Our ChallengeMany people seeking

behavioral health services have both mental health and

substance abuse issues, as well as other complex needs.

Our MissionTo create a community

system where the people seeking help engage in

meaningful partnerships with the people providing help.

Our GoalEvery person and program

in the MC3 will become welcoming, culturally

intelligent, trauma-informed, recovery-oriented, and

co-occuring capable.

MC3 efforts continue to grow every year as more and more talented people and incredible agencies get involved. Each subcommittee is diligently working on tasks for the betterment of the co-occurring system in Milwaukee County. As a group we are always looking at ways to increase involvement and share our values with others in order to offer welcoming, co-occurring capable, trauma informed and person centered recovery services. We are excited about our accomplishments the past few years and especially in 2014 when many of the coalition’s ideas became reality, such as completion of our Values, a website, and the collection of Change Projects. It is our hope to continue to expand our partnerships in the community and offer the highest quality of services to those seeking assistance.

Welcoming: All individuals,

regardless of their presenting issues, are

greeted warmly, without judgment or criticism, in

every environment in which they seek help.

Trauma-Informed Care:

Care that is grounded in and directed by

understanding the impact of trauma and violence on humans.

System Integration: The development of long-term structural linkages among different sectors of the human services

system as a whole.

Stage-Matched Recovery Planning: For each condition or issue presented, care

plans and outcomes must be matched to a specific

stage of change and phase of recovery.

Service Integration: The creation of linkages

among services, resources, people, and/or

processes involved in every person’s recovery.

Recovery: A self directed process

of change whereby individuals work to improve their own

health and wellness.

Person-Centered Care:

Helping relationships that begin with the goals of the individual, their

strengths, and their ideas on their treatment.

Cultural Intelligence:

Culturally and linguistically appropriate services

integrated throughout an organization and system in

partnership with the communities being served.

Co-Occurring Capability:

The capacity of helping environments to provide integrated services to all individuals and families

with complex needs.

Person-Centered Care CommitteeIntegrates and ensures that Person Centered Care pinciples are in each program and service. To improve satisfaction and recovery outcomes for individuals and familes while promoting stigma reduction.

MC3 Charter / Document / Bylaws CommitteeDevelops and revises documents which outline the history, purposes and challenges faced by community stake holders working to integrate the system of care.

Representation & Recruitment CommitteeIdentifies those whose involvement in MC3 Steering Committee would maximize the impact of the MC3 approach throughout Milwaukee County.

Outreach CommitteeProvides organizations and individuals with information about MC3 to encourage them to become involved; targeting diversity and unrepresented groups.

Evaluation CommitteeTracks the activities and progress of the MC3 Steering Committee, the MC3 sub-commitees and change agents working to promote system change.

• Constructs Committee - Defines the values of MC3 and the indicators of these values as well as the possible assessments to utilize for this process.

• Annual Report Committee - Produces a report of MC3 accomplishments throughout the year. • Electronic Communication Committee - Manages and maintains electronic methods of communication

throughout the MC3 change agent community. Also, this committee helps maintain quality and improvement of the change agent meetings.

Cultural Intelligence CommitteeImproves recovery outcomes for individuals and families representing diverse cultures.
