    2. Voter has no acceptable ID

    Follow procedures under Problem #12 for Provisional ballot, except: Check box #3 on the back of the green envelope as the reason

    the voter is voting a Provisional Ballot, check reason #3 on theProvisional Voter Notice, and enter reason #3 when completingthe Precinct Provisional Ballots Log.

    Note: For the H or HA voter, the fully completed, signed provisionalballot envelope satisfies the ID requirement. The enclosed provisionalballot will be counted if the voter is otherwise qualified.

    Use Affirmation of Identity form. Complete the information at the top of the form.

    Have voter sign and print his name on Section B of theAffirmation of Identity . State the voters name and address Mark off the next PBC number. Enter PBC number and S (for

    Statement) in appropriate columns in pollbook. OR on EPB , check in the voter and set voter flag to " S" per

    instructions. Have voter cast vote on voting equipment in use in the precinct.

    Note: A person without ID who refuses to sign the Affirmation of Identity

    may not vote. Do not enter a PBC number or S in the pollbook.

    Important! Do not offer this voter an Affirmation of Identity. Do not enter voters name and address on pollbook. (As with

    other Provisional voters, do not enter PBC number next to thisvoters name.)

    If using EPB, check voter as provisional if so instructed (PBC willnot increment).

    Do not offer this provisional voter a new registration form unless needed for another reason (moved, changed name, etc.)

    Do not allow this voter to vote on voting equipment in use in theprecinct.

    Do not pull this voter out of the main line to complete theAffirmation of Identity.

    Do not detain or delay this voter in any way unless there areother problems.

    Do not send this voter home to get an ID. Offer the Affirmation of Identity form.

    Important!Never turn a voter away because of lack of ID.

    Officers need to check the pollbook and determine whether an AB is listednext to the voters name. An AB indicates an absentee ballot was issued tothe voter. If there is an AB next to the voters name on the pollbook, followthe procedures outlined in Problems #19 - 22 concerning all absentee voterswho appear in the polling place.

    For F voter, if there is no AB listed next to the voters name on the pollbookand the voter does not appear on the Final Absentee Report, the voter was not issued an absentee ballot and is allowed to vote a ballot for federal offices only.

    State voters name and address. Mark off next PBC number, enter PBC number and F in pollbook,

    and write F across voters PBC number in the Pollbook Count form. OR, on EPB , check in the voter and set appropriate flag for " F" voter

    per instructions. Issue the Federal Only Ballot . Follow instructions for equipment in

    use in your precinct. (Be sure to set machine back to full ballot for thenext voter.)

    For the R or T voter, if there is no AB listed next to the voters name onthe pollbook and the voter does not appear on the Final Absentee Report , thevoter was not issued an absentee ballot and is allowed to vote full ballot for alloffices.

    State the voters name and address. Mark off next PBC number and enter PBC number in pollbook OR, on EPB , check in the voter (PBC increments automatically). Have the voter cast vote on voting equipment in use in the precinct.

    A person who has moved back to Virginia from overseas should be offeredan opportunity to complete a Voter Registration Application form to ensurethat he or she will be eligible to vote in the next election.

    10. Voters nameomitted frompollbook in error

    Only the Registrar, who has access to all voterregistration records, may authorize the Officer toadd a voters name to the pollbook.

    18. Paper Pollbookshows, an A in thecolumn to the left of thevoters name (on EPBmay be shown in voter'sdetails or otherwise)

    This applies to very few voters.A column of "A's" (with anoccasional "I") on the EPB indicates"active" vs. "inactive" status not "assigned number."

    These voters do not have a Social Security Number in thesystem. The Officer is required by law to ask if voter has aSocial Security Number.

    Constitution of Virginia, Art. II, 2 24.2-418 24.2-643(D)

    Note : The SSN is required for the registration record, if thevoter has one, to prevent duplicate registrations and fraud.

    Ask voter to complete a new Voter Registration Application form. Next to orabove the Social Security Number on the form, the Officer should write SSNUpdate. Do not write SSN on the pollbook or ask voter to state it publicly.

    Note : The " A " status does not otherwise affect the voter's qualification to vote.Proceed to process the voter normally.

    19 . Absentee ballot notreceived or lostby voter and votercomes to polling placeto vote

    For this voter, the AB List should state Issued under theBallot Status column and no date under the Returned column. If the Ballot Status shows as Issued with nodate in the Returned column, the voter may cast aprovisional ballot at the CAP or their assigned pollingplace.

    This voter will be marked on the pollbook with AB indicating the voter was sent an absentee ballot -- but noballot is shown as having been returned marked orunmarked .

    Officers must check the Final Absentee Report anddetermine that there is no date shown in the Returned column and that it states Issued in the Ballot Status column.

    If the AB list states Unmarked in the Ballot Status column, see Problem #22 .

    Note : Any voter marked with an F on the pollbook mayvote a ballot for federal offices only. The provisional ballotissued to this voter must be the Federal Only Ballot . SeeProblem #8.

    Note : The ID requirements applicable to other votersapply to this voter, including the HAVA ID requirement ifvoter record is marked " H " or " H A " and a federal officeis on the ballot. (See Problems #1 and 2 .)

    Have voter complete identifying information and sign statement onthe green Provisional Vote envelope.

    Have voter check box #5 , under Statement of Voter. Ask voter to show one of the IDs listed under Problem #1 (or #2 if

    marked " H " or " H A " and a federal office is on the ballot). Enter voters information and circle reason #5 on the Precinct

    Provisional Ballots Log .

    Do not enter PBC number or any other information on the printedpollbook for this voter. On EPB, check voter as "Provisional" as instructed. Have person vote and seal ballot in green envelope. Place sealed green envelope in ballot box Give voter the Provisional Voter Notice (HAVA-5 form), with reason

    #5 checked. This informs the voter when and where Electoral Boardwill meet and provides other required notifications.

    The provisional voter does not need to complete a separate Ballot

    Lost or Not Received form. Record or include this voter along with any other In-Personprovisional voter on the Statement of Results.

    Upon surrendering the unused or defaced ballot, the voter can vote normally atthe CAP or their assigned polling place . Confirm that AB is marked in the column to the left of voters name on

    the pollbook and the name is on the AB List .(See Explanation above.)

    Mark on envelope containing voters ballot: Unused Ballot or Spoiled Ballot.

    Write date of election (e.g., 11/4/08) on the AB List in the columnlabeled Date Unused Ballot Returned.

    State Voters name and address. The ID requirements applicable to other voters apply to this voter,

    including the HAVA ID requirement if voter record is marked " H " or"H A " and a federal office is on the ballot. (See Problems #1 and 2 .)

    Mark off next PBC number and enter PBC number in pollbook OR, on EPB , check in the voter (PBC increments automatically). Have voter cast vote on voting equipment in use in the precinct. Make note on both copies of SOR: AB Voter (name and ID #) came to

    precinct with unused (or spoiled) ballot and was permitted to vote in same manner as other voters

    Place envelope containing Unused Absentee Ballot in Envelope #5. Place envelope containing Spoiled Absentee Ballot in Envelope #4. On SOR, count this voter as an in person voter not as an absentee

    voter.
