Dr Ian Coutts is a practicing dermatologist in Wellington Hospital and NZ Dermatology and Skin Cancer Center for over 10 years. He trained in Auckland, Amersham & Oxford rotation in UK and spent a year doing histopathology at St John’s Institute in London UK. His main interests are skin cancers and complex dermatological medical diseases.

Topic: How we can be the best of colleagues. The Crossover of the Double D’s: Dermatology and Dentistry

We will cover how dentists can spot incidental skin disease such as skin cancers inflammatory diseases of the oral mucosa. At the end of this talk I hope there will be a better understanding of dentists diagnostic skills, appropriate prioritization, the consideration of dermascopy and teledermatology in dental practice.

President’s Report

As I write my thoughts for this newsletter on a Sunday morning, I’m enjoying the warm weather from our fantastic Wellington summer and thousands of fun run participants are hitting the roads doing the same. It really is a great time of the year – one we all should try to make the most of by taking advantage of all the opportunities and activities that are available.

The first meeting of the year last month at The Wellington Club was a success on many fronts. Those that attended the dinner from 6pm were treated with divine canapés and complimentary drinks sponsored by Giestlich, followed by a beautiful meal served by attentive staff in comfortable surroundings. We did have some small glitches with access to the Club for those who only attended the meeting, however this should no longer be a problem.

The branch, as guests of The Wellington Club, are privileged to have access to the facilities of this private club which is normally reserved for their members, but there are a few intricacies unique to this venue such purchasing drinks - arrangements have now been made so you will be able to buy drinks from the 6th floor bar but you must pay by eftpos (no cash).

Those that attended the meeting afterwards were introduced to the new committee, and we had short presentations from some of our colleagues updating them on the branch’s work with DCM (The Downtown Community Ministry), Healthy Futures and our initiative for Wellington dentists to sponsor the cost of a water kit to an identified school or community organisation committed to the health benefits and objectives related to ‘switching to water’. We had a great uptake for sponsorship of the water kits and we would like to hear from those who couldn’t make the meeting and would also like to contribute – please contact Laura Ichim.

Another ongoing project is the support for more water fountains throughout Wellington – the branch is one of several interested parties which include WCC and Te Araroa trust.

I also mentioned that we Wellington dentists, as a branch have been asked to host a number of final year students this year both for observerships and accommodation for the week – a recent email from Dave Excell has been sent recently explaining the details. I am embarrassed that the last time we were asked to do this (a few years ago), a lack of interest in the scheme meant that accommodation had to be organised for the students at a youth hostel (most likely at their own expense). I can only remember the generosity of the Christchurch dentists’ at my final year placement which also included a dinner for a group of us at one dentist’s home followed by dancing late into the night, so I hope that this time it will be different.

It was great to see a number of Oral Health Therapists & Hygienists at our meeting and particularly to connect with them enabling us to share our passion and knowledge in the field of dentistry, as well as local community initiatives. We look forward to seeing them more often at our meetings.

Our new Periodontist, Frances Ruddimen gave us a fantastic presentation on ‘Preservation of the Extraction Socket’, and we thank her and welcome her to Wellington. With the support of our specialists and Bridget Keenan from Giestlich, I am hoping that more dentists when being faced with having to remove a tooth, consider the option and benefits of a simple bone graft in terms of restoration of the site.

We look forward to seeing more of you there at our next branch meeting which will feature Dr Ian Coutts, Dermatologist who will speak on the dental clinician’s role on the very relevant topic of ‘Skin Health’.

Mural: Drs Ichim & C Wong informed the Committee that finding a wall for our Mural is extremely frustrating although they are still actively looking at options it has been suggested that the Lower Hutt City Council are very receptive to initiatives such as our wall so Dr Ichim is going to approach them to see if having the wall in the Hutt Valley may be an option.

