by being helpful and positive in his or her comments. The student will not name-call, make rude remarks, or tease.

• The Academy student will work out his or her differences with all students in a respectful way. The student will not hit, kick, bite, spit, etc.

• The Academy student will tell the truth and will not destroy or take others’ property.

• The student will act as a role model for his or her class-mates by showing correct behavior in the lunchroom, at assemblies, in special areas, and on field trips.

*Because of the difficulty in consistently monitoring behavior both at home and at school, badges will be awarded based on school behavior and at the discretion of the classroom teacher. However, we certainly encourage you to monitor this behavior at home.

Good Citizenship Badge As a part of McFerran Preparatory Academy, your child will have the opportunity to earn a Good Citizenship Badge. We have chosen ways that a student may demonstrate good citizenship in the school environment. We encourage you to foster good citizenship at home and in the neighborhood.

classmates and will participate when working in a group learning activity.

• The Academy student will show respect for other classes by walking quietly through the halls.

• The Academy student will follow safety rules for walking in the hallways and stairwells. This means walking on the right side, in a straight single line; keeping hands and feet to oneself; and not sliding down the stair railing or hanging over it.

• The Academy student will not litter. He or she will pick up litter in the restrooms, in hallways, in the classroom, and on school grounds.

• The Academy student will be able to recite The Academy Oath, The Academy Rap, and The Pledge of Allegiance and sing “The Star Spangled Banner.”

Additional Permission toRelease Student Information

McFerran Preparatory Academy2016-17

Throughout the school year, there may be situations where your child needs to be picked up early from school and you, as their guardian, may not be able to do so. On the back of the Enrollment Form is space (under Medical and Emergency Information) where you can provide the names of two emergency contacts. However, for nonemergency situations, it would be helpful if you would provide for us a list of any additional adults whom you might send to school to pick up your child for various reasons (e.g., doctor appointments, he or she is sick and you can’t leave work). This will help us alleviate issues where someone is sent to the school to pick up a child and is unable to do so since his or her name is not listed on the Enrollment Form.

(Note: A picture ID is required when picking up a child.)

Jefferson County Public SchoolsPhoto/Videotape Release Form

Throughout the school year, there may be times when Jefferson County Public Schools (JCPS) staff, the media, or other organizations, with the approval of the school principal, may take photographs of students, audio/videotape students, or interview students for school-related stories in a way that would individually identify a specific student. Those photographs and/or videotaped images or interviews may appear in district/school publications, in district/school video productions, on the district/school website, on district-authorized social networking sites (such as Facebook or Twitter), in the news media, or in other organizations’ school-related stories or articles. To authorize your child’s photograph and/or videotaped image or interview to be used for these purposes, please complete this form and return it to your child’s school.

I hereby grant unto the Jefferson County Public Schools permission to use my child’s photograph and/or videotaped image or interview for the purposes mentioned above. I understand and agree that JCPS may use these photos and/or videotaped images or interviews in subsequent school years unless I revoke this authorization by notifying the school principal in writing. I further grant unto the Jefferson County Public Schools permission to permit my child to be photographed, audio/videotaped, or interviewed by the news media or other organizations for school-related stories or articles.

Volunteers are Needed! Contact Ms. O’Hair if You Are Interested!

Dear Parents/Guardians:

Your student has the opportunity to check out a book from the school library. We have spent some time discussing the responsibility that comes with checking out a book and/or media and we hope that parents/guardians will share in the responsibility in the following ways:

Read the book with your student and discuss the story elements with your student (Plot, setting, characters, illustrations, etc.)

Help your student determine a safe place at home where their library books/media will not be lost or damaged.

Remind students to keep liquids, food, crayons/markers, and pets away from library materials.

If accidently damaged, send all materials back to the library to be repaired– do not tape or try to repair them at home. In addition, please alert the library media specialist of the damage.

Remember that any library materials that are lost, stolen, or damaged beyond repair must be paid for before your student may check out other materials.

