offering the Active&Fit program, designed to keep your employees at their

healthy best.*

With the Active&Fit program, your employees will have a choice of access to a

fitness facility membership or a home exercise option. The Active&Fit program

also includes access to a host of online tools such as fitness trackers, exercise

planners, nutrition monitoring, online courses, and more. The Active&Fit program

is available for participants 16 years or older.

Two ways to a healthier company.The Active&Fit Basic Program The Active&Fit network includes over 10,000 fitness facilities across the nation,

including many national chains, as well as local facilities. For payments made

between October 1, 2012 through December 31, 2012, your payment will be

collected by Active&Fit using a credit card. For payments made after January

1, 2013, members pay their fee directly to the fitness facility. They pay $100 per

year** and can switch fitness facilities as often as monthly.

The Active&Fit Basic Program features standard amenities offered at most fitness

facilities such as:

• Cardio and resistance training equipment

• Fitness classes

• Saunas, spas, and pools where available

The Active&Fit Home Exercise ProgramFor employees who prefer to exercise on their own, we offer convenient home

exercise kits. They pay $10 per year** and can choose any two from a variety of

dynamic workout kits equipped with a motivational DVD, instructional booklet,

and quick start guide.

In a competitive market, the Active&Fit program is an affordable way to add a meaningful fitness benefit for employees without adding significant costs. Our annual fee is a fraction of most fitness facility membership dues, making this program a good value regardless of how the fees are shared.

*The Active&Fit program is open to members who are 16 years of age or older and is not available to members enrolled in a Kaiser Permanente Senior Advantage plan.

**Payment will be collected via credit card upon enrollment in the program and will be required again each subsequent contract or calendar year (refer to your Kaiser Permanente Group Service Agreement to see if fees are based on a contract or calendar year basis). The $100 or $10 member fees will not be prorated, nor are they refundable. Members enrolling in a facility from October 1, 2012 to December 31, 2012 will pay their annual member fee to Active&Fit. Members enrolling January 1, 2013 or later will pay their annual member fee directly to the facility.

Members should consult their personal tax advisor for specific tax consequences of this program and determination of tax liability.
