Proposal Narrative: Please provide a comprehensive description of the program for which you are requesting funding. Typically, this is a document, 2-5 pages in length, describing who you are as an organization, the program for which you are requesting funding, and how your organizational mission and focus aligns with that of The White Family Foundation. This is your opportunity to present your organization and your funding needs to the Trustees. This is where you tell us who you are, what you do, why the foundation should consider partnering with you in your mission, and, finally, this is your opportunity to make your “ask”. This may be on letterhead in letter format or typed up as a word document.

Non-Profit Letter of Determination from the IRS: Please include a clean and legible copy of your 501(c)(3) qualification letter from the IRS. Please include a copy of the complete letter that specifies your organization’s legal name and the qualifying status. If you operate under the umbrella of the qualifying organization, please include documentation to show that connection. For example: a parish school operates under the non-profit status of the Diocese to which it is connected. The Diocese holds the non-profit status letter.

Program Budget: Please include a detailed report on the program budget. Typically, this is a spreadsheet of how your resources are allocated for the program. Please provide information on where you are currently receiving funding from and how this program fits into your overall organization budget. Some organizations choose to include their P&L and Balance Sheet as demonstrations of full disclosure of how their organizations are fiscally structured and responsible.

Board Members/Board of Directors List: Please include a list of your current board of directors and leadership team with information on the positions they hold and their duration of service with your organization. This is your opportunity to share about those people in the community that actively support your organization with their time and energy. These connections are important and assist our Board of Trustees in their due diligence processes.

Additional Documentation: This is your opportunity to include fliers, pictures, project bid quotes or additional information that you believe would be helpful for the Trustees to have during their review. Many organizations include their annual reports or recent newsletter, highlighting their impact in the community. Other organizations have included photos and bid quotes as evidence of their research gathering processes for specific programs and projects.
