How Others Are Evaluating PCMH

Why Not an IDOTS System To Studying the dimensions of PCMH Success?
Current studies indicate PCMH implementation takes four to five years
to yield substantial results and these could come in various forms.
However, many health systems are conducting evaluations well below
this timeframe and may be drawing premature conclusions. As such, we
came up with the It Depends on The Situation or IDOTS system of
evaluation when it comes to answering questions or directing studies
and analyses.
Conducting special studies and analyses may require development of a
“menu” of Input, Process, and Output (IPO) measures. For example,
leaders wanted to know the effect of emergency room utilization with
overall continuity and satisfaction while others may be interested in the
impact of HEDIS or how well leaders are allocating (e.g. promised FTEs,
versus actual) and managed resources.
Developing an IDOTS system based on IPO, senior medical leadership
could clarify the intent and guard against drawing wrong conclusions.

2. Develop an IDOTS special studies plan to assess and evaluate the various aspects of PCMH
3. Remember, it takes 5-7 years for PCMH to mature so don’t draw premature conclusions
a. Let the relationships flourish!
b. Study effect of discontinuity during PCSs
c. Give coordination of care time
4. Promote PCMH to internal and external audiences with fact based success stories
a. Local collaboration with public health and community leaders builds healthier communities!
5. Improve integration of chronic disease management programs such as Diabetes using the AFMS Diabetes Center of Excellence (DCOE)
6. Expand the role of the pharmacists and others to assist with engagement and adherence
7. Integrate digital health and proactive patient engagement tools and principles (especially before and after visits) to create quantum leap success