IF YOU MOVE TO A NEW SCHOOL, PLEASE GIVE ANY MCFERRAN LIBRARY MATERIALS YOU MAY STILL HAVE TO YOUR NEW SCHOOL’S LIBRARY MEDIA SPECIALIST AND THEY WILL RETURN IT TO US.

McFerran Preparatory Academy School Library Media Center Library Check Out

Students in second through fifth grades may check out two items at a time from the library media cen-ter. First grade students may check out one item at a time.

The checkout period for all items is one week, however, items can be renewed.

If you have any questions, please feel free to contact me at school and I will get back with you as soon as possible.

Thank you for all you do!

Ms. O’Hair, Library Media Specialist Phone: 502-485-8297 E-Mail: [email protected]

Please detach and return to your child’s homeroom teacher.

I understand the procedures and accept responsibility for any library book or media I check out.

Buses—If your child is a bus rider, we prefer for him or her to ride the bus beginning the first day of school. It helps the bus driver to establish stops and also enables us to do a better job of getting students on the correct buses.

Bus Referrals—If a student violates the bus rules, the bus driver may issue a written bus referral to the principal/assistant princi-pal/security. The principal/assistant principal/security will discuss the violation with the student and send home a copy of the referral to the parent/guardian. It is to be signed by the parent/guardian and returned to the prin-cipal/assistant principal/security. Fighting on the bus will result in an immediate bus suspension from one to five days. Other bus violations will result in consequences as deemed appropriate by the principal/assistant principal.

Bus Changes—If the student is going home a different way than he or she normally goes home (e.g., usually a bus rider and will be a car rider, or going on a different bus), the student must provide a note from a parent each day a change occurs advising the bus driver, teacher, and office of the change. If a student is going home with another student, the oth-er student must bring a permission note from his or her parent also. Please do not send a note listing several days’ changes. Teachers and/or the office staff cannot be expected to keep an accurate record for prior notes written.

Car Riders—Car riders will be dismissed from the front of the building. Each family will be assigned a car rider tag and number. These tags should be hung on the rearview mirror and will be necessary in order to pick up car riders. All students, even those who are typically bus riders or walkers, will receive car tags. Parents may give a car tag to anyone they want to be able to pick up their child from school. Anyone picking up a child other than those in his or her own family must also have a tag bearing the number of that child’s family. If parents or anyone else come through the car rider line without a car tag to pick up a child, they will be directed to a parking spot. They will then need to go into the school office and present photo identification. They must be listed as a contact on that student’s enroll-ment form for the student to be released to them.

CEP—Students who stay for CEP will go to the cafeteria.

Daycare Riders—Students who go to daycare (other than Keystone Academy) will be supervised by staff members and dismissed into daycare vans on the right side of the school.

Walkers—Walkers and students who go to Keystone Academy will be dismissed from the gym. Walkers will exit from the back of the school building on 13th Street. Students should remain on the sidewalk while exiting school grounds. Kindergarten and first-grade students must be picked up by an adult.

Late Pickup—Students not picked up by 4:10 p.m. will be taken to the office. Parents/Guardians will need to come in and sign the Late Pickup Binder. The school attendance clerk will monitor the binder. After three occurrences, the principal/assistant principal and/or social worker will be notified and a meeting with the parent scheduled.

TransportationDiscipline Code The Jefferson County Public Schools (JCPS) Code of Acceptable Behavior and Discipline has been prepared and adopted by the Jefferson County Board of Educa-tion (JCBE). Each student will be given a copy. Parents should read the booklet and share the information with their child.

Discipline GuidelinesEducation is the main goal at McFerran. Education prepares each student for the challenges and demands of the future and is a means of helping the student become more self-directed and better self-motivat-ed. The McFerran Preparatory Academy Program stresses self-discipline and sup-ports the concept that achievement itself is the major award. In order to ensure a safe learning environment and productive behavior, we have high behavioral expec-tations for all McFerran students.

