The BCTF Salary Indemnity Plan will assist BCTF employees who are unable to maintain, or return to their positions due to a medical illness or disability by funding an early intervention rehabilitation program. This program is offered on a voluntary basis.

An early intervention and return-to-work program for employees

Who can access the

BCTF staff Health and Wellness Program?

Any BCTF employee who contributes to the Salary Indemnity Plan is entitled to request a referral to the program.

Privacy

The BCTF staff Health and Wellness Program respects your right to privacy. In order to develop a plan unique to your needs, the rehabilitation consultant will gather personal and medical information. This information will be used solely to assist you and remains confidential between you and your consultant. Your functional abilities will be shared with your union representative and the Human Resources Director as needed; however, information regarding your diagnosis and treatment will not be shared without your consent.

The BCTF Salary Indemnity Plan will assist BCTF employees who are unable to maintain, or return to their positions due to a medical illness or disability by funding an early intervention rehabilitation program. This program is offered on a voluntary basis.

How does the program assist an employee?The Health and Wellness Program contracts professional rehabilitation consultants to work with you. In consultation with your health care provider, these consultants will co-ordinate services that help to improve your health and functional abilities. The services provided by the rehabilitation consultants can be as straightforward as facilitating a durable return-to-work plan or as complex as the development and implementation of a co-ordinated rehabilitation plan that draws on input and direction from your medical team.

Return-to-work planning and supportFor those individuals whose medical team has indicated that a return-to-work is possible and advisable, the rehabilitation consultant can be a valuable resource in helping to design, communicate and implement a return-to-work plan that is both durable and practical. The rehabilitation consultant will work with the Human Resources Director to review the plan and can assist at the Joint Accommodation Committee meeting, if desired.

A step-by-step approach to reach your goalsFor more complex situations, your rehabilitation consultant may work with you to develop a goal based plan following four sequential phases:

1. Assessment. A local rehabilitation consultant completes an initial interview with you and may consult with your medical practitioner(s), depending on your situation.

2. Planning. You and the consultant develop a rehabilitation plan based on the recommendations of the attending physicians and health care practitioners.

3. Implementation. You work with your consultant and health care team toward achieving the goals as set out in your plan.

4. Follow up. Once the goals of the rehabilitation plan have been achieved, the consultant provides support and ensures that you have strategies to sustain your health.

In developing this plan, your rehabilitation consultant will partner, as needed and as required, with your union representative and/or the Human Resources Director.

How can I be referred to the program?Potential candidates can be referred to the program in four ways:• self-referral• by your union representative or designate• by the Human Resources Director• by the BCTF Salary Indemnity Plan

Administrator.

The referral processAn employee may self-refer by contacting either the BCTF Plan Administrator or the Human Resources Director. An employee may also be referred by their union representative or the Human Resources Director. Referrals are also generated by the Salary Indemnity Plan Administrator if the employee is receiving LTD benefits and Great-West Life advises that the medical reports indicate that recovery could be enhanced through rehabilitation assistance.
