What Families Are Saying “You were a phone call away, a security blanket that helped whenever we needed it. I don’t think we could have taken care of my mother at home without your help.” “From visits to laundry to shopping...it is so evident that you all really care and thus you provide such deep comfort.” The Oklahoma Team Nurse Case Managers, in consultation with the patient’s physician, are responsible for all aspects of care and comfort. Home Health Aides are available to help patients with the activities of daily living. Chaplains/Spiritual Counselors support the family and patient in their spiritual needs. All services are non-denominational and consistent with the patient’s individual beliefs and values. Social Workers assist in identifying community resources and provide counseling and support for the patient and their family. Trained Volunteers are available for companionship and to help with household errands. The Hospice Medical Director, working alongside the patient’s primary care physician, provides medical guidance for the Oklahoma Hospice Care team. Professional Care.Personalized Comfort.klahoma Hospice Care offers our patients the very best care available in their home, without unnecessary

emergency room visits, hospitalizations or trips to the doctor’s office while in cooperation with their physician. Our professional staff of nurses, social workers, chaplains and home health aides brings maximum care and comfort, developing personal relationships with patients and families in the process.

OInsurance Covers Most ServicesMuch to the surprise of many families and patients, hospice care is covered by Medicare, Medicaid and most insurance companies. Patients pay nothing for OHC services if they meet Medicare’s hospice benefit guidelines. To qualify, patients must: (1) have an irreversible illness, (2) have chosen not to aggressively fight the illness and (3) and their physician recommends them for care. We provide your day-to-day care under the guidance of our medical director, but we also work closely with your referring physician to get valuable guidance and keep him/her informed about you, as well as any emerging conditions.
