Welcome to our thirdedition of Hand in Hand. SecuriCare hashad a superb year which we could nothave achieved without your support. Thishas allowed us to provide invaluableresources to our patients, with Nurses inhospitals and Stoma Care Advisors inthe community.

In this edition you will read about theenvironmental impact of disposing ofpouches and how we at SecuriCare arehelping to combat this with the FreeStyleFlushable™ Pouch. In light of recent‘global warming’ debates this issue isrising in importance. You will also readabout a 90-year-old abseiling colostomist– a good reflection of the ‘go get them’attitude most ostomists have.

A day in the life of a Samples Administrator

Taking pride and joy in her workShe’s fit, she’s slim, and she never sitsdown – that’s Sharon, SecuriCare’sSamples Administrator. Sharon is always onthe go because every working day she hasanywhere between 80 to 120 individualorders for samples to personally organiseand despatch!

Samples of stoma care items are anexcellent way of ostomists being able to tryout new or different products. Sampleorders come in to Sharon’s area in thewarehouse throughout the day – usually viaCareline staff where stoma patients haverung in for advice, or in response toadvertisements, or from stoma care nursesneeding samples for patients.

The samples requested vary from pouchessuch as the new FreeStyle Drainable,™ to awide range of stoma care accessories, e.g.Appeel® no-sting adhesive remover, or LBF®

barrier wipes. Each order is different andSharon appreciates the training shereceives on products so that she knowswhat they are for and how they are used.After printing off the orders and checkingthem out, she picks the products andarranges any special ‘customising’ needed,encloses relevant literature, packs up thesamples and gets them ready for the dailypickup.

There’s organising of stock to carry out tooand Sharon has developed strategies toensure she never runs out and can meet allorders promptly (she says she has ‘waysand means’!) It is her very proud boast thatorders are fulfilled either the same day asthey are received, or if it is late in the dayafter the post has gone then early the

following day. She rarely has one ‘backorder’ and most patients receive theirsamples within 24 hours. Sharon statessimply that ‘I just really like my job; I takepride and joy in the job I do’.

Sharon has worked for the stoma divisionfor nearly 8 years, and was the first femalein the company to gain her fork lift trucklicence. She has a 2-year-old grand-daughter plus another grandchild on theway (Sharon’s daughter has justannounced she’s pregnant with her firstchild).

Problems with your stomaSORE SKIN

This may be due to leakage of faeces orurine commonly caused by an incorrectaperture of the pouch or flange. Thestoma may have altered in size and/orshape possibly due to weight gain or loss.Your local stoma care nurse will reassessthe stoma and adjust the template oradvise changing to an alternativeappliance more suited to your currentneeds. There may be sensitivity to theadhesive and this will show as a red,irritating rash so advice should be soughtfrom the stoma care nurse to try alternativeappliances. The stoma care nurse mayalso suggest the use of a barrier wipesuch as LBF® no sting barrier film whichwill protect the skin. All stomas should bereviewed at least annually to ensureproblems are minimal.

PANCAKING

This is a common problem especially forcolostomists. It is usually caused by thefilter removing excess flatus from thepouch. The pouch then becomes flatagainst the stoma and faeces cannotdrop down into the bottom of the pouch.Ostomists may experience leakage offaeces from the side of the flange. Seeyour stoma care nurse for a review ofyour existing products. Newer, more upto date appliances such as WellandFreeStyle and FreeStyle FlushableTM havemore efficient filters which may causeless of a problem. The stoma care nursemay also suggest the use of a filter cover(supplied in the boxes of pouches) andapplication of a lubricating gel inside thepouch may also help.

There are over 361/2 million stomapouches thrown away in landfill sites eachyear from 150,000 existing ostomates andfrom 34,000 new stomas fashioned everyyear (Black 2003). With some plasticstaking up to 450 years to biodegrade, thishas substantial consequences for theenvironment and global warming.

Ostomates are taught to dispose of theirpouch in three ways:1) Wrapping the pouch and its contents

in a bag (often plastic) and throwing it away into a bin for collection (to landfills)

2) Cutting the bottom off the pouch and emptying out the contents down the toilet, wrapping the empty pouch up and again disposing of it in the bin

3) Wearing a drainable appliance (not suitable for everyone)

Recent evidence from the BritishColostomy Association (Berry 2005) andSwan (2001) found that sixty per cent ofnurses are recommending to theirpatients that they should cut the stomapouch and squeeze out the contentsdown the toilet. Many ostomates are overthe age of sixty, often with dexterityproblems or failing eyesight. This hasobvious health risk consequences, not tomention the psychological implicationsattached to this disposal technique.

The BCA Report (Berry 2005) showedthat many ostomates are finding pouch

disposal a real problem.Some ostomates describedhow they discarded theirpouches at night in dog binslocated in parks. So thequestion that everyone isprobably asking themselvesis what can be done to helpresolve this situation?

SecuriCare’s Delivery Drivers“My customers are lovely people”The SecuriCare home delivery servicecontinues to grow as increasing numbersof ostomists realise the benefits of havingtheir stoma goods delivered direct to thedoor. Thousands of prescriptions areorganised by the company every monthand there’s great teamwork in operationarranging and co-ordinating supplies.

At the front end of the service are ourdelivery drivers, who, even if we say so

ourselves, provide an excellent service.Randolph is one of them and he covers thearea around Head Office in Loudwater,Bucks, out as far as Hertfordshire and overto Newham in East London. Randolphloves his job – he must do as he’s beenwith the company for 14 years! Five days aweek, through all the seasons of the year,Randolph is on the road, covering around200 miles a day.

Randolph has two children – Jordan, 7 andRenee, 3 years old. He has known greattragedy, just 3 months after Renee’s birth,his wife Dionne went into a coma and neverrecovered, she died last year. Randolphhas been helped through this time by hisfamily (his mother-in-law does a great jobhelping out with the children), his friendsand work colleagues and also the clientson his round. The job has helped to keephim going and keep cheerful. He smilinglyreports that “all my customers are lovelypeople” and says that so many of themhave set him a good example by the waythey cope and get on with life. He has lotsof laughs with them.

Randolph enjoys the flexibility of his workand being out and about. The onlydownside of his job is the M25!

I have been asked towrite this article bySecuriCare. I have hadmy Colostomy for 6years. I had beenhaving a lot of painand trouble and whenthe doctor examinedme he sent me

straight for a check up and I wasadmitted within 6 weeks. I had myoperation at Crawley General Hospital onApril 1999 aged 85 and it was a lifechanging experience. The surgeon andthe nurses were all very helpful andtreated me very well. I am a very strong-willed person and after about two monthsI returned to good health.

I thought it’s now time for me to getcracking with the time I have left, so I’moff to college to try and learn aboutcomputers. Did the course, even got acertificate, had a lot of laughs! Took upswimming lessons too. I also wanted toprove that having a stoma need notprevent you from doing anything, so lastyear I abseiled to raise money for the ‘olddears’ to take them on a day trip toWorthing. Not only did I raise £300 but Iloved it and now fear I may be addicted!Don’t get me wrong; I am not superhuman or super fit but my strength of willcarries me through and I can’t ‘sit aroundand do nothing’.

In fact the local newspapers have printedarticles about me and a journalist hasdiscovered I may be the oldest abseileron record. On 28th September 2005,aged 90, the ‘daredevil’ pensioner struck

Abseiling 90 year old colostomistagain. This time I scaled 30 metres downthe side of a building to raise money forSt Catherine’s Hospice, Southgate. I amwaiting to see if I make the GuinnessBook of Records but even if I don’t I amglad I took the plunge!
