What is Rehabilitation?

The term “Rehabilitation” can be used in a variety of settings – from single injuries in Sports Medicine to multiple catastrophic injuries after a motor vehicle collision or the devastating effects of a severe stroke. It can involve a single health care provider or a large team of professionals. In the US, the most comprehensive setting for r e h a b i l i t a t i o n i s t h e A c u t e I n p a t i e n t Rehabilitation Facility (IRF). This nomenclature is slightly confusing since the term “Acute” refers to the insurance resources being utilized and not the time after injury – there is no need to have a recent stay at an Acute Care hospital prior to admission. Some individuals that have subacute, chronic or even congenital conditions can still be appropriate for care at this level. As such, the term “Comprehensive Intensive Inpatient Rehabilitation” is more accurate. This level of care is new to the UAE and the region. It involves care of individuals with loss of function due to injury or disease and the potential to restore function with the goal of returning home. Generally, patients receive 3 hours of therapy daily including an individualized plan of Physical Therapy, Occupational Therapy and/or Speech Therapy. Patients also have needs for 24-hour rehabilitation nursing with a high level of integration such that nursing staff follow through with approaches for functional gains established by therapists and they advise therapists on progress or limitations. The patient and family are an integral part of the team and the rehabilitation team also includes professionals in n u t r i t i o n , s o c i a l s e r v i c e s , p s y c h o l o g y, rehabilitation equipment, bracing, and others. The team is led by a Physiatrist (a physician with special expertise in the field of Physical Medicine

& Rehabilitation). This physician meets with the team in daily huddles and in formal patient team meetings on a weekly basis. Other physicians may assist in management of active medical issues on the unit and the Physiatrist assures that medical management augments the rehabilitation process rather than slowing progress. Spinal Cord Injury (SCI) rehabilitation represents one of the most obvious arguments f o r C o m p re h e n s i ve I n t e n s i ve I n p a t i e n t Rehabilitation. SCI patients with paralysis due to trauma present with paralysis, pain, severe functional impairments, needs to adjust to disability, bowel & bladder complications, skin concerns as well as frequent post-operative and/or infection concerns. They present a clear need for this comprehensive and intensive level of care which has been unmet in the UAE, resulting in many patient leaving the country for care. Many other diagnoses including Traumatic Brain Injury (TBI), stroke, complicated orthopedic injury, neurodegenerative diseases, amputation, de-conditioning due to prolonged hospital stay can all benefit from this level of care. Essentially ANY diagnosis that results in a significant functional loss and ability to benefit from the above program criteria with measurable functional gain are appropriate to consider for Comprehensive Intensive Inpatient Rehabilitation. The goal of discharge home at a maximum level of functional independence helps reduce long term costs of care and maximizes the individual’s ability to contribute to home, family, and the workplace for years to come. As such, developing this level of care in the UAE will be of major benefit to the individual and the UAE culture.

Enterprise Resources Planning (ERP): the ORACLE Cloud first phase of the project has successfully been completed, which includes: Human Capital Management & Payroll, Employee self Service, Talent acquisition, Finance, and Supply chain. The second phase will include Goals and Pe r f o r m a n c e m a n a g e m e n t w h i c h planned to be completed in the third quarter of 2018.

Health Information Management (HIS): Capital Health has contracted with the HIS industry leader InterSystems to implement their application (TrakCare) across Capital Health entities. The new system should be ready replacing Insta in the second quarter of 2018. The system come with various modules to cover all the clinical streams including:


Patient Access Clinical Foundation Laboratory Foundation Patient Billing Operating Theatre ICU Anesthesia Active Decision Support Ordering and Results Communication Radiology Pharmacy Medication Management Discharge Summary Active Clinical Notes

PACS System: Picture archiving and Communication System, the biomedical depar tment has contrac ted with Carestream to implement their PACS system which should replace any radiology films printing and change it to digital.

Infrastructure: All servers, network, wireless network and telephone system will be provided as state of the art technology from Nutanix and cisco.

End Users Equipment: contracted with Dell and Zebra to deliver desktops, label printer, wristband printers and barcode readers.

Patient experience: IT works also on other projects which should have positive impact on patient experience, such as, pat ient mobi le appl icat ion , se l f-registration kiosk, way finder kiosk, queue management system, and others.
