Should be given to parent/guardian of any player suffering an illness or injury requiring the care of a doctor or trip to the ER.

Must be completed by parent/guardian before player returns to play.

The Participation Release Form asks parents to confirm the player is cleared to play and that a medical release has been obtained in States that mandate it for possible concussions.

Concussions• Occur when a blow to the head or body

causes the rapid movement of the brain inside the skull.

• Result of fall, head-to-head collision, striking the goalpost /ball or even a mild bump/blow.

• Can occur without loss of consciousness.