MATTERS ARISING:

DCM: Dr McKelvey confirmed that he and Drs Cho and Nicol did have a positive meeting at DCM. Dr McKelvey updated the meeting on a voucher system that GC has. It works when product is purchased then a dollar amount is accumulated and when a value is reached it a voucher is forwarded to the Practice for redeeming on their next order. This would allow Dental Practices to donate that voucher to DCM for their benefit. It was agreed that this would be advertised in the Newsletter and Dr McKelvey will reinforce it at the Branch meeting.

Dr Woods suggested that it may be beneficial for the Dental Assistant working at DCM to attend an infection control workshop being run by Megan Sharpe. Dr Woods is to contact Megan to see if they will sponsor her attendance.

Dr McKelvey reported that sponsorship from suppliers for our Branch meetings has proven to be an issue this year as he already that speakers confirmed for our meetings and it appears that suppliers now prefer to have their speaker attend any meeting sponsored by them. Exceptions to this are Healthcare Essentials and Ivoclar with Ivoclar sponsoring the September meeting and Healthcare Essentials still to confirm. Dr McKelvey will follow this up.

GENERAL BUSINESS

In the absence of Dr Logan Dr McKelvey informed the meeting of Dr Logan’s concerns regarding Snorepro Devices and it was agreed in the first instance that Dr Excell would take the question to the NZDA Executive “can any person provide a snoring device to the general public”

Dr Excell spoke regarding the email he sent to the Branch requesting submissions from members to the Porirua City Council regarding “local Council role in sugary drinks harm reduction”. It was agreed that Dr McKelvey would on behalf of Branch and its 190+ membership send a submission.

There was discussion regarding applications from the Branch for Outstanding Dentist and Young Dentists of the year. It was agreed that an advert would go into the Newsletter and Dr McKelvey will also raise it at the Branch meeting.

GENERAL BUSINESS:

• Dr Cho on behalf of the Committee thanked Dr Ravi de Silva for his continued support of the Branch by making his Boardroom available for Committee meetings.

• Dr Ichim will write a letter of thanks to Ivoclar on behalf of the Branch for the autoclave that they assisted with for DCM.

• Dr A Wong will write a Branch report for both our newsletter and the NZDA news for the beginning of 2018.

• Dr A Wong will write the evaluation report needed following the Wrigley’s grant as this is due in December.

• Dr Cho reminded the Committee of the DCM AGM scheduled for Monday November 20th November. Drs Cho, A Wong, Templeman and Nicol indicated that they are at this stage able to attend.

• Dr Nelson will email the Branch asking members to consider becoming a mentor for new graduates.

• Dr McKelvey asked for another email to be sent to members regarding the Presidents Dinner.

• Dr Excell confirmed the dates for the NZDA Conference for 2018 being 15th – 18th August.

• Dr Excell will contact Dr Ferry to see if he is interested in returning to the GPDP role.

• Dr Ichim will email the membership to encourage articles for the newsletter that are not necessarily teeth related,

• Branch Christmas Roster: The Committee agreed to follow the same process of last year with Dr Ichim contacting Practices who are open during the statutory days to confirm they abide by the rules of the Branch relating to inclusion in the roster.

• Branch meeting venue for 2018: Dr A Wong will look into alternative venues and confirm with the Admin Assistant within 2 weeks so that dates can be confirmed.

• Meeting Dates for 2018: A calendar of dates was confirmed for 2018 with the exception of the first Committee meeting being brought forward a week to the 7th February as the scheduled date is Valentines Day. It was agreed that a Branch meeting in Lower Hutt will be organised for 2018 a preference being the June meeting. Dr M Wong will liaise with the Admin Assistant in organising this.

• Branch Speakers for 2018: Dr Cho went through a prospective list of

speakers for 2018. Dr Cho asked the Committee to feedback to him any suggestions they have for inclusion in his programme. Dr Excell stated that Dr Bill O’Connor (NZDA President) has indicated that he would like to attend a Branch meeting next year. Dr Excell will follow this offer through suggesting July as a preference.

DCM Dental Service

End of December 2017 update

In December 2017, John Buckerfield took his 40th session at the DCM Dental Service (photo above taken on that day). John takes a session every second Monday, and his commitment to the service has been a critical component in its success.

