• Organizational Culture – the shared basic assumptions that are developed in an
organization as it learns and copes with problems.
– the basic assumptions that have worked well enough to be considered valid are taught to new members of the organization as the correct way to perceive, think, act, and feel.
• Safety Culture– An organization’s values and behaviors – modeled by its leaders
and internalized by its members – that serve to make nuclear safety the overriding priority.

• Who owns the IST Issues?– IST Engineer?– Component Engineer?– System Engineer?– Maintenance Engineering or even from a Broke/Fix standpoint, Maintenance?
• Each site will have their own unique issues, or culture.– Work in progress, constantly evolving as we address issues
• Detailed Program Documents will help improve your IST Culture:– IST Program Bases Document (LAN) to show what is/what is not/and why.– IST Database (LAN) with adequate depth of content.– IST Implementing Procedures – Generic, Pump, Valves.– Visible Interface with other key groups on site.

• Instruments used for IST out of tolerance – Adverse trend? What is the interval? First time event?
• Stroke Time – What is the trend? How to know when it is adverse – what rules are applied?
• IST Pump - Close to ALERT value, or fails surveillance test – What was the trend? What/who is tracking the issue?
• Relief Valve fails setpoint test – Repeat? Contingency? Expanded Scope? What other valves are impacted? What S/N is involved?
• Relief Valve not tested within required timeframe – How and who is tracking? Change in model?
• Check Valves – How to describe Check Valve Condition Monitoring activities – Forward, reverse, interval, PMT issues?

adjust, etc.– Known locations for this info.– Reviewed by IST Group for acceptability.– Adequate Technical Rigor used in these reviews? Partnering
with Pump and System Engineer for margin issues? (KPIs)– Database feature – select by instrument, historical sheet
showing AF/AL results, past AR’s and corrective actions.– OX1456.86 contains all relevant acceptance criteria info – used
as the technical bases for Operations.
