• Welcome Packet sent via email upon entry into SLUSON –Also HERE– detailing following requirements:• Criminal Background Checks (Comprehensive) • Signed Knowledge of Blood Borne Pathogen form• Copy of CPR card• HIPPA (Health Insurance Portability and Accountability Act)• Required Health Forms with Immunizations • Current Influenza Vaccination• TIME FLEXIBLE – On Campus test(s) worth 3 credit hours• Optional Student ID