• Jane Gaudie has put in many hours at DCM over the period. Her focus has been on developing protocols to meet current standards for infection control. This has involved implementation of codes of practice which can be easily followed by all dentists and assistants working within the DCM clinic. The donation of a late model autoclave by the Wellington Branch of NZDA has been a major part of ensuring that this is possible. In 2018, Jane plans to introduce an induction training programme for dental assistants who work at DCM.

• We have had a new volunteer DA join the team. Margaret Jackson worked at the Wellington Hospital Dental Department in the 1980s, and then in private practice for many years. More recently she has completed a degree in Māori Visual Arts and is currently working as the Arts Coordinator at Kāpiti College. With some flexibility around her working hours, she is now able to volunteer as a dental assistant at DCM, and has supported three different dentists to date.

The DataDentists

During the 2017 calendar year, we were able to offer 71 sessions at the DCM Dental Service – 36 during the first half of the year, and 35 during the second half of the year. During the month of November 2017 there were 9 sessions, the most we have ever been able to offer in a single month. This meant that we were able to provide treatment to many of the people on our waiting list ahead of the quieter (in terms of dentist availability) Christmas/summer holiday period.

This year we were very pleased that we were able to offer one dental session during the Christmas holiday period; Oliver Ferry and his dental assistant Belinda Buckley took a session on the 29 December 2017, the final business day of the year, providing much-needed emergency treatment for taumai. John Buckerfield has also taken his first session of 2018 on 8 January; hence we have not had a long period without any service available as was the case last year.

One new dentist joined the team during the second half of the year. Louisa Leathart took her first session in July, and has been a regular ever since, taking 5 sessions to date.

The five dentists who took the most sessions at DCM during 2017 were John Buckerfield (23 sessions), David Corcoran (9 sessions), Andy Wong (8 sessions), Tihema Nicol (7 sessions) and John Taylor-Smith (7 sessions).

Other dentists who took sessions during 2017 were Louisa Leathart (5), Oliver Ferry (4), John Burton (3), Liz Hitchings (2), Jesslyn Praganta (2), David McKelvey (1) and Charlotte Cocks (1)i.

Taumai

At DCM, we call the people we work with taumai, meaning to settle. This reflects the journey we embark on together to become settled, stable and well. We support taumai to find and sustain housing, to access their correct benefit entitlements and manage their money, and to connect to primary, oral and mental health, specialised alcohol and drug services, and to whānau and other supports.

Through the DCM Dental Service we have been able to provide emergency oral health care and pain relief to 186 taumai over the 2017 year, across 262 appointments. During the second half of the year, our dentists saw 103 taumai over 133 appointments.

Taumai Experience

“I was begging on the street and a lady told me about this place and I’m so glad I found it. My tooth was hurting really bad and making me feel really sick. I’m so happy with the dental service. They removed the tooth and it’s so much better. I’ve never come across a place like DCM before. It is quite amazing actually - dentists, nurses, assistance with food, someone to help me with other things…Thank you!’’

• A woman had been staying with the Wellington Women’s Homeless Trust. In a great deal of pain due to broken teeth, she rang DCM one morning to see if she could get an appointment. Fortunately we had a cancellation that day and she was able to be seen urgently. She left DCM pain-free and is now concentrating on her long term housing options.

• Another woman had a number of loose and missing teeth but was not in a space to receive dental treatment due to mental health issues. After an assessment by DCM’s partner Te Aro Health, she began taking medication and her mental health improved. She was then able to make and keep an appointment, and was seen for dental treatment where she had extensive work done.

• One man was very happy with the service he received at the DCM Dental Service. “They were very professional.” He had a busted tooth right down to the nerve and needed one extraction and one filling. He had been having trouble eating and was worried that he would end up with no teeth. “Brilliant service” was his assessment of the DCM dental experience.

Thank you all for your continued support of this much needed service, and the difference you are making in the lives of the most marginalised people in our city.

