ENROLLMENT FOR 2017-18 FOR IN-DISTRICT 4 AND 5 YEAR-OLDS We are searching for ALL three and four year-olds living in the Lake Country School District to help us project the numbers for the 4K and 5K classes for 2017-2018. If you are aware of a three or four year old in the district who will be 4 or 5 years old by September 1, 2017, please have the parents contact Nancy Kuehl at [email protected] or call 262-367-3606, ext. 108. While we have tracked some through our census, we want to make sure to contact every eligible child. Thanks for your assistance!

LCS earns 1% of receipt totals from Sentry (Delafield) and Piggly Wiggly (both Hartland/Oconomowoc). Simply collect and send receipts to school in a timely manner as there are date limitations. We appreciate your help in this fundraiser. Every receipt makes a difference for our kids and LCS!
Box Tops... Who is clipping for LCS?
Have you ever asked grandparents about collecting Box Tops? How about other relatives/neighbors? It’s a small task that earns funds to help improve LCS. Every Box Top counts, so please spread the word! Designate an envelope and have your children help with the collecting and clipping before sending to homerooms!

Important Details –
PLEASE DO NOT RETURN REGISTRATION FORMS TO YOUR CHILD’S SCHOOL. Forms returned to the school do not guarantee enrollment in Mad Science. Registration is limited and on a first come, first served basis. Avoid disappointment and register early! Please return registrations directly to MAD SCIENCE. Registrations MUST be postmarked or completed by phone at 414-858-9990, or online at milwaukee.madscience.org prior to the registration deadline. Please consult the front of this form for specific enrollment information. Mad Science is held onsite at your child's school. Students will be grouped by like grades when possible. Class sizes range from 12 to 25 children. All supplies and materials are included in the class fee. Each week the kids will receive a take-home item and take-home card that will enable them to continue the fun and learning at home!! You will not receive a confirmation letter. You will be contacted only if there is a problem with enrollment.

To avoid any mix-ups, please remind your child to go to Mad Science after school on class days. It’s also a good idea to send the teacher a note instructing the child where to go. Our instructors are not able to leave the classroom to look for absent children. An announcement will be made on the first day of class, directing the children where to meet. Mad Science is not responsible for children before or after the stated class times. Please pick your child up promptly after Mad Science at the school designated location at dismissal. If you are going to be late, please make arrangements to have your child picked up by someone else or attend after school care if available. Late pickup will be charged after a 5 minute grace period at the rate of $1.00 per minute and must be paid to the Mad Science office prior to the start of the next class for your child to continue. If your child attends after school care, they will be walked there at the end of class.
For refund requests made prior to the start of the program, a $15 processing fee will be deducted. No refunds or prorated fees for missed classes. There is a $30 fee for all returned checks.
If a class is cancelled due to inclement weather or an unexpected school closing, we will attempt to schedule a make-up class. We are unable to offer a refund for classes missed due to unforeseen circumstances. An alternate topic for a particular class may be substituted without notice if necessary.