Academy DisciplineProceduresA Pupil Disciplinary Referral Form will be filled out by the teacher or staff member and will be sent to the principal/assistant principal/counselor or in-school security personnel, who will then deter-mine appropriate consequences. A copy of the form will be sent home with the student. The form will have to be signed and returned by a parent/guardian.

The JCPS Code of Acceptable Behavior and Discipline will be consulted based on the severity of the offense. An immediate behavior referral will result from any violation of this code.

Discipline

Severe WeatherInclement weather may cause the closing of schools, delayed openings, or early dismissals. As soon as these decisions are made, they will be announced on the area radio and television stations and on the JCPS website at www.jefferson .kyschools.us. Please do not call the school. We receive our information from the radio and television stations.

If a storm should occur during school hours, rest assured that all students will be adequately cared for by capable personnel of the school and will not be released until it is safe to do so. Since weather conditions are unpredict-able and school closing may happen suddenly, develop a family plan that clearly designates what to do or where to go if parents are not at home. This is vital to your child’s safety.

28

Personal Property—Please ensure that your child does not bring valuables to school. This includes, but is not limit-ed to, cell phones, iPods, tablets, MP3 players, and personal gaming systems. Students are responsible for their money and property while they are at school. Stu-dents should bring only personal property to school that is not hazardous, dangerous, or disruptive. The school staff will retain any items of this nature. Parents may pick up the toys/items after consulting with the principal/assistant principal or counselor.

Telecommunication Devices (Cell Phones)—Students in the JCPS District may not use, activate, and/or display a telecommunication device on school property during the course of the school day. The tele-communication device will be confiscated by an administrator and may only be returned to the parent/guardian. If a child must bring a cell phone or other electronic device to school, it must be kept in a locker during school hours.

Tobacco-Free Schools—Tobacco usage is completely prohibited in JCPS facilities, in personal vehicles, and on school grounds. This policy is for every-one—staff, parents, students, and visitors. McFerran is a tobacco-free environment.

Birthdays—Class treats are welcome. However, these need to be individual treats—cookies, cupcakes, or Rice Krispie treats, for example. Please contact your child’s teacher to make arrangements.

Bookstore—Red McFerran T-shirts and red ties may be purchased in the school bookstore.

Classroom Visitation—Classroom visitation is permitted when arranged 24 hours in advance through the school office and the teacher. The teacher will schedule the most advantageous time for you and your child. Teachers are involved in teaching your child, and we want to minimize any disruption in the educa-tional process. All visitors must sign in at the school office and wear a nametag for identification and security purposes. Nametags are available at the reception-ist’s desk. Please check with your child’s teacher before videotaping or photograph-ing in the classroom.

Conferences—Parent/Teacher communi-cation is vital to support student success. A minimum of one face-to-face confer-ence with your child’s teacher is expected. There are scheduled districtwide Par-ent-Teacher Conference Days on the JCPS school calendar. Teachers will request parent conferences throughout the school year as needed. Parents may request a conference by calling the school office or by sending a note to the teacher so that a date and time can be arranged. Please check the JCPS school calendar for con-ference dates for the upcoming year.

Field Trips—Field trips are an integral part of the educational process. Teach-ers will schedule field trips to support and enrich their curriculum. The fee regulations of some attractions prevent refunds from being made when a student is absent on the day of a trip. No student, however, will be denied the opportunity to take a trip because of a lack of finan-cial resources.

Gum—According to JCPS policy, chew-ing gum is not permitted.

Homework—Students are given required homework Monday through Thursday. JCPS set a policy for the length of home-work to be required at each grade/level. The teachers are aware of this policy and will assign homework accordingly. Students are responsible for all materials needed to complete homework assign-ments.

Illness and Accidents—Should your child become ill or have an accident during school hours, facilities are avail-able for first aid and temporary care until parents can be contacted. Be sure the of-fice has an emergency phone number on file, should you not be at home during the day. If your child has been ill and needs to be given medication during school hours, please contact our front office staff for the JCPS procedures/forms on medi-cation given at school.

Students should be kept at home until they are free from fever, diarrhea, or vomiting for 24 hours.