Summary of Last February Speaker

Alveolar Ridge Preservation – Who, What, When, Why and Health. By Dr. Francis Ruddiman.

Ridge preservation is any procedure undertaken at the time of or following an extraction that is designed to minimise external resorption of the ridge and maximise bone formation within the socket.

Follow extraction of a tooth, ridge resorption occurs. There is a reduction in both height and width. There is approximately a 29-63% and 11-22% of horizontal and vertical resorption respectively. Most of this reduction occurs within the first three months and continuous slowly over the next twelve months. Ridge resorption occurs differently in different sites and in different patients.

The goal or alveolar ridge preservation is to minimise the loss of alveolar ridge volume. Evidence shows that alveolar ridge preservation reduces alveolar volume loss.

Case selection is important, both aesthetic (e.g. buccal bone thickness), patient (e.g. smoking status) and site risk factors (e.g. periodontal heatlh) need to be assessed. A CBCT is used to assess buccal plate thickness.

Indications for ridge preservation include:

• Age of patient (i.e. too young for implant placement). • Pregnant patient. • Patient unable to complete implant treatment in a short timeframe. • Site factors are unfavourable for implant placement.

There is a huge variation in materials and procedures out there in the literature. Therefore, it is difficult to come to a conclusion about materials and techniques.

Techniques – Flap vs. flapless.

Materials• Membranes – Resorbable vs. non-resorbable. No observable difference in

clinical outcome.

• Bone grafts. Sources of bone graft: Autograft, Allograft, Xenograft, Alloplast.When considering bone grafts, three main properties determine what we use: Osteoconduction, Osteoinduction, Osteogenic, Bone graft efficacy (Xenograft and allografts have similar efficacies. Any graft is better than no graft.)

In summary, alveolar ridge preservation leads to better aesthetic and clinical outcomes.

News

Congratulations to Liz Hitchings (our Hon. Treasurer) for being elected as Salaried Representative on NZDA National Board.

Water Kit Initiative NZDA Wellington Branch in association with Healthy Futures Trust: Myself as the Hon Secretary of the Branch and the Wai Water Manager and our entire Committee would like to extend a huge thank you to all

the generous members of our Branch who pledged to purchase Water Kits to support 16 local primary schools in Wellington region who have sought our help and support to become ‘’water only’’ schools. Helathy Futures Trust is the process fo assembling the kits and having them ready for delivery to the schools and I will be in touch personally with all the dentists and practices that have signed up to support this fantastic initiative once the kits are ready in the next few weeks.

There are more schools in need of water kits and Healthy Futures Trust is going to aim for another batch of kits to be assembled for the remaining schools and already some of our members have asked to have their name down for the next round of schools. Plus if you know of any other schools (primary or secondary), community centres, organizations,daycares etc that may want them in the next 3 months and if you are interested in purchasing them a water kit please contact Laura at [email protected].

The kits cost $200(plus GST) and are portable,reusable,sustainable and easy to use. They are also a donation and tax deductible.

As a branch we are making a huge impact in our local communities by supporting the ‘’water only’’ movement. The Heart Foundation and Regional Public Health who organize the implementation and successful use of these kits in schools wanted me to extend their Thank You to the NZDA Wellington Branch for our support of this cause and they will publicise this once the kits

have been implemented in the schools. They were blown away by our support and generosity.

NZDA Student Observer Programme: We need your help to make this run smoothly and successfully. Wellington is hosting the NZDA STUDENT OBSERVATION PROGRAMME this year and WE NEED YOUR HELP!

The programme will take place over five days during the university holidays: 28th May – 1st June . The aim of this programme is “to provide final year BDS students from the University of Otago with insight into what it is like to work in private practice”. This will be achieved through them observing at a different private practice each day, whilst spending the week with a local dentist in their home. WE NEED YOUR HELP to make this programme a success. If you are interested, then you can help with one or both of the following:

• Billet a student(s) for the week in your home• Open up your practice for observation or both
