisoftenrequiredbyMedicareorthehealthinsurancetoinsureproperpaymentofthedevice.Ourcurrentcompliancerateisconsistentlygreaterthan90%.

Home sleep studies (HST) Weworkdiligentlytokeepyourfacilityandprovidersuptodateonthelatestchangesintestingandreimbursementinourserviceareas.OverthepastfewmonthstherehavebeensomechangesnotedinOutofCenterTesting(Homesleepstudies,HST)thatmayaffectsomepatientsinourservicearea.

TheAmericanAcademyofSleepMedicine(AASM)hassetinplacetheguidelinesforHST.AsasleepcenterthatisaccreditedbytheAASMasanOutofCenterTestingfacilitywearerequiredtofollowthestandardsthathavebeensetinplacebytheAASM.TherearesomeinsurancecompaniesthatoverridetheAASMguidelines,andhaveestablishedtheirownguidelines.Thesevaryfromcompanytocompany.

TherearecurrentlyonlyafewinsurancecompaniesintheareathatarebeginningtorequireHSTasaninitialstudyfortheircustomers.TheneedfortheHSTismostoftendiscoveredduringtheinsuranceverificationprocessthatwecompleteforthepatient.IfitisdiscoveredthatapatientisrequiredtohaveanHSTwewillcontacttheprovider’sofficetoletthemknowofthischangeinlevelofservice.Thenthepatientwillbecontacted,byourcenter,toletthemknowandarrangementsaremadefortheHST.

WinmarcurrentlyhasthemostuptodateequipmentforperformingHST.Wemeetwiththepatientindividuallytoassureagoodunderstandingoftheuseoftheequipment.

Thereisan“oncall”numberforthepatienttocallincaseofquestionsorproblemsduringthenight.Uponcompletionofthetestingthepatientreturnstheequipmenttoawaitanalysisandinterpretation.

Ü Patientswithahighpre-testprobabilityofmoderatetosevereOSA.

Ü Patientswithnosignificantco-morbidmedicalcondition.Examplesofco-morbidconditionsincludemoderate-severepulmonarydisease(cysticfibrosis,pulmonaryfibrosis,activeasthma,COPD),congestiveheartfailureandneuromusculardiseases(ALS,multiplesclerosis,Parkinson’sdisease).

Ü Patientssuspectedofhavingnoco-morbidsleepdisorderotherthanOSA.

Ü Patientsunabletobestudiedinasleeplaboratory.

Ü Tomonitorresponsetonon-PAPtreatmentsafterthediagnosishasalreadybeenmade.

ObstructiveSleepApnea(OSA)isahottopicofdiscussioninandaroundtheDepartmentofTransportation(DOT),FederalMotorCarrierSafetyAdministration(FMCSA),andFederalRailroadAdministration(FRA)agencies.IntheMarch2016issueofSleepReviewMagazine,article”AASMUrgesFederalAgenciestoDevelopSleepApneaPoliciesforCommercialMotorVehicleDrivers,TrainEngineers,”theAmericanAcademyofSleepMedicine(AASM)urgestheseagenciestocomeupwithpoliciesregardingsleepapnea.TheDOT,FMCSA,andFRAareaskingforpublicinputduringthenext90daysregardingtheimpactsoftreating,evaluating,andscreeningpersonsforOSAwhomworkfortherailroadorarecommercialmotorvehicledrivers.TheAASMcommendsthiseffort.FMCSA,andFRAwillmeetinWashington,D.C.,andLosAngeles,andwillbehostingthreepubliclisteningsessions.TheFMCSAandFRAareconsideringwhethertopurpose

specificrequirements.TodothisajointAdvanceNoticeofProposedRulemaking(ANPRM)hasbeeninitiated.AnANPRMisadocumentthatanagencymaychoosetoissuebeforeitisreadytoissueaNoticeofProposedRulemaking(NPRM).Itmayalsobecalleda"noticeofintent"orsimplya"requestforcomments."TheANPRMisusedbyanagencyasavehicleforobtainingpublicparticipationintheformulationofaregulatorychangebeforetheagencyhasdonesignificantresearchorinvestigationonitsown.TheAASMpresidentNathanielWatson,MD,MScsays“Thecomprehensivedevelopmentandswiftimplementationofevidence-basedsleepapneapoliciesisnecessarytoprotectthewell-beingoftransportationoperatorsandmaximizepublicsafety.”PeoplewithOSAstruggletobreathe,whichresultsinsnoring,canoccurseveralhundredtimeseachnightwhichexplainswhytheirbodyisexhausted,nightafternightevenafter8to10hoursofsleep.

Leftuntreated,sleepapneahasbeenlinkedtoheartproblems,highbloodpressure,diabetes,weightgain,depression,impotence,andevensuddendeathwhilesleeping.Itisestimatedthat25millionadultshaveOSAand20%+arecommercialtruckdrivers.Daytimesleepinesscancausedrowsydrivingwhichiswhytheseagenciesaremovingaheadwiththefirststepsofchange.“Healthysleepandoptimalalertnessareessentialfortransportationsafety,”saysWatson.“AcrosstheUS,thousandsofboard-certifiedsleepmedicinephysiciansareavailabletoprovidehighquality,patient-centeredforpeoplewhohavesleepapnea,whichhelpstokeepourroadsandrailwayssafe.”

Nowthatyouhavehadyoursleepstudyyoumayhavetochangethewayyouaresleepingduetotheresults.Youarenotalone.PeoplewhoarediagnosedwithsomeformofsleepdisorderbreathingmayhavetowearaPositiveAirwayPressure(PAP)device.ThetypeyouwereprescribedmaybeaCPAP,BiPAP,orother,dependingontheresultsofyoursleepstudy.Youmaybethinking“Whyisthishappeningtome?”or“WhydoIhavetowearthismask?”Itishardatfirsttoacceptthistypeofchange.Dependingonhowyouhandleanychangewilldeterminehowfastyouadapttothisnewchangeinyoursleepingroutine.Restassuredwewillhelpyoumakeittotheotherside,whichisfeelingbetterwhenyouwakebecauseyouwereabletowearyourCPAPallnightlong.

DesensitizingisawayyougetusedtousingyourCPAP,sothatitbecomessecondnaturetoyou.Everytimeyousleepyouuseyourprescribedequipmentconsistently.Thiswillgiveyouthebestresults,quicker.How,youmayask,canwegetthisdoneforyou?Thinkofitthisway,ifyouregularlywearawatchonyourleftarmandthenallofasuddenstartedwearingityourrightarm,itwouldfeelweird,right?Thisishowyourmaskandequipmentfeel,initially.Helpingyouacclimatetoyournewequipmentiswhatwedoforyou,ourpatients.Thefirsttwoweeksarethemostimportant.SetpersonalgoalsfortheamountoftimeyouhopetowearyourCPAP,takeabreakifneeded.Themostimportantruleistoputonyourequipmenteverynight.IfyoupickandchoosethenightsyouuseyourCPAPyouwillneverreachdesensitization.Ifyouarestrugglingwithyourpressure,mask,orhumidificationwehavecliniciansonstaffhereatSleepWellnessCenterwhoknowtheinsandoutsofyourequipmentandwhocantroubleshootwithyouinpersonoroverthephone.Wewillcontinuetofollowupwithyoutoensurethedesensitizinghasworkedwellforyou.Wewillhaveyoufeelingbrighteyedandbushytailedinnotime.
