Welcome from your East Primary Care Network
(PCN) Clinical Director
I wanted to take this opportunity to introduce
myself to our potential partners and residents
across the East Neighbourhood of Doncaster.
I arrived in Britain from Kenya in 1977 at 16 years
of age to study for my A Levels. From here I
attended Sheffield University graduating in 1984
and specialising in Orthopaedic and Trauma
medicine. I then decided to change direction to
General Practice and took up Locum roles in
London before becoming a GP Partner in the areas
of Northwick Park and Harrow.
From here I started work at Hatfield Health Centre
on the 1 April 1994 and have worked here for the
past 25 years.
As you can imagine I have seen lots of changes over
the years in the delivery of health care including
the 1990 GP Contract and the subsequent changes
to the Health and Social Care Act.
I have seen the inception of PCG’s, PCT’s and more
recently CCG’s.
Some of you may also recognise me as the Chair of
the Local Medical Council.
Very recently I was extremely privileged to be
appointed as the Clinical Director for the East
Primary Care Network.
You can read more about PCN’s and some of the
work being undertaken across the East in this
newsletter.
As Clinical Director my personal view is that this is
a really exciting opportunity for all of us to look at a
new ‘system wide’ model for health and social
care. No longer are we focussing on ill health and
symptoms but looking more towards promoting
the wellbeing of our local communities and
working with partners to ensure we deliver a
whole-systems approach to supporting them. We
are looking to building stronger communities in
which to do this effectively. Working across a PCN
means we can direct time and resources and
provide the most appropriate support to those
who need it the most.
Since my appointment on the 1 July I have been
overwhelmed by the level of knowledge and
expertise working across partners on our own
doorstep and within our own neighbourhood and
PCN and I am really looking forward to working
with you more over the coming months to explore
opportunities for ‘doing things differently …
together’.

An update on PCN’s …. In line with the national direction of travel, all Doncaster GP practices are part of a primary care network (PCN), set to further improve access, availability and quality of primary care services.
The East PCN is one of five networks (along with North, South, central and 4Doncaster) that exist in Doncaster, providing services for a similar numbers of patients: PCN’s are a key feature of the NHS Long Term Plan and will build to support the development of proactive, personalised, coordinated and more integrated health and social care services. Health and care professionals commonly describe this as a change from reactively providing appointments to proactively caring for the people and communities they serve. Patients are not likely to notice much of a difference at first to the care they receive as care in Doncaster is already provided in a way that aligns with the formation of PCN’s. A key feature of practices coming together as part of a primary care network means that every patient registered at those practices will now be able to access appointments early in the morning, later in the evening, as well as making use of those practices that open at weekends. It doesn’t mean that every single practice will open earlier and close later, it means that a number of practices within each network will be able to offer these appointments to all patients across the network.
The formation of the five networks in Doncaster also brings considerable funding to primary care services across the borough. Each PCN in Doncaster now has the ability to employ a Clinical Pharmacist. They will support all practices in their network and are highly qualified experts in medicines that can help people in a range of different ways. This includes carrying out structured medication reviews for patients with on-going health problems and improving patient safety, outcomes and value through a person-centred approach. The introduction of Clinical Pharmacists in each network will support GPs to focus their skills where they are most needed, such as diagnosing and treating patients with more complex conditions. This will ultimately help GPs to manage the demands on their time. PCN’s will continue to develop at pace and over the next four years, a number of additional roles will be added to each network. These include:
First contact Physiotherapists Physician Associates and Community paramedics.
An Update from the East Primary Care Network (PCN) Additional Workforce for the PCN Within the last month 6 Social Prescribing Link Workers (SLPWs) have now been appointed to work across the 5 Primary Care Networks. They will be hosted by South Yorkshire Housing Association, who already provide the core Social Prescribing service to Doncaster. The SPLW role will be to take patient referrals from PCN member practices and support their health and wellbeing through individual home-based consultations, during which the patients will be able to co-design their own personalised solutions, often using services within the voluntary & community sector.

Developing Primary Care Networks into the next phase As we move into the next phase of development of PCN’s we will be hosting a GP workshop on the 19
November. The purpose of the event is to provide our GP colleagues with the opportunity to identify potential partners to work with across the PCN going forward. Hosts for the session will include:
Rotherham Doncaster and South Humber NHS Foundation Trust
Communities and Wellbeing Teams Family Hubs Voluntary Community and Faith Community Pharmacy
There will also be presentations around population health management and from Your Life Doncaster, who may also be pivotal in the further development of PCN’s. Keep posted for an update in next month’s edition. Other projects …. Other PCN activities have included:

Commenced work around Cancer Care Reviews and Cancer Recovery pathways across the East PCN (in partnership with DBTHFT)

Offer of ‘bitesize’ Mental Health Awareness training across the East PCN
Integrated Neighbourhood Working Some of our practices are really starting to embrace the mutual benefits of integrated neighbourhood working. Some examples have included:
South Yorkshire Fire and Rescue Safe and Well Scheme – you may remember this from last month’s newsletter. Our Thorne Moor Medical Practice have now signed up to the scheme and in return will receive FREE Fire Safety Training and Fire Marshall training for their practice – win, win! Mark Harrison who is based at Thorne Fire Station is really keen to engage local communities across the East. Young Carers Toolkit - I’m sure you are aware that local authorities and their partners have a duty to identify young carers, to assess their caring responsibilities and offer support in order to reduce the impact of their caring role. Through working with partners (including Primary Care and GP’s) DMBC have managed to increase the number of young carers known to them to 182 over the last year. However there are an estimated 720 young carers who live in Doncaster so there is still some work to do. To support partners in identifying and supporting young carers a toolkit has been developed, which includes information such as:
Who is a young carer? How to identify a young carer Young carers assessment How you can help Information about the Young carers service
You may also not be aware of the Young Carers card enabling a young carer to identify themselves as a carer to professionals such as teachers or health workers. The card has a traffic light image on the rear of the card this allows a young person to tell someone how they are feeling without words, if they do not feel like talking at that time or are unable to explain what the problem is they can simply point at the red traffic light. This will then
allow the young person and the professional to talk about what may be upsetting them at a more appropriate time. You may wish to identify a Young Carers champion to provide a specific point of contact who can link in directly to the Young Carers service and a young carers practitioner for further support. year to bring organisations across the UK together
to help carers in their local community know their
rights and find out how to get the help and support
they are entitled to. This year’s Carers Rights Day
will be held on 21 November and the theme is
‘Helping You Find Your Way’. You may wish to
promote Carers Rights Day in your own group or
organisation by:

Making Space Pamper Session for Carers –
Bitesize Mental Health Awareness Programme – Some of our practices have signed up for the full 2-day Mental Health First Aid course in December, kindly facilitated by RDaSH. However, RDaSH can now also offer a ‘bitesize’ Mental Health Awareness Programme, specifically aimed at businesses and health professionals. The session involves 1 ½ hours of presentation to a group of up to 30-40 people and includes:
Description of two most common mental health conditions - Anxiety and Depression
What it is and how it can manifest itself How it affects people How to manage it Available help Brief description of IAPT Service and the
help that we can offer
